I do resistance training and walk daily a couple miles. One cup of coffee in morning. No alcohol. Intermittent fast 16/8. Tried low carb but it makes me feel like crap. Have enlarged prostate for many years. I’m 61 now. Had prostatis a few times, but have been able to resolve with antibiotics so far. Thank you for the education on this video. Very informative!

Hi Dr. Great video. I am 65, had BPH, tried Flomax and diet and finasteride, all to no EFFECT. After 2 years of trying and up to 6 times per night getting up, my Dr and I decided on Urolift and viola. I had immediate benefit, my first urination after surgery was my best void in many years. It has been over 2 years since and I am still very pleased, rarely have to get up during the night and when I do,only once. I'd do it again in a heartbeat!

Very informative and comprehensive discussion on BPH. My urologists simply don't have the time (or desire?) to fully explain the options available. Your presentation is absolutely necessary to make an effective decision. It took me several years to arrive at my decision going forward. It would have been a lot easier to see this video long ago. In any event, I'm going for the hoLEP procedure next week. I believe it would be the gold standard considering my condition and the size of the prostate lobes growing into the bladder. I'm fortunate to live in an area to find a doctor qualified to to perform hoLEP. (Only a single doctor within the UCLA system is qualified.) Afterwards, I'll be totally off medications which are mostly ineffective.Thank you for this video. More doctors should take advantage of social channels!

Sadly, doctors will not tell you secrets like that. They like to push prescription drugs. Couple of years ago I was having some blood pressure issues and my doctor wanted to put me on meds. I did a little research and found L-Arginine. Picked up some over the counter and two three per day. Went back to the doc and no issues. She said what did you do? I said something you know but will not tell. I left without telling her. :-)

I solved my prostate problem a few years ago by having a simple robotic prostatectomy. I had 86% of my prostate removed. I am now shooting blanks but still shooting. It got to the point that several times I could not urinate at all and had to go to the emergency room to have a catheter inserted. My advice, take real action instead of "living with" the problem which I did for too many years.

Questions you will want to ask your doctor/surgeon before you have prostate surgery
    1. How long will it be before my symptoms get better? ( Can be up to 3 months )
    2. How long am I going to need a catheter?
    3. Am I going to need to spend the night in the hospital?
    4. What is my chance of sexual side effects? (i.e retrograde ejaculation or erectile dysfunction )
    5. What is the probability that I might need surgery again?
    Great video!

I went plant based 3 yrs ago at 62 and gave up dairy, 5mg cialis daily for BPH / prostate cancer . I check PSA regularly and it’s been 0.7 for 20 years … do worry that primary no longer does a digit test for prostate … I guess I’ll have to see a urologist … with small fingers. Unfortunately being British I have to have a mug of tea at 9pm which doesn’t help with peeing during the night .. especially as I wake up with morning glory .. but .. luckily I can just go outside and pea without worrying about aiming … it’s also extremely satisfying .. lovely arcing stream .. like being a kid again.

I've had some sinus benefits from reducing my dairy intake, to zero for about 18 months then just some cheese and yogurt these days. My diet is mostly veg/vegan , occasional fish, and I have a pretty active job and lifestyle but haven't noticed any urinary benefits from this myself, though likely a number of factors at work anyway. I agree that diet, exercise and state of mind are areas worth looking at for this too. Great video and channel.

  • @alexmorgan3435

The big elephant in the room you do not even talk about is DIET. Dairy is a big cause of prostate enlargement because of the female hormone oestrogen which causes enlargement of the prostrate. I cut out dairy and now I can pee like a water cannon. Also a diet high in sugar and carbs causes high insulin levels which also contributes to prostrate enlargement. Cut out sugar and eat a diet low in carbs will cause a marked easing of difficulty peeing. Finally fasting is a great way to stimulate reduction in prostrate size and ease peeing. For me when I fast for 24-36 hours I can pee so much more easily after 12 hours and beyond WITHOUT having eaten. Also when you do eat, eat lots of green cruciferous veg such as broccoli, brussel sprouts, cabbage. Beetroot is good too. Also nutmeg for relaxing smooth muscle. I would change diet FIRST before doing drugs and certainly contemplating surgery. I did take the drugs several years ago but they were useless. Diet and exercise is the best. Sitting down for long periods is NOT good either. Most doctors just want to prescribe drugs and carry out invasive surgery as they have financial incentive to do so. Diet and lifestyle first!

