I have two by my ex coworkers:
- refactor a critical part of project, deploy untested into production and leave for 3 weeks vacation. (of course, the code broke many things, team spent days and nights to get this back to work)
- once a coworker encrypted a code base because he (unasked) added his own code from his private project and told the boss to pay for it... he got fired and sued.
