  • NOTE FROM TED: This talk, which was filmed at a TEDx event, contains assertions about treating depression based on the speaker's own theory. While some viewers might find the prescriptions to be helpful, please do not look to this talk for medical advice. TEDx events are independently organized by volunteers. The guidelines we give TEDx organizers are described in more detail here: storage.ted.com/tedx/manuals/t...
    Dr. Heitler discusses a treatment for situational depression (even when we don't know it is situational!) WITHOUT psychopharmacology. Instead, she offers three prescriptions without pills, outlining these prescriptions using an example from her practice. She shows how investigating what makes us angry or upset can help us to re-empower ourselves, and our mental health.
    Susan Heitler, Ph.D., a Harvard and NYU-educated clinical psychologist, writes a Psychology Today blog that has received over 7.5 million reads.
    Dr. Heitler first earned recognition when her book From Conflict to Resolution brought understandings of collaborative conflict resolution from the world of legal and business mediation to the practice of psychotherapy.
    Her Power of Two and Power of Two Workbook- published in six foreign language editions-plus her interactive marriage education website PowerOfTwoMarriage.com, teach the communication skills for marriage success.
    Dr. Heitler’s most recent book and website combo, Prescriptions Without Pills, offers self-help versions of the techniques she uses in her clinical practice-techniques for relieving depression, anger, and anxiety plus strategies for sustaining well-being.

    When people are depressed, they don't give up what they want, they don't want anything!!!! They can't even remember what they wanted when they were better.

      Yes, the more deep the depression, the less ability to have any positive thoughts. That is why the visualization includes "pumping up." When you see yourself as larger, your energy levels increase. It then becomes more possible both to think about what you want and to create potential solutions.

    • @drheitler

      That is why the visualization works...with your eyes closed you see beyond conscious thoughts to understandings that you would otherwise not have been able to access.

      Pablo, For relief reduce negative thoughts. Your breathing is closely related to the brain [mind] and gives good relief from stress-anxiety. To relax sit on a chair or lie down, neck straight, eyes closed and observe the sensations of your incoming--outgoing breath at the entrance of the nostrils for 5-10-15 minutes or more. Don’t fight your thoughts. With daily practice the mind will relax. No deep breathing needed. Do the practice without any expectations. When taking a walk, at office, in school, when reading, before sleep etc observe your breath sensations. Like me, make this a lifetime daily habit to have a good life. Reduce negative social media and avoid constipation as it affects the mind instantly. Best wishes--Counsellor.

Reduce watching negative social media and avoid constipation as it affects the mind. Read my msg and do the practice regularly without expectations--let change happen by itself. . Best wishes. I had major depression during the end of last year into the start of this year. I lost interest in in the gym, in painting, in reading. I don’t know when it started creeping in. I couldn’t stop crying, if felt like a massive hole in my chest, nothing made it go away, and the thoughts I had were horrible. I was suicidal. I made a folder and put bank statements, car documents, passwords and pin codes, letters to my niece, nephew, siblings and parents, and an informal will because I wanted everything to be organised for when I died. I knew how I was going to do it.
    Medication saved my life. I needed it to be able to feel capable of getting to the root of where those feelings came l from. If I’d went to the doctor and he told me everything this woman is saying then I probs my wouldn’t be here typing this right now. Sometimes a person needs medication to be able to see things clear headed.
    Depression isn’t feeling down, it goes much deeper than that, and it’s bloody hard work pulling yourself out of it and look at things objectively (it took me nearly a year). The problem was, people would tell me that there were no issues but I didn’t believe it because of all the horrible things I was telling my self. Trying to convince yourself that your inner voice lies is so difficult, and when you reach a point where you can distinguish between who you are and what the depressive part of you is it’s easy to say you need to change your mindset.
    Buts it’s bloody hard, and you go over it and over it until it sticks. Sometimes though medication is the only way out of a hole. My doctor made sure to tell me that it wasn’t a long term solution, I needed to help myself as well, but without those tablets I didn’t have fighting chance.

      Thank you for sharing this vivid description of a major depressive episode. I completely agree that for this kind of depression, and especially with suicidal thoughts, medication is a vital treatment component.
      So glad to hear that you did succeed in emerging out of that dark depressed place. Your story hopefully will inspire others.

