Cataract Surgery for Wet Macular Degeneration | Things to Consider

  • How do I approach Cataract Surgery for patients with Wet Macular degeneration, or Wet AMD?
    This patient has wet macular degeneration which required multiple monthly intravitreal injections for treatment. She’s also diabetic & hyperopic.
    After multiple intravitreal injections for macular degeneration, her vision improved from CF to 20/300. That’s as good as her vision would get and glasses couldn’t make her vision any better. The patient was referred to me due to her visually significant cataract. Her cataract is fairly dense and this patient would be thrilled at even a modest improvement in vision.
    For patients with wet macular degeneration who have a vision potential of less than 20/40 vision, what surgical technique and lens implants would you consider? My approach is to keep it very simple. Manual cataract surgery and a monofocal lens implant. Could one do laser-assisted cataract surgery? Could one implant an LAL (Light Adjustable Lens)? Could one implant a Monofocal Toric? If a patient would be unlikely to tell the difference, I typically put in basic monfocals and perform the surgery manually. The advantage of toric lenses and LALs and laser are that they may be more likely to get a patient improved fine vision. If a patient doesn't have the potential of fine vision, then implanting the technology would probably not benefit them. That’s just my personal opinion. What do you guys think?
