When I was young (around my teenage years), my father advised me to pee sitting down. Ever since, I don’t have to aim and I can urinate at full power when my bladder is really full without making a mess everywhere. (But I do stand up to pee when I’m away from home because it’s really inconvenient to do it sitting down and often the toilet seats are dirty).
Thank you Dr. Rena Malik. I come across patients with complaint of "split stream" very commonly in my practice as a urologist. This video gave me a deeper clinical insight in dealing with this problem, moreso because this complaint, of all other storage and voiding symptoms, is much less discussed in clinical literature than the others.
Great information as always Doc. I had this with frequent occurrence. Last year had green light laser surgery, extremely large prostate, over 130 grams. In retrospect I waited too long for this procedure, which helped resolve because flow was restored. Now a different issue. Because of the extensive work the bladder sphincter(s) were weakened. Now doing kegals to work on that. Guys....my message to you is be vigilant and don't wait. Consult your urologist.
That was a good post. Thank you. I've had a split stream since I was a young man. My urologist never seems to concerned about it. The explanation was good.
Yes. I don't know where you are, Frank. But here in Manchester UK, I was sent for a operation on my prostate. But the problem was, in fact, stricture. Indeed a surgeon did a probe just prior to the operation and stated he was a little surprised a it didn't seem large to him. I'm quite sure it's not just my area, or just the UK but a problem that goes much further. There is clearly a fixation with the prostate. At no point over the years I have been seeing doctors has it seemingly twigged. Not one person has said 'It could be a stricture, you know'? I find it astounding.
I just really appreciate your professionalism on these topics. As for me...I'm holding it and squeezing the head and making sure the very opening isn't in a semi open position...there is where I found where my stream would split...hope that helps someone... sometimes you have to hold it in and then... adjust and then...let it flow...👍🏿👍🏿
Didnt know I had a stricture for over a decade. Had it treated with several surgeries and doing much better now. Massive improvement in quality of life.
I've had a split and sometimes slightly weak stream a lot of the time for several years, and that tip of just pulling the foreskin back before peeing has instantly fixed it. Very thankful for this video!
Okay, I feel better now know that I only get the split stream soon after I've launched a load of baby batter. What's bad is that once you've adjusted your position so that one stream lands in the bowl the other stream is arching over the tank or hitting the wall. It's impossible to get both split streams into the bowl. A few times, I've managed to get outside in the back yard and stream away; this way there is no mess to deal with. My tip of the day.
Once again, an incredibly interesting bit I've learned about myself. This was something I'd noted but now I understand. Thank you, Dr. Malik, I don't know about anybody else, but I really appreciate and value your educational videos.
Great video. Had this problem on and off along with a very enlarged prostate. Had Holep laser treatment in June 23 and no more spray since. Wonderful being able to pee properly again after symptoms for around 10 years.
Thank you so much for making this and all of your videos. I’ve been wondering why I have had a splayed stream, and was starting to get a little worried. I appreciate your expertise and time to explain things.
Wow I’m not alone. I asked my primary care about it and he had never heard of it . He just got on his knee and said now explain it to me again. So I went to my urologist. Three times for other issues, hi how do you do ,oh that will have to be another visit. I call him Dr fifty visits. I don’t see either anymore. I resolved to just sit down to urinate. They never mentioned any thing like you did. How is it I can know more than them by watching u-tube? Thank You so much. Now I know more to find a real urologist.
It's strange as I get older things like that happen occasionally, one time it splits another time it goes sideways. Sometimes it's forceful other times it's weak. Not always fun getting old. 😢
Thank you so much for your expertise in this area. I don’t always have to ask my urologist so many questions when I go there and I understand him more now because of you. Thanks again Doctor Malik.
Very informative video doctor. I had a cystoscopy done in november. When the nurses prepped me for the procedure , they inject a numbing and lubricating gel into the urethra. I felt the tube with the camera and light going in , but it was no pain, .Or anything like that. So when are you guys doing it don't worry about it. Additionally , one month ago I had a prostate procedure where my doctor removed some tissue that with surrounding the Bladder. My urologist said this is a permanent solution. It was an outpatient procedure and was really nothing. I had no pain afterwards and had the catheter in for a few more days. And then once it was removed I was urinating like I was eighteen years old.
For me, it is simply by getting older. As a boy or a young man, I had no problem with aiming. But getting 60+, it became difficult and I started 'spraying', although I have not the impression that it takes longer to empty the bladder. Sometimes I get two streams. So I am now sitting down except I can go to an urinal where it works mostly like in former days.
When I was little, I was at the mall with my grandfather and he took me to the restroom for a leak. He needed to take a slash too, and we peed side by side at the urinal. I asked him why he shook during the pee and not wait until the very end. He said, "I'm not shaking the pee out. That's called Parkinson's." 😂 That was nearly forty years ago and I still tell that story.