    • @michcookies

      Susan Heitler thank you, I appreciate your reply. When researching the medication I was on when I started taking it there were a lot of people who had been on it for a year or more, and I was scared I would end up dependant on it. I’d tried to do positive thinking mantras over the years but they didn’t work, and I now think it’s because I had to learn *how* to do it rather than learn a phrase. Along with that I learned *how* to get to the root of where my negative thinking came from. I often think if I hadn’t have had my breakdown I wouldn’t have been forced to deal with it properly, and so I’d probably still be stuck in a loop of negative thinking, so I needed to feel all those things before realising I needed to help myself. (Funnily enough, it was a RUclips video that helped the most that popped up in my recommendations, I didn’t even look for it. I guess the phrase ‘when the student is ready the teacher will appear’ has some truth in it).

    • @drheitler
      @drheitler 3 года назад +29

      @@michcookies You make a vitally important point here. As multiple research studies have validated, medications work best in conjunction with therapy. Otherwise, people are less likely to learn from their depression experience and therefore are at higher risk for becoming dependent on the medication.
      "Therapy" can mean all kinds of options, not just traditional talk therapy. Visualization treatments like in this TEDx talk, Emotion Code, acupoint tapping, and other of the newer energy treatments, for instance, can be similarly effective. The key is BOTH, to utilize both medication and other therapies, especially those that help you to learn and grow.

    This is similar to CBASP therapy. It teaches you to get what you want in situations. The problem with severe depression is you dont know what you want. You can't pinpoint anything. You don't care. You want it to end so you can get living again. There's no simple logic to it. But this is probably helpful if you're a bit fed up.

    • @drheitler
      @drheitler 6 лет назад +144

      You are exactly right that in severe depression you don't know what you want and can't pinpoint anything. That's why people need to close their eyes to do this visualization. With eyes closed, your mind can access deeper self-knowledge.

    The real gem in this talk is “depression is a disorder of power” this is exactly how I view it. It’s not simply a chemical thing. It’s relational. It’s about the experience of disempowerment.

      @selliott7043 2 года назад +16

      Is it helpful to the depressive to say it’s simply a matter of empowerment when all the mechanisms of imagination are beyond the depressive mind? From where does empowerment stem? It can not be some external manifestation like work place assertion, it requires a healthy functioning mind which a depressive simply does not have and so it goes full circle; chicken and eggs.
      To break the chain we need a reset button. I have found my creativity and now understand that I have been depressed since the age of 7, this being a defense mechanism; couldn’t put two words together at times! Couldn’t connect words with concepts (bit like Donald Trump). Lights are on but nobody’s home! Oh boy what a journey! Thanks Donald! And I mean it; what a shining light of how not to be a human being! Depression can be a basic loss of our Sense of Humanity; the depressive sees it, Trump is incapable of seeing it!

    I'm currently writing a book on Depression. To begin with i have had everything available prescribed nothing ever worked. I even ended up resulting to Alcohol which made everything worse. The dark days of not wanting to get out the bed and sitting in those cold showers. Anxiety and Panic Attacks didn't help my situation either and ended up in the hospital multiple times. Nothing was missing in my life. This dark cloud of Depression is something many of us are scared to talk about and I'm glad more people are becoming open to it. And to be honest at times it's even hard to speak about it. Medication works if used correctly as a boost for the short term. But long term I strongly believe we and I say "we" because we are a community that together we can help each other learn ways of coping with the dark cloud. If you're reading this just know you're not alone,breath and use the power of your mind. Little by little things get better. May God bless you all. This was really hard for me to even write. But slowly I'm trying. And there's no judgment on Depression it happens to the best of us. Much love and blessings to everyone. 😊🙏💯

I have a great career, even in these uncertain times, I have a home and food and family and all kinds of wonderful things. Yet this weight of depression walks with me still at 46. I’m on a mild daily ad, I’ve been talking to therapists for 20 years, tried cbt methods, exercise, healthy eating, light therapy, etc and yet there are days that are soul crushing. The big events in life don’t seem to phase me, it’s the day to day living with this weight of depression. It’s a vague sense of doom, why if everything in my life is peaches do I wake up and live every day with this doom, lethargy, spontaneous crying. I can be laughing with friends at dinner and feel this awful sinking sensation. The best I can do is consider this a journey, be grateful for it all and live in the now.

      but i see your point, if you in the worst depressive state your problem isn't really bump in a road, its yourself, constant negative thoughts, inability to do the easiest tasks and so on. but i still think her advice is good.

