Some context was removed from the adaptation of manga to anime, something that was in the manga was that Mahito intentionally removed some of his armor to bait Yuji into hitting an area he reinforced with all his cursed energy but Yuji countered with his Divergent Fist. And one thing the author said was that if Yuji didn’t hit that black flash before Mahito transformed, the max power black flash at the end wouldn’t have beaten him

Mahito can't just turn Itadori into a monster because he needs to touch Itadori's soul, ehich has Sukuna in it, and Sukuna would just off Mahito.
But with the 0.2 seconds Domain Expansion, he's safe because it was such a quick tap that Mahito bet that Sukuna wouldn't have the time to off him.
The only characters with Heavenly Restriction so far are: Toji, Maki and Mechamaru.
The disaster curses wanted Sukuna to ensure an age of chaos where the strong (curses) survive.
After Gojo got sealed, Mahito said the chances were 50\50 and may as well leave it at that because he wanted to kill Itadori.
Choso agreed, Geto didn't care, and Jogo wanted Sukuna free to guarantee the curses' victory.
Black flashes are known for being "random" and Itadori basically forcing one to come out is unheard of.