I took Tamsulosin for 10+ years and never thought a thing about it-until side effects started showing up. Just check out the side effects before you take any meds.
Thanks for the explanation Doc!!!! I guess I can stop calling it "squeezing the water hose syndrome ". When I was younger the term was adhesion!!!!! I'm older now Doc I now have a broken water valve (slower stream) or the medical term a large prostate (slightly bigger than the walnut size). I'm learning more and more from you Doc!!!! Thanks and enjoy the rest of your week!!!!!!❤❤❤❤
Omg don’t even go there because that happened to me last week right at midnight before I went to bed!! It was half way too!! I could feel it and I was scared as hell because i just knew that the only way I was getting it out was going to the hospital and getting catheter in me. Well I tried something, since it really wasn’t hurting and I didn’t have to pee anymore I decided that I would just try to go to sleep with it stuck in there and deal with it when I woke up. I could hardly sleep because all I was thinking about was going to the hospital when I woke up. Well a miracle happened, I woke up and went to the bathroom scared as shit but I did, and luckily it popped out. Thank goodness it was small and perfectly a round one because if it would have been rectangular like most I get and turned sideways I would have been screwed!!!! I sure hope that shit never happens again.
Thank you for your videos. I was very blessed with a lot of hair everywhere, especially around my junk. One day I was urinating and I had that spray, and I noticed I had a pubic hair in my pee hole. So I decided to trim the "bush" and the issue went away. I was in my 20's then , now I'm 56.
I recommend this channel to my buddies that are growing older as I am. Lots of good information that lots of guys are not willing to ask their health care provider
I have two urethra starting further back and ending at the tip with smaller opening below larger pee hole. My urine stream is wide. I had a urologist explain that it is a rare malformation that one in 2 million men have.
I also have 2 openings the higher one usually passes urine while the lower ejaculate. If I have recently ejaculated my urine can start coming from both then reverts to the higher opening.
I noticed in your lengthy list of pharmaceuticals you didn't mention saw palmetto, which is a natural substance and far less expensive than those other medications. I told my doctor I was taking it and she's on-board with it. Nevertheless, a well done and information video.
Thanks Doctor I have had lots of problems since I had surgery to remove a tumour and not been right since up 8 times a night and proper struggling to pass urine and cannot fully empty my bladder.
I had a split stream once , it was a simple simple fix. At the time I was very hairy down there and one of the hairs got stuck in the tip of the urethra. So I did a little bush trimming.
The meatus naturally has a little bump where the two sides meet. Weak streams make it not open fully and cause the slit stream, which is what you get from having an enlarged prostate.
Ive had three procedures for the prostate n only get better for a year. Seems like it keeps growing. After last time now have e d issues. I need it again but im lookin for somrthing else ur recommendation please if u will.
I think the most common reason in circumcised guys is meatal stenosis. Without the foreskin, the penis head gets irritated from constantly rubbing against clothing, so it gets keratinized, where it develops a rough outer layer of skin, and also this irritation causes the urinary tract to narrow, a condition called meatal stenosis. Studies say it's present in 10-15% of circumcised men, but it's probably a lot more because most guys don't even know they have it if it's not severe enough.
I am a circumcised and purified man after my penis foreskin was cut up after birth. I rarely peed split and triple-stream, but I controlled them to single-stream urine. It did not harm me anything. I am not concerned about the split-stream. 👍😊👍
Thanks Doc!! Iam 66 a weak stream them it breaks to a full stream. After a small crystal or mineral passes iam thinking kidney stones? Had a little blood in urine but it cleared up.
As a normal person with minimal medical knowledge, I'd say the most common cause is that a pubic hair is stuck to the tip. Same concept as the finger covering the faucet/hose, you don't always know what's gonna go where.
It is circumcision, with a band of scar tissue under the penis it will still pull the frenulum down even in a flaccid state. You can try pulling the scarred skin forward before urinating and it will allow for a more solid stream but the off chance you let go and urinate on your hand.
High dose Vitamin D has worked well enough for my split stream, but with a full bladder, the stream is more powerful and does not split. Even so, I still prefer to use a bottle since I am relaxed about not spraying beyond the toilet bowl. I take over 20,000 IUs daily.
Hi Doc, is there a vitamin supplements that you would recommend to take for preventative of swollen prostate? I’m 53, I do not have anything yet. Thank you very much:)
I had a stricture as a child, which was corrected and recently had a aquablation surgery and developed a splayed stream after that. It occurs from time to time. Any thoughts on what might be causing it?
They used a large catheter or the tool used in the aquablation was so large in diameter that it was slowly tearing the urethral opening. Examing your person would show that the urethral opening is no longer an "I" but is more like an "J" so that hook is what is causing you split when the bladder is not full but you still have to go.
In the post you mentioned genital warts . That's when I remembered what i meant ask about. How to remove them? Home removal methods? Why and how you get them.
You forgot Prostate Artery Embolization for enlarged prostate. Urologists tend to forget this option because they don't offer it. These are done by interventional radiologists.