      When your depression is reactive, even quite serious depression can be helped with this visualization. If the depression is biochemical in origin, by contrast, you will find that in step two when you "pump up," you cannot sustain the larger size. That's one way, in fact, to test if the depression is, in fact, reactive or organic, and also whether it will be important to go ahead and take medication to relieve it.

    I honestly believe that there are at least two kinds of depression.
    1.Circumstance or Situational depression that is cause by a circumstance in your life.
    A loss for instance.
    2. Clinical depression that is triggered by changes in brain chemicals.
    Depression is not the same as sadness.
    Sadness is an emotion.
    From my experience, clinical depression shows as an absence of emotion.
    I recall one morning some years ago.
    I dressed as a matter of habit...put on my coat as if to go out.
    As I was about to open my front door, I realized that I had no idea where I intended to go or why.
    I closed the door and stood in the room with my coat on and my car keys in my hand and ran over the possible places I might go or things I might do.
    To each option I said to myself, "Why Bother?"
    Then, I sat down ,still with my coat on and keys in hand and I just sat there staring out into the void.
    I couldn't generate enough motive to even stand up again.
    Why Bother ?
    Those two words sum up for me, the essence of clinical depression.
    You see no light at the end of the tunnel and, frankly, you don't really care!!
    Prozac brought me out of it.

    • @drheitler
      @drheitler 6 лет назад +2

      I agree with you about the two different causes, biochemical and situational. Hormonal changes for instance can trigger depression. And any kind of loss or inability or hopelessness about getting something you want can trigger depression also.
      After either trigger, the intensity of the depression can vary. Hormonal difficulties for instance can trigger mild deprression, or severe. Likewise, situations can trigger either mild, moderate or severe depression. The most severe depression I ever worked with, for instance, was a woman whose husband insisted that they move to another and far-away city. When they brought her to my office, she sat in fetal position and couldn't move or talk.
      When the depression is hormonal or otherwise caused by biochemical changes, it's important to diagnose the specific triggers.
      Similarly, when the trigger is situational, it's important also to address the triggers.
      In both cases, and especially if the depression is long-lasting and/or where there is family history of depressive tendencies, it can be, or may not be, helpful to add medication.
      The vast majority of depressive reactions can be treated without medication however.

    • @drheitler
      @drheitler 6 лет назад

      A very clarifying description of the two depressive states. At the same time, for some people a specific situation triggers the extreme kind of response you so beautifully describe. So sometimes the phenomenon is both situational and chemical.

    Dr. Susan Heitler is providing great insight in this video. I realize from the comments below that this video may not resonate for people who are struggling with severe depression, having difficulty functioning and are experience profound levels of pain. Based on Dr. Susan Heitler's other teachings, she does adamantly recommend people get help when they are experiencing severe depression, by the way of a therapist and psychiatrist. Once someone gets back to some level of functioning, they will still be faced with how to deal with their life challenges. Over time, if someone practices her 3 Step Prescription, I believe they can transform chronic tendencies towards depression into empowerment and vitality. Albeit, it is super important to get therapeutic support to put these new strategies into place. What Dr. Heitler is suggesting in this video is simple, but powerful. Often times, complex teachings need to be reduced to simple terms, so that they are more digestible and accessible. If you are someone you know if struggling with significant depression, I encourage getting help, so that there is support, safety, empathy, and compassion. From a stable foundation, Dr. Susan Heitler recommendations can help shift some major patterns of functioning.