Finasteride was fairly effective for me, but it also turned me into a eunuch. The Urolift did not help me, but aside from needing a catheter for three days it wasn't a terrible experience. It did not hurt sexual function. One procedure urologists don't mention is prostatic arterial embolization where tiny beads are inserted through a catheter in the arm or leg to block the blood flow to the prostate, causing it to shrink about 30 to 40 percent. It is done by interventional radiologists. It is currently considered an experimental procedure for prostate reduction but it seems to have promising results with minimal discomfort and no effect on sexual performance. It normally does not require a urinary catheter and is done as an outpatient. Improvement in urine flow generally takes a couple of months to appear.

    • @katazack
      @katazack 3 года назад +3

@@code_6 Hopefully the finasteride will keep your prostate growth under control with no further action needed. Up four times or more a night is no fun, hope you are getting some rest. It is very frustrating. I had a PAE three weeks ago (I'm also 67). The procedure went very smoothly, very little discomfort. But, unfortunately, to this point I have not seen any improvement and I am still getting up often each night. The prostate is supposed to continue shrinking for up to a year, according to my doctor, so hopefully I'll soon get some relief. Best of luck to all of us dealing with big prostates!

Great summary, thank you! A few questions about the HoLEP procedure if I may:
    1) Why do they cut into the bladder sphincter and put all the prostate tissue up into there, then use a 'morcellator' to remove it afterwards, rather than leaving the bladder alone and just removing the tissue down through the urethra as it is cut out of the prostate?
    2) Why do men usually recover bladder function after this surgery, but not always normal sexual function?
    3) Isn't there some way to accomplish the laser enucleation of the prostate - or, use another type of treatment or procedure for large prostates that will preserve normal ejaculation?
    4) One more question: Are surgeons using factors such as the speed of the surgery, the amount of bleeding afterwards, and the overall recovery time as their measurements for "success" rather than subsequent sexual function? If so, who is driving this?

I take stinging nettle (root) (500mg 2x daily) and this helped me from having to go so all night to only once or twice. Saw palmetto (from Swanson) did not do anything for me so I don't take it anymore. Maybe I should have tried another brand. Also taking pygeum (bark)(250mg 2xdaily) and pumpkin seed oil (1000mg 2x daily). (All the these are from Swanson as well.) I understand that walking and other leg exercise helps because of the improved blood flow. When I sit too long have noticed that the symptoms get worse sometimes! So I walk and climb stairs regularly now, so much better.

I tried saw palmetto alone and it did work. I've tried 3 different brands, e.g. some had different minor co-factor ingredients in addition to the saw palmetto. However, at first the saw palmetto did not work but I was only taking the recommended dosage, e.g. 1-2 caps twice a day. But as soon as I started taking like 4-5 cap of any brand, 2-3 times a day then I got results. Some nights now I don't get up at all(70% of time), but if I do, it now is only once. Also, my urine flow is at a good strong flow when I do go about 80% of the time.
I'm going to stop taking the saw palmetto soon and see if I can maintain my current 'status' with some minor changes to diet only.
Ironically I still have some stinging nettle (root) on hand I could take too but just haven't gotten around to taking it. It's good you mention root because I've noticed there are many stinging nettle caps being sold that use leaf. I guess from your research you already know it should be root - I have an ayurvedic text that describes nettle root as the part to be used for prostrate(as a side note, since true ayurveda goes by balance of doshas not treating symptoms).

Good point. Since changing to a modified vegetarian diet -90% veggies and 10% animal proteins). Note, cutoff all processed meats (cold cuts) and meats with hormones. Also cut soy and soy products. Main source of protein is goat cheese, salmon and lamb. I have noticed a significant improvement with my urination. Agree, you also need to reduce high sugar diet and processed carbs( chips, donuts, junk food). And last but not least- workout 3-4 times a week to burn calories, improve muscle tone and do a regular form of cardio (swim, walk, bike, row, hike, run),and also fast.

