I found out recently from someone's blind playthrough of Episode 2 that you can actually softlock the game. When the Combine use forcefields to stop your car in "Riding Shotgun", you have to disable an energy ball power source. Normally you eject the ball and it explodes outside. If it goes back in, the power core reactivates and resets the energy ball. Well, somehow this guy ejected the ball and had it re-enter the core the exact same time it exploded. The core acted like there was an energy ball in it and could not be deactivated, thus softlocking the game. Very rare occurance I imagine but he did it somehow.

Not sure if this was included in one of the earlier episodes, but if you propclimb up the tall building where Barney drops the crowbar, you can then backtrack and reload the level. All level entities already spawned in, including Barney, props, the crowbar, and even the doors will duplicate. You can Repeat this over and over until the balcony is filled with Barneys, and endless crates & pallets flood the stairwell below.
This is usually why valve makes it impossible to backtrack to certain points. But loading specific maps overrides this as technically it’s not a standard way of playing chapters. Hence having to load it from the command menu.
Fandom wiki says that "Hunters have also occasionally been seen unleashing all its ammo in a shotgun-style blast at a target just before death, in the hope of taking its killer with it." and there's a sound and an animation about this behavior, but this never happens in my testing. Can this behavior actually happen in the game or is it really implemented?
I don't think you should trust that source (heh), generally if there are multiple wikis for one thing the fandom one is the least trustworthy, as it's usually smaller and less maintained leading people adding things that might not be correct and having it stay there. A lot of the things that don't happen when looking backward is to ensure that players don't miss key moments incase they were distracted by something. The train collapse being a perfect example.
On the bottom floor of the Citadel Core in Episode 1, once you pressed the switch to get the light bridges to split and rotate to the centre, you used to be able to press the use button on them to rotate them back part of the way.