I have this procedure done on a regular schedule due to having bladder cancer and haven’t experienced it. But when I was having problems with kidney stones occasionally I did. I had the surgery done to remove those twice 😪
I have always emptied my bladder standing up and never made a huge mess or shot it everywhere. I often lose an entire pound by simply emptying my bladder.
Though the closed captioning is goofy, and that you could speak somewhat slower, your info is (urine) spot on... 😇 My prob is both sometimes, restricted flow + stream goes to one side unexpectedly and hits my foot or leg.. Like others below I think it's time to start sitting down to pee and or a visit to a Uro Dr. I'm 76 soon... Thanks, M Los Angeles
I have to disagree with replacing the foreskin when finished. If a drop of liquid seeps out and the foreskin is covering it, it will create a breeding ground for Balanitis - which leads to Phimosis. It's best to keep the foreskin retracted for a few minutes after urinating to give it a chance to dry.
I always use a piece of toilet paper to wipe it off. Capillary tension usually keeps a drop of urine and a touch of paper sucks it away. The same paper can be used to wipe any possible 'misses' that happened.
There is a reason why you get educated (aka "forced" 😅) as a male German student to sit down while peeing. If you ever visit a female students dormitory or a shared apartment with female students, you'll see the "Don't stand while peeing!" sign at the bathroom door. And you know you better sit down if you ever want to have a chance with any of them. Because THEY don't care about the physics of male urination. They just frown upon the splashes. 😉
@Kelle0284 Yeah, that's what I mean -- if you're the one doing the wiping-up (later), why not catch it as soon as it happens and avoid the bad karma . . ?
I always pee sitting down, been doing that since I was 10 years old and I'm 60 now, growing up with lots of women in the house I had no choice but to join them and not hearing about why I can't hit the bowl or keeping the toilet seat down, hey gotta love women what I do for them! thankyou Rena for the video
@@dingdong2103 well what ever works for you, it worked for me because I love them and that wasn't a hard thing for me to do,makes them happy & doesn't bother me
@@dingdong2103 Good then don't I didn't ask you to do that, if you have a female partner she would appreciate it down I'm sure 100% by keeping the seat down That's all I'm saying, if you love your partner then it's not hard to do that, most men pee standing up not me! in my house, I Sit Down that's my preference there's no right or wrong thing here I do what works for me!
I suppose mine is the result of trauma, as the duel stream developed after the removal of a kidney stone. At home its just easier to sit down to empty the bladder. In a men's room I stand between two urinals. I get a few glances (not glans) but it works. Thanks. Narragansett Bay
0:53 after my circumcision, I often starts peeing like a broken spray.. because my boy tip hole are flat wide, the urine stream often split into two, the upper end and lower end of the hole or since its flat steam the flow twisting, especially when my boy is like log its a disaster i need to calm it down or stretch open the tip to remove the sticking.. so yeah maybe also because of my frequent ejaculation my ureteral are getting dry that tip hole stick together.... 😅😅

You forgot Prostate Artery Embolization for enlarged prostate. Urologists tend to forget this option because they don't offer it. These are done by interventional radiologists.
After I had a cystoscopy done I’ve been having a split stream, have you heard of this?
Yes. Return to your doctor. Several possible reason but we should not be guessing. Best to go in and find out
I have this procedure done on a regular schedule due to having bladder cancer and haven’t experienced it.
My prob is both sometimes, restricted flow + stream goes to one side unexpectedly and hits my foot or leg..
Like others below I think it's time to start sitting down to pee and or a visit to a Uro Dr. I'm 76 soon...
Thanks, M

Great show! I get that once in a while.
Excellent as always. thank you !!
Can anyone please enlighten me about urethra stricture? Does it cost chest pain? Waist pain and constipation or diarrhea
Yeah I’ve had this before especially in the morning waking up
Thanks for the information. Very helpful.
Or you can just tear off a couple squares of tissue and wipe down the rim every time.😉
@@rickstadel5285 Or you can just learn to pee without making a mess. I prefer that method...
They probably won't know about the splashes. They'll get wiped up.
@Kelle0284 Yeah, that's what I mean -- if you're the one doing the wiping-up (later), why not catch it as soon as it happens and avoid the bad karma . . ?
Assert your dominance 😂
Great information thank you
The first thought that came to mind was kidney stone...😮
And I hate drinking prune juice. And other home remedies to shrink the stones.
I would never accept to be emasculated like that :)
@@Fletch8714 She doesn't mind, actually.
Usually when that happens to me, i just open the pee hold at my tip because sometimes it closes slightly and once i do that no problems🎉
Thanks for that. I had TWO strictures. How rare would that be?
You missed the most common reason for a split stream. A stuck hair over the exit.
True ... " Open the pipe first ! "
Lol
Good one, Doc! Thanks!
I have never heard of it ever happening before seeing this.