I'm not sure which one of the surgeries my urologist performed (two weeks ago). He called it robotically assisted prostatectomy. He told me that I definitely needed this surgery as opposed to the TURP. He went through the bladder and removed 80% of the prostate, and said the size was larger than 90% of all prostates. I definitely prefer laparoscopic to open surgery, so I am glad I only have two little slits that are healing up. Urinary function seems okay now, with little post op discomfort.

However I’ve contributed it to always wearing long pants when in public no matter how hot it is here in South Florida and having my knees in the breeze riding my motorcycle. 4 weeks ago I underwent a medical procedure removing two large bladder stones and been recovering ever since passing blood and blood clots when I void. As of last night after passing one of the largest blood clots so far, there’s less blood in my urine since.

Hello, your videos are very informative and quick, thank you for taking the time to explain all these options. I'm 50yo, with BPH, and considering Rezum, because I'm tired of taking tamsulosin, and retrograde ejaculation. However, I don't like the idea of using a cathether, however the benefits outweigh the sacrifice, and I'm getting a biopsy done first to be clear for the operation, and then Rezume my prostate. I'm scared, because I don't want to lose my ejaculation power, or have any other side effects, I want to remain fertile so this is probably one of the safest procedures with min side effects. I considered the urolift but I heard it's short lived and Rezum will last longer. So my question is: Do I wait until my prostate gets so bight the no medication works, or should I do the rezum now, and get it over with.

Does long sitting in a chair or prolonged bike riding effect, irritate prostate, BPH symptoms? Is there a certain way to pee for best emptying of the bladder with BPH? Can this ever go away on its own or is it just inevitable to get worse and worse over time?

One you could have added was, do any of the numerous prostate supplements work?
Those that emphasize with ingredients such as Saw Palmetto, nettle, pumpkin seed oil , pygeum, etc...and those that emphasize on plant sterols, such as Beta Sitosterol, etc.
I'm attempting to correct my BPH, which includes acute urine retention with a combination of prostate supplements, diet, exercise and non tobacco use.

Eat a healthy diet lots of green cruciferous veg and do some exercise. Don't spend all your time sitting down munching on crap processed foods which is bad for the prostrate. Also intermittent fasting and autophagy is very powerful at restoring your immune system and shrinking the prostrate. I know as I did this having had big problems peeing for a few years. Now fine. Most doctors only want to prescribe you drugs or carry out invasive surgical procedures as they make more cash if they prescribe or operate, but not if they suggest modifying your diet or lifestyle.

I justhad a T.U.R.P. operation after having to wear a catheter and collection bag for 11 months. The monthly catheter change at a nursing clinic was very unpleasant until they found I need a special catheter called a cude type. It has a small curved tip on the end of it that makes it skide through the swollen prostate gland , easier. Much less discomfort. Then one day after ten months of hell, I had a serious blockage and could not clear it myself by inject 50 ml of saline solution. I ended up in an emergency department, with a full bladder that could not empty. I was in agony. I could not drive my car to the hospital due to the pain, and had to call an ambulance. I had to wait two hours in emergency. When they nurse finally saw me she drained my bladder, and there was a LOT of urine in their. Talk about relief !!! Then she inserted a new catheter. I asked, "Is it in ?" The nurse smiled and replied, "Yes, it's in." I was surprised. The first time I never felt any pain nor discomfort with a new catheter going in. She had used a silicone catheter ! What a remarkable pain free difference ! The nursing clinic where I went for monthly catheter changes did not carry silicone catheters. I was awaiting urologic surgery all this time, which was backed up due to Covid. Two months ago I went into the hospital, and was operated on. They put me under with gas, and stuck a miraculous instrument up my urtethra and into the blockage in the prostate where the urethra was being pinched and compressed. That instrument had a camera, a cutting tool, and an electro cauterizing tool. The urologic surgeon cut through the urthra and scrped and removed tissue in the prostate. That would give the urethra adequate room to let urine flow properly like it used to. The operation was a complete success. I was out of the hospital in excellent shape after one night (26 hours. There was never ANY pain nor discomfort after the operation. It was a BREEZE ! I did have blood in my urine the next day and it continued for almost four weeks and diminsished over that time period. The urologist explained that the blood in my urine was completely normal and would eventually disappear, which it did. I am back to normal. However I do have a bit of a problem reaching ejaculation, otherwise I am in perfect shape. I hope this calms an anxiety for anyone out there scheduled for a transurethral prostatic resection. Seven moths ealier I had a prostate biopsy which I made them put me me to sleep for. They puncture your rectum to get at the prostate. I was worried sick about a diagnosis of prostate cancer (I am 64) No cancer....benign. I am still celebrating !!!

