This video highlights one of my medical mission trips to the Dominican Republic years ago. My team and I were lucky enough to perform dozens of sight-restoring Cataract Surgeries for patients.     Thank you for such an informative video. Helped me make my final decision on which IOL to get. Going for LAL+. I had previous LASIk 24 years ago and I am severely myopic with some astigmatism, with refraction of -9.75 prior to LASIk. Never became 20/20 even after LASIK and one enhancement. Vision still progressively got worse over the years, -4.5 both eyes now. Then this year, right eye jumped to -9.5 😫 Found out I had rapidly enlarging cataracts in right eye, also have small cataracts in left eye. I am 54 years old. My ophthalmologist first thought it was corneal ectasia causing the problem. After several tests and eye measurements, decided it was the cataracts after all. Waiting for my surgery to get scheduled. I’ve worn glasses pretty much all my life so i don’t mind having to wear reading glasses, but it would be nice to have great distance vision without glasses for once 😊 Thank goodness for LAL+, I think there is hope for my eyes after all 🙏🏼

    Can’t understand, with such experience, why would patient like to have vivity in one eye at first place? There was no single benefit of vivity over Clearview3 listed in the video but still recommendation looked like mixing the two lenses! But why? Seems more like promotion of Clearview3.

    Excelllent video, Dr. Krad! I have been searchhing pubmed and have not come across any case reports of opacification with CV3(SBL3), most described reports are associated with the mplus lens of Lentis/oculentis. I like your explantation technique!

    If cataracts are not reversible, and you are developing them, why would you WAIT till you're older, and less able to recover and heal, compared to younger and more resilient? It's like needed a new head gasket on your car and trying to decide whether to put it off. You need it, it won't get better, so, the sooner you attend to it the sooner the problem will be fixed.

    Thanks for the informative video. I just got back from an evaluation. My Dr. at first didn't recommend LAL because he said my pupil wasn't big enough after dilating. Then he rechecked it and said it was 6.1, which passes the minimum. Will that be ok. Should I be concerned. Thanks

    I just had the vivity lens but in one eye. The other eye can see better up close do now they work together but I’m still struggling to see it’s blurry at distance and straining my eyes. And yet when the other eye gets done I want to be able to not where reading glasses so hopefully they can do a different lens? I’m only 4 days post op. I had laser back in 2008 and since Covid vaccine I ended up needing glasses and then two years after that they discovered cataracts. So the other eye is just starting but I’m afraid I will still need glasses for distance and up close down the road

    Are lenses off the shelf and chosen by your surgeon for your personal preferences or can they be set up and made for a patients individual needs? I was looking at probably extended. Nighttime truck driver Wanting to keep legal without glasses . Also Getting as close as possible to do other closer tasks without glasses. Would rather compromise and wear glasses for general reading and doing paperwork which is inside with light and out of the rain . But if I can bring extended down to legal driving level and do some reading all the better.

    In terms of near vision, does ClearView 3, PanOptix, or Odyssey have the best near vision when targeting plano? What are their diopters of continuous visual range? It is my understanding that ClearView 3 has less dysphotopsias but does it sacrifice on near vision vs PanOptix or Odyssey? And does it have dead zones? And is the Odyssey just slightly better than PanOptix for contrast, but same amount of halos? And the B&L Envy was just approved in Canada, not sure if it’s just another PanOptix or Odyssey, but it says it has 4 diopters of continuous visual range. Thank you so much for your information!

    Had my right eye done yesterday with no sedation. It went well and I will do the other in a couple of weeks the same way. For anyone wondering it is without pain with only numbing drops.

    Great case. This is why I don't do RLE anymore. Capsules contract and outcomes are not always predictable. Contraction seems worse in younger patients. I probably would have used an indirect goniolens and tried to drive the eye, pushing into different gaze positions to help look farther into the sulcus and capsule. I love that you give the original surgeon credit and not bash them for an undesirable outcome.

      I came to the comments section to see if any experienced surgeons would have tried to use a gonio lens. I have not encountered a situation like this before, but while watching this video, I thought I might have tried a gonio lens to visualize the CTR. Great case, thanks for sharing.

    Cataract surgery is not benign people. We too often state that this is going to be easy and you’ll see better than ever. Until you end up with multiple surgeries and a CTR in an unknown location. Crazy. Good work on helping her.

    Great video as always. One method to help in your hunt for the CTR would be to use a gonio prism. Can use a bit of visco to tent up the iris a bit and give some visualization of the sulcus/peripheral bag. love your videos man
