The part from 1:14 on is so important and rare. To be aware of your own biases. Truly a sentence to be commended.
Just a correction - inflammation does effect the Male pattern baldness not just a temporary shedding, it shortens the Anagen phase which accelerates the permanent loss of the follicle, also not sure if the comparison with finasteride/Minoxidil was necessary as Ketoconazole Shampoo's are a complimentary treatment best taken with finasteride.
i am 22 and experiencing hairfall since I was 19.. an eye doctor prescribed ketoconazole shampoo .. i had no expectations tbh.. but to my surprise it stopped my hairfall completely within 4-5 days.. and ever since then I have to shampoo my hairs everyday. But it's not really working anymore..
I have been using minoxidil for a month now after consulting a dermatologist.. i will get on a topical minoxidil+finasteride+procapil treatment without re-consulting the doctor from tomorrow.. the doctor refused to give me finasteride.. but i did thorough research and i am pretty sure i am the perfect candidate for finasteride.. I am guessing he doesn't have much knowledge about finasteride and it's probability of side effects and ways to alter doses.. it sounds bad but i think i am gonna take things in my own hands..

pop down to the nearest border town if you're still waiting and it's not too far because you're going to be paying out of pocket anyway. I lived in Canada for two years and I did this frequently to get in to see people like the dermatologist or get a cheaper cell phone with all over north american roaming (if you have an American friend, use their address to bypass all the BS)
Canadian waiting lists for various medical needs is wild and most Canadians have no idea that you can get in to see doctors like dermatologists for as little as $70 US when you're not seeking treatment in egregiously priced cities like New York. Look for doctors that take Medicaid and/or HMO, they cut mad deals.

What about the nizoral cream though? The shampoo is used only for 5 mins while the cream can be left to absorb in the scalp, wouldn't that reduce DHT to a greater degree than the shampoo would?
This could potentially be better in regards to everyday use, keep in mind anti androgens don’t decrease dht they beat DHT and Testosterone to the androgen receptors in the scalp, KT is a weak anti androgen so the way it would beat DHT and Test to the receptor is not by binding affinity but by sheer number of the anti androgen. However you have to think about the risks of satellite interactions with other receptors in the case that it goes systemic. You could argue that with it being weaker it shouldn’t have any satellite interactions with let’s say receptors in the musculature. However if it cannot bind to receptors in the body in the case it goes systemic it will most definitely not heat DHT and Testosterone at the scalp
you are correct about ut competitively binding to androgen recpetors. But also, many doctors have metioned that there is a theory that it also interferes in the conversion of test to dht. also, it was shown to increasew gorwth and density like minoxidl but we dont understand that mechanism yet.

Obviously finasterite and minoxidil are the drugs used as first line, but Ketoconazole has shown in studies (small sample size and low quality) it hsa potential to help in hair loss. Thus a 3 month trial on it every 3-4 days, i dont think would be bad given the person uses it with realistic expectation.

Your best doctor is you, give it a try. It has work wonders for many people including me. The only thing is that it makes ur hair dry. The thing is that shy can build up on ur hair causing hair loss, thus shampoo is very good at getting rid of it.
Hello Doctor, I've been a loyal subscriber since day 1. Thank you so much for all the information and knowledge you're sharing. It's keeping thousands of men from practicing wrong methods of preserving hair. Also had a question about mixing Retin A with minoxidil solution. Does it provide and greater pentration capacity?
Trust minoxidil is formulated correctly. One wouldn't want minoxidil to penetrate any further than it does else you would grow hair in your brain ( joke) but seriously you would actually absorb it into the test of the body ( not good).
@@dantheman1337 makes sense. But my concern was that because it takes time for the minoxidil solution to be absorbed in the skin. Hence some of it goes waste, suppose let's say i apply it and rest my head on some surface or that while applying it with my finger tips most of it sticks with the finger etc. So what remains of the solution on the sclap has to be absorbed properly because some of the prescribed amount of the solution does goes to waste
@@hoejackborseman1059 As Dan said, if you increase absorption then you are increasing the chances for sides. Having said that, it could be argued that Minox is a very safe drug to use (it is available over the counter after all) so the potential risk vs reward for improved absorption is worth taking for some.
Risks: heart-related anomalies, brain fog, dark bags under eyes, wrinkles etc.
Rewards: studies have shown Minox to be 4 times more effective with microneedling (could be due to the repair mechanism rather than increased absorption).
Companies could formulate Minox to absorb more, but then they would likely lose being able to sell it over the counter. It's more of a commercial decision for them rather than what gives the best results.
If that's true why don't company bring out this idea to their product ....don't spoil the composition ....
The anti androgen effect is not through the inhibition of 5ar enzyme, it doesn’t not block the aromatisation of testosterone to dht. From theory it should locally bind to androgen receptors in the scalp, beating DHT and Testosterone, potentially halting miniaturisation of the follicle. However the theory around it’s localised presence can be debunked by the fact that if it can be absorbed it will inevitably go systemic. It is a weak non steroidal anti androgen… not a 5ar inhibitor. It may work for people who are not genetically prone to AGGRESSIVE androgenic alopecia (99% of males are prone to hair follicle miniaturisation to a certain extent) but it will not be an effective compound if used as stand alone mono-therapy for androgenic alopecia. Keeping in mind with that, the use of finasteride will also not successfully halt androgenic alopecia because even though you are running your DHT into the ground there is an increase in testosterone. Testosterone has a lower androgenic effect at the hair follicle. Meaning the speed of miniaturisation is decreased but not completely halted. This is where there is potential found in the use of better more potent non steroidal anti androgens to combat hair follicle miniaturisation on both the androgen receptor front and then using finasteride to decrease DHT and indirectly reduce the level of androgens the anti androgen needs to beat to the receptor
Do you have any topical anti androgen recommendations. I use fin and ketaconazole now
@@clarkemikesell6029 did it help? do you have any side effects?
Hey Doc. Im a 19yo Boy and had an itchy and oily scalp, often came with dandruff. There were some red spots on my scalp too, exactly there on my vertex, where my hairs shedded. Those spots disappeared after one month and i showed my derm those pics and she said: nothing but worry but you can try Ketoconazole Shampoo. One week after starting it, my hairloss went from an average daily loss of 100hairs to 20/25. I track them while taking shower by approximation. But it has reduced the amount of lost hairs significantly. I still see the pattern of MPB, but its slowed down extremely.
Do you have a statement for this situation? I started it like one month ago. Could this help me in the future to regrow hair? Im on NW2+vertex
If you are stable then that is great and what you want. I would suggest going back to the derm and coming up with a plan together.
The Hair Loss Show okay, Thank you for your answer. In short, at least the half year i have lost around 100hairs. There were sometimes fluctuations but thats not significant here. Since using, i experienced an stable loss of only 20-25 hairs a day. No noticeable side effects, i am using the shampoo every 2 or 3 days, letting it working for 5min and then wash off.
This is a great achievement for me and i would definitely recommend it to your clients, if they have an unhealthy scalp and ask them afterwards if they noticed any benefits.