    Loved the speech. I just went thru a situation where we got this amazing house and couldn’t understand why I wasn’t happy... everybody was telling me I should be!!! The more I was told I should be happy and grateful the more miserable I felt... It just didn’t feel like a home.... Now I know why... it wasn’t a house I really chose, it was almost imposed by my husband and by the expectations of society... and, instead of happy, now I know, it just made me feel small. It’s on the market and we got this lovely and simple half the size house with a gorgeous garden. The first night I felt more at home than at the other house in a year... I wish I had heard this long ago... God bless!

    Depression itself complicates bad moods into a downward spiral, it is highly intelligent and tricky that kinda get you cornered no matter what you try to do. But depression is a part of you, which means you are actually fighting against yourself. The intelligence depression displays by caging you in, is the same intelligence YOU own by trying to avoid future pain and suffering. So if you can get into depression, you can get out of it for sure. Knowing how to not let in negative self talk and knowing that depression is a natural defensive mechanism is important, because only then can you detach yourself from it. Depression is not you, it is a symptom of fear. It does not speak for you nor does it represent who you are as a person.

    Thought I would put my two pennies worth into this subject.
    The speaker first and foremost means well, she is a qualified psychiatrist and is giving the advice gathered throughout her career based upon her experiences.
    In an ideal world it would be absolutely fantastic to follow this advice and be well.
    Unfortunately it just doesn't work like that for a large part of the populace, including myself. I am also of the belief that a lot of mental illnesses are passed on genetically, predominantly depression & schizophrenia.
    Point being no, I don't want to be on antidepressants. But at the same time they have probably saved my life, and to be honest I wouldn't wish depression on my worst enemy.
    The best cure for depression that I have found is excercise, self reflection and more exercise, but have a goal.

    • @drheitler
      @drheitler 2 года назад +4

      People who are biologically and/or genetically depression-prone, I agree, are likely to find medication helpful and even essential. At the same time, the treatments are both-and rather than either-or.
      I have found that many clients who are successfully taking anti-depressant medication still benefit from the understanding of depression and the 3-P's visualization I explain in this talk. Life events and frustrations can knock anyone down, including people on mediation. Best to have good pick-me-ups, including exercise, sunshine, energizing social interactions, and visualizations.

      Susan Heitler. You are offering one type of approach that is about "doing something" constructive. Many professionals don't really seem to offer a formula to try. Maybe it will help, and if not, on to something else. I do not expect an answer but if I were to receive one I would be very grateful. I spent about 35 sessions with a therapist who helped me but I could not seem to get her to help me deal with my underlying anger! My anger stems from what feels like betrayal, even divine persecution! (So you don't think I'm suffering delusions...lol. I have had many recent "events" that could lead to situational depression). I am no stranger to depression. However, my normal methods are not working. Pharmaceutical methods only leave me empty, unable to feel the good either! At 49 my home, the safe haven I had worked so hard to create for my family, burned to the ground. A year later I lost my sight in one eye due to a very rare condition, a stroke/bloodclot to the artery supplying my retina. A year later another TIA. Testing led to quickly scheduling a triple bypass. Six days before that surgury and in excruciating pain and two hospital misdiagnosis, it was finally determined I had another rare condition, a spinal abscess. Since I had been on blood thinners and the paralysis was becoming obvious, they needed to emergently proceed to surgery giving me a 30% chance of surviving surgurey but a 100% guarantee of permanent paralysis. T1-3 were cut in half and a significant amount of debridement in the area was done. But down the track I'm still on the drugs.
    And from this Tedtalk I think I realised I have stagnated. I did not look at how I could work on how I resolve issues when I am in a situation of oppression.

      nope, it truly is not. Some people just don't have power anymore. They are (I know it by myself) like ran out of fuel after running a couple of marathons without any break. Combined with a life situation which offers no further options for a soon outcome, it literally keeps you down. And in this position there are really much wrong decisions, wating to be made. And if you are additionally afraid of anything at anytime because there isnt much left to be made the right way, then you are very soon paralyzed and time keeps going faster than you can even imagine without any progress in your life. And the latter can even pull you down again and every little step forward, even when noticing it though, feels like still being stuck. So thus, depression is and can be very complicated without wanting it to be what it actually is like in most cases. I went the way to a therapist because I wanted a change and I'm still working on it, but I can really tell you that this, I admit good video, is not the only truth. There is MUCH more to be done to get out of the mud. And btw* I'm not taking any pills and nevertheless I often thought about taking them because for me...these kind of prescriptions without pills aren't enough by far.