I don't know if it is a phobia or something, but the thought of it just puts me totally on edge. I could see myself avoiding surgery/treatment etc, just because of that aspect. Great topic, but really hard to cover that much ground in a short video. It seems to me that most lay people who are unfamiliar with medical terminology may not comprehend what you're describing in terms of surgeries. For example, your description of Urolift as using a suture to lift the prostate does not fit well with my understanding of the Z-shaped stainless wires that attach the urethra to the exterior of the prostate. Similarly, describing "water vapor" for Rezum is also vague. "Steam ablation" would be better. If you said "steam is injected thru a tiny needle that heats and kills off prostate tissue near the urethra", I think people would understand this option better. Perhaps a single video on "surgical" options would give you time to cover these in more detail.

Excellent description for the lay public Dr. Malik, I wish to respectfully add that there is good reasons to take heart is that at least a third of the cases of mild to moderate BPH would give no grief and on top of that after about a decade or so (some times sooner) the BPH goes away as the prostrate shrinks in size on its own. A somewhat sinister news is that because both BPH and cancer of prostate are common conditions in many cases the less malignant prostate cancer can be hidden in the BPH. If there is some suggestion of cancer from blood in the urine or from ultrasound, it might become necessary to do a prostate biopsy. But biopsy is hit and miss so the best course of action could be taking the whole prostate out and have pathologist take a look at the gland thoroughly. The trouble with the prostate removal is that in many cases the man would be rendered impotent. There is plenty of wisdom in trying to guard against prostate cancer and BPH starting in second and third decade through having a robust sex life or keeping up a high orgasm rate of between 10-20 per week. Having multiple partner is associated with prostatitis and BPH which seems to make a case for monogamy and refraining from casual sex and promiscuity. But it seems to be fine to practice swinging as they regularly use condoms. It is good idea to try to escape genital herpes but that is easier said than done because its dissemination remains bit mysterious.

Hey gents, Mee Too. I've suffered with chronic bouts of incontinence, for years, including the plague of dribbling. I'm 70 yo, and have had these urinary problems which I think stem primarily from spinal stenosis and other spinal related malady of degenerative disc disease, and scoliosis , all affecting the central and Peripheral nervous systems. This doctor Rena is very good at describing the problems of BPH, / Prostatitis. My PSA has always been in 2 to 2.4 range. However, two weeks ago, after feeling intense burning while peeing at waking in the morning, prompted me to go to Urgent Care the next day. Urine dip tested as showing an infection. So prescribed Cepro and Tylenol for the pain and fever (which varied from 99.5 to 101. After 4 days, pain and burning subsided, but urinating has become more problematic. So off to Urologist for the dreaded battery of bladder tests. Blood showed a PSA Jumped to 22.5!!! Initial prostste exam revealed not to have an enlarged prostate. The NP prescribed Flowmax ( Tamsulosin), and Phenazopyridine. I have NOT taken either. Dr. Rena, like all non holistic doctors simply prescribed drugs to treat the symptoms, not attack the cause. I'm going to through these tests, but I am going to try natural supplements approach. Another poster here who has become embattled in media fencing about diet do indicate to stop all caffeine, eliminate sugar and carbs, and eliminate Dairy!! Include Saw Palmetto, Stinging Nettle, black pepper, Pumpkin seed oil ( which I heard of before but never tried). I guess it can't hurt . Once you start Flomax, you can't stop , like statins. Bottom line is Look at any possible spinal related issues you may have. FYI, check out cervical instabilities on YT , Caring Medical YT Channel. All the nerve pathways flow down from the brain stem and cervical spine, affecting so much in your body including bowel and bladdel. Stay well.