Hi, great videos, lots of info. Can you please make a video in regards to proviron (Mesterolone) and it's interaction with finasteride? I am wondering if proviron cancels out the effects of finasteride? TY
you cancel most of the DHT conversion from testosterone. But proviron is littarly the same as DHT in the body just made in a lab. That DHT/proviron finasteride would not help against as it does not deal with the androgene reseptors, or the DHT itself, but inhibit the enzyme 5a who creates DHT from testosterone.

- there will be hard or harder tissue below your nipples

The Nizoral did wonders in ridding me of seborrheic scalp!!! 😮😊
I have used minoxidil for one month it started shedding a lot i know about initial shedding phase i was afraid so i don't want to continue..can that shedded hair due to minoxidil will regrow without using minoxidil further or that lost hair was lost?
Just keep using it dummy
Which 1 is more better ketomed syampoo or dht bloker syampoo and 1 more if I use ketom/dht syampoo can I mix with other syampoo like ginseng syampoo or biotin syampoo coz I know if using ketomed/dht syampoo only 2 a week so the balance I will use other syampoo
so you say it doesn't work in the beginning of the video but then cite studies that suggest it does work. i think ill go with the study over your subjective experience with patients.
Thats the thing. A lot of doctors that advocate it probably do becasue they see the positive results in their own patients. Just cuz the research is limited or whatever, doesnt mean it doesnt work.
Is it safe to use Nizoral containing 2% ketoconazole (recommended application 2 times/week for the first month followed by 1 time/week thereafter) alongside Revita hair stimulating shampoo that contains 1% ketoconazole?
Ive been using this after 3 weeks def less hair fall for me, a dry scalp for longer so the baby hairs stay up longer for volume.
You could add a mild sulphate free shampoo for alternate days as ketoconazole dries the living daylights out of hair.
How it works now?
Thanks for the video! Do you have any suggestions on frequency of hair washing, especially for thinning/fine and oily hair for women? I see lots of mixed commentary online. Thank you!
Once a week with keto shampoo. Any more can damage scalp
Which shampoo is good for supporting minoxidile treatment ?
Is this have reaction to perms or color on hair I suggest no color or perms to my clients if have skin allergies that' have psoriasis or sebaceous cyst or any kind of dermatitis of the skin until that is healed ..
Does ketoconazole go systemic when applied topically? It is a pretty potent antiandrogen when taken orally.
What about acetolone shampoo or can you recommend any other good shampoos?
How about ketoconazole cream or ointment so you can actually leave it in the scalp?
hi doctors i really need your reply i am having sever hairloss i have been using minoxidil for about 5 months now my doctor recommended that im losing most of hair from my crown i am only 18 my hair come off very easily and they have some sort if white bulb on their roots what type of hair loss is this? my hair very thinning now the girths are thin

