If, by your question, you mean suggestions for switching from Qwerty to Dvorak, I do not know, but I feel a universal similarity is to work on your max speed by doing 10 words test, so you can gauge what your max potential at the moment is, then practice more with various forms of tests, and then do another 10 words test. I recommend MonkeyType, for this, although NitroType offers more raw speed improvement. If I misread your question, I'm sorry, I just woke up xD

I've never gotten better than 80-90 wpm because _I don't think faster than that._ If I were transcribing something, then more speed might be helpful, but even when I did transcribe the speech of others, they would stop and say "no wait, I meant ..." often enough that it was _better_ if I lagged a little bit behind with the typing.
For transcription I guess it depends more on if it is an overview vs more verbatim. Most people talk between like 190 and 230wpm. For court reporters and real-time captioning for hard of hearing (using steno), to graduate you usually need to take down 225wpm dictation for 5 mins straight at at least 95% accuracy.

He uses a DVORAK keyboard. So the keys on the actual computer are programmed to have different inputs. It's in the description