      Billy Mugs, In life there are only 2 problems--mind and the body. To feel better reduce negative thoughts. When taking a walk, in the kitchen, at work, when reading, before sleep etc observe your breath sensations. Like me, make this a lifetime daily habit to have a better life. Morning sunlight walks, a simple prayer, reduce negative social media and avoid constipation as it affects the mind instantly. Best wishes--Counsellor.

In fact I was doing this not only for myself but for my 41 year old sister, she took her life 1 month ago and I'm done. I've tried for so long and for this to be the outcome is just too much. I totally agree with you that this technique is not for people suffering with a major depressive disorder. As I say in the opening of the talk, this technique is for the "common cold" version of depression. Probably I should have emphasized this point more: When a depression is Depression, like the difference between a common cold and pneumonia, the technique is not going to suffice.
      I'll say it again now. When a depression is clearly a) severe and b) of clear hereditary and biological origin, this technique is not going to lift the depression. It is not an appropriate technique for these situations.

    My twin died during my mother’s pregnancy, and I’ve learnt that the area of the brain that deals with abandonment etc, is still being developed; you could say unprotected, very vulnerable and raw. So when a close interaction with a twin suddenly stops, this can ‘bake in’ a unique deep psychological feature for that person. I experienced grief before I was even born, before my mother even found out.
    I don’t know where to begin to describe my upbringing, I can’t think of a word to label it; my feelings or attachment to most of my family has been destroyed, due to being a dysfunctional family, I don’t ‘love’ any of them, I’m not sure if I know what love means, there’s no love in my family. I’ve also never had luck with dating or finding a person I could care for. I feel like I’m refused the opportunity to love anyone, and the slightest hint of someone I’m interested in appreciating my efforts. I jump in too fast and scare them away. Because I’m literally dying to have a companion and to love someone.

    Awesome video about depression! Of late I've looked back on my life & the 1st huge mistake I made as a young woman was getting involved with a guy I didn't love & ended up marrying him. Then 4 decades later after 10 relocations and somewhere along the line realizing what kind of place I really wanted but not being able to move there. So I hit that bump this lady is speaking about many times in my life and been chronically depressed for many years.

    Its important to change thought patterns, to learn how to overcome mental obstacles and pick up better habits that exist in our minds. Thoughts are exactly that, an illusion that exists only in our minds but literally composes us and effects what we project outwards in every aspect of our lives and on to others. I am definitely an advocate for pills because sometimes the frustration can become so overwhelming that it keeps me stuck in my tracks unable to even make a change. For people with a higher severity of depression, it is completely necessary to use pills as an aid, but if you aren’t making any internal changes along side this your not really making any progress and essentially just numbing the way your negative thoughts hold you under its thumb. Thats what I realize for myself anyways after being on meds for a year and relying solely on that. What we allow to feed our minds can also be detrimental. Take a minute and think about the people you allow around you, the music you listen to, the things you read etc and how all of those things make you feel as well. Remember that healing is not a race and you are able to take as much time as you need to become stronger even if it takes 10,20,30 years as long as everyday your working to become better than yesterday ❤️

    Little meditation gives us an idea about where we are stuck, what's looking big, what we are interpreting which is not, what we are hearing which is not for us, we eventually figure out how our mind is playing with us, awareness is the first step to get out of depression.

Diet, being active, journaling through problems, routine; these are all things that WILL improve your life. And guess what? Depression IS common. All these comments are ridiculous..... don't act like you are the only person that has it bad. We are in fact PRONE to developing bouts of depression in our current society because we lack the balance in life that this doctor is proposing. Some people are biologically predisposed, but almost everyone will experience depression at some point in their life. That is a fact. I've been told that many times. Take every perspective you can instead of shooting down the first person that gives you something new to try.

We need all the reminders we can get because it’s more comfortable to externalize our emotions than to look at our internal thought choices. We don’t get enough reminders because so many speakers tell people what they want to hear. So this is valuable, as an alternative or a supplement to the chemical prescription.

