Rammstein actually teased this song/video on April 1st (purposely), saying they invested in the "ZickZack Beauty Clinic", later posting photos of the cast in makeup/costume as fake ads for the clinic. You could call the (fictional) clinic and either hear the release-date, an instrumental piece from the song, or Til reciting a short part of the lyrics. The video fits into the ongoing theme of (wanting to stop) the passage of time, further feeding rumors that "Zeit" is their final effort.
Given that March 31 (the day before) was International Transgender Day of Visibility and my social media feeds were filled with both trans related and April Fool’s stuff, I was kind of taken aback for a second by the photos of the band in what was basically over the top drag. They’re a socially progressive band with a non-PC sense of humour so I found myself asking myself "where are you going with this, boys?". The video is great. It was just having those ads mixed with trans rights stuff that felt a bit off-key (nice curating job, Facebook…).

Their looks are actually partly based off real celebrities, Til looks somewhat like the "Botox Twins" while one of the guitarists has strong "Harald Glööckler" (german fashion-designer) vibes with the rest of the band shooting for a certain hair metal/glam rock look.

The guy with the pink hair that gave you chills every time he opened his mouth. He died April 5th of 2002 of a drug overdose in his condo on Lake Washington, he was found 2 weeks later by his Mother. Althuogh the video is hilarious, the message is very serious. Look for example what happened to the famous pop artist Michael Jackson and all of his plastic surgery (mainly due to his disturbed self image) In the end it even did cost him his life because of an overdose of medications and pain killers he had to take because of increasing health problems he developed. And no 'medical specialist' who told him to stop at any time, just for the money (and fame) they gained from him. While they should have protected him from himself, no one did.
As someone who understand German perfectly I know why you had to laugh so hard at that point. Well over here in the Netherlands we tend to say on that regard. Nothing grows in the shade :P

I don't think they're making fun of people who want to have work don to improve their self esteem. I think they're actually making fun of those who can't accept that they're no longer in their 20's and undergo these procedures out of mere vanity. This is why the audience is made up of elderly women whilst the band is trying to look as they did 30 years ago. They're also indicating the entertainment industry who pushes that people have to be ageless and superficially beautiful your entire life in order to remain relevant. The ultimate moral of this story is no one will be 21 forever; just accept that fact and learn to accept yourself regardless of how your body looks as it ages.