Is a shampoo with Piroctone Olamine better than a Ketoconazole shampoo for anti inflammatory purposes?
I’m curious too
Thank u doctor!
Could u please tell me how to make diff between MPB and other type of hair loss?
Thanks
Before I buy this shampoo, can you use it without conditioner? I heard that using only shampoo can make your hair too dry and brittle. Does anyone here use Nizoral by itself?
Im 17 i have been using 2% ketoconazole shampoo for a month and i noticed a lot of shredding than normal and it made my scalp soo sensitive.what can i do dr.
Can Ketoconazole shampoo and finasteride be used in conjunction?
Oh yes
some recommend to put contraceptives in the shampoo... what do you think? since estrogen protects the hair follicle?
What about Alpecin?
It's just caffeine shampoo might help your hair grow slightly faster but thats about it.
I saw very good results using Alpecin + Nizoral 6 days a week, minoxidil once a day for 6 days a week and on the 7th day microneedling 1.5 mm without any drugs on that day (don’t want anything to go systemic). I started the microneedling recently, saw good results even without it, so if you don’t want to do it... you don’t have to, but studies have shown it to be very effective. The results have been amazing. Alpecin is probably the least effective part of this treatment though. I still use it since it’s as cheap as any regular shampoo and I need to wash my hair anyway. The important part with any shampoo is to let them on the scalp for 10 minutes before washing them off. So, maybe go in the shower apply Nizoral + Alpecin, wash the rest of your body, then wash off the shampoo from your hair at the end.
@@darthghitza8037 funny you say that because i also rotate between nizoral and alpecin. Difficult to say if its working but I found both shampoos regardless. Re time: I think I read a study that you shouldnt leave it in for more than 90 secs of Alpecin at a time. Nizoral: i thought 4-5 mins should be ok but cant recall if there was a guide to have it in for more. I also do microneedling, but only do it every few weeks (I should probably to it more often). Of all my treatments, I beliebe Avodart worked by far the best
Do you recommend using products that contain human growth factor extracts like LC Cell company products and does it work for male pattern baldness and is there any side effects?
I don't think they make a significant impact on MPHL.
What about using it for shading at the times when the scalp feels itchy and burning?
Very helpful video. Thank you.
You guys are amazing.
I as a female actually used to use this shampoo (Nizoral) about 20 years ago on my hair, but probably more so to have a clean effect on my hair than for hair loss since I wasn't peri-menopausal at the time. But as I am older, wondering if I should go back to trying this again and curious to see if it works? Wondering also after using Minoxidil on my husband, maintaining with this shampoo. I realize as a doctor you have to push the pharma and office procedure agendas, but it just seems like you are against all things natural. It would be nice if you could confirm success stories without medication use.
try any other good quality shampoo that has ketoconazole instead of nizoral cause nizoral dries out your hair so much. I have been using one called regenepure dr and its not harsh in your hair at all, doesn't dry it out but leaves it clean and I have grown more hair than before
I don't wanna attribute all the hair gains to ketoconazole cause I have been doing other things like applying diluted rosemary to my scalp and derma rolling.
Because most natural things simply don't work because if they did then scientists wouldn't waste time and money developing treatments. They'd simply extract from natural sources and sell it rather than doing expensive research and development. Not everything that is natural is good for you and not everything man-made is bad for you. Many studies showcase natural remedies have a very low efficacy if any at all.
Viruses and bacterium occur naturally yet kill millions a year. Vaccines and antibiotics are man-made and save millions. It's a spectrum. Not so black and white
Hi I'm a cosmetologist for forty years since 18 I believe unnatural products n I suggest tea tree. Shampoo o. On psoriasis as well asdermatitis at the hair and skin but I also believe that vitamin e oil is the best alternative because it's a healing oil.
Slightly off topic but what would you say with regards to the dosage of finasteride if I take creatine as I understand creatine has some implication on DHT production.
I've not yet started on finasteride but wanted to ask if I should perhaps take 5mg a week instead of 3mg.
(I'm 24. Temples receding and crown is okay atm)
Also, on an earlier video you mentions a DHT threshold below which minituarisation doesnt occur. Is the effect on minituarisation linear until this threshold is reached or is it a binary effect with regards to the threshold?
Thanks
Wait. Creatine makes you lose hair??
Over the counter (1%) and prescription (2%) Ketoconazole both made me LOSE hair! Once I stopped using it, no more comb full of hair. So, I just live with the itchy scalp.
Happened to me too but it all grew back stronger than before, turns out all the weak hair that were about to fall off all shed at the same time so i would persist for at least 6 months.
@@samali6109same here .how long did it take for regrowth

What do you suggest for prevention for subjects using DHT-derivates in non sky high dosages?
So my hair was getting thin and at the same time I had a real issue with sebhoric dermitis. Inflamed scalp, thick skin build up and bumps on my head. I got ketocortizol shampoo and it cleared my head. Shortly after my hair was way fuller.
What about topical finasteride - how do you compound it?
What's you recommended in dandruff problem
I 😊 HAVE USED IT ALOT AND HE HASNT DONE ANYTHING FOR ME I STILL ITCH AND BURN AND MY HAIR FALLS OUT IM GOBS STILL
Can you please do a video on the effectiveness (if any) of vitamins, saw palmetto , biotin etc .. Thanks !
John Milano Jr. I’m 17 and experience shedding and a little bit of thinning on my temple, however no receding hair line or bald patches. Should I be worried and consult a dermatologist to get an evaluation?