      Honey Don't think that you're depressed think that you're having a bad time,and you just need to figure it out whats wrong am doing right now and how i need to solve it, trust me it doesn't take years to solve, it would probably take only few months, And stop being alone and overthink things, Do one thing
      1) Take a book, write dow wts happening to you
      2) Write what you should do to make it solve
      3) Keep doing things which makes you happy
      4) Don't ever dependent on other's to get happy
      5) Give permission to yourself to keep your mind in present
      6) At least for a moment you can come out of your bad time, and then it's a good sign
      7) Keep reminding that you're RARE.
      8) Don't be selfish, be happy for what you have right now, and be satisfied.
      9) Don't daydream, just know reality and face the world.
      10) Only if you know how to cope with world, nature of life, then there's no place of bad time in your life.
      Honey you're not depression you're just having a bad time.
      *And if you really think it's depression

      Finding the trigger for a depressed state is exactly the purpose of this exercise.
      At the same time, additional therapies may be needed for someone to have the energy to implement new insights from this exercise. This need becomes evident if someone does the three steps of the exercise and finds that they cannot hold the larger size that they envision in step two, Pump up.
      Emotion Code and Body Code are treatment methods that sometimes can help to identify further causes, and treatments, for a major depression such as you are describing. Google these terms (or search youtube) to learn more.
      Also, it's important with a significant depression to get a medical workup to be sure your body is not missing a chemical (e.g., potassium) needed for a normal emotional state.
      Lastly , in situations such as yours, it can for sure be worth giving medications a try, including ketamine if a combination of the techniques above plus traditional anti-depressant medication does not lift your energy level and your spirit.
      Wishing you all the best.

Wanting to be loved, wanting friends, judging myself way to much, changing of the seasons. The list goes on. One of my biggest changes is finally realising that I have to stop putting everyone else before myself and phoning or texting everyone first because honestly, stop phoning or texting and then you really find out who your true friends really are. Great talk, visualisation is incredibly powerful, I’ve used it before when struggling with confidence and nerves. Most people simply need to talk through their issues and problem solve. A pill can help numb negative/self inflicting feelings but will not stop them forming. I’m a strong believer exercise is a great healer, a nice long cycle or run can help to clear the mind allowing you to refocus,. Interestingly your body releases chemicals similar to those found in weed when you exercise, which is why you can get that euphoria after a long hard session, look it up 🤓

I see a lot of comments saying depressed people don’t “want anything,” but that is the entire point of this conversation. that is the reason depressed people are depressed: they don’t want anything; they have no reason to live. she is attempting to reignite what you lost. you all are entirely misinterpreting the point of this presentation because you don’t want help, but you searched this up for a reason and this is probably the best advice you can get.

    I can relate to what she said on this point when it comes to low motivation and mood while being used to not getting what you want/need. Could be anything, like friends easily dropping from your life. You wonder how you can ever get that back, potentially not feeling you can convince them to try again, and you just accept you won't have any close friends & suffer the repercussion of that dearth. We start to think there's no alternatives, a lonely road ahead & it's similar to "learned helplessness". For others, a different stressor or longterm insurmountable problem/threat.

      What this woman is talking about is depression that has no self-accountability. Lets be real, if someone else is making you feel bad/sad, the challenge of getting over that is honestly not that hard....as say trying to get through depression that clearly comes from our own doing. This video started off good, but then puts the blame on others. Thats all good if that is truth, but for many of us in depression, we are clearly the reason for how we feel, not someone outside ourselves.

    I cause my own depression at times. Dwelling on everything I'm not and want to be. I've been on medications for 10 plus years. I got off all medicine for my second pregnancy. I want to learn to cope on my own. Medications is masking and I don't want to mask anymore. My mindset is the problem.

      What would happen if you focused with appreciation for what you are...and looked for solutions, for a plan of action, with regard to what also you would like to develop?

      Trust me you'll feel better when you get a real job and financially stable, there's still time for that, now all you've to do is think about your goal and work hard for that.
      Just imagine you having your own money, you get no reason to get hurt.
