Earthbind (I believe is the name of the totem) kiting with a 2H is the fastest leveling for shaman. Kargoz had a full list of leveling speeds which I believe was tested by Melderon.
Almost correct - Granted you stack tons of water, pure elemental is faster for levelling. 2handed with rockbiter and earthbind kiting while managing swing timer is however almost as fast and does not require the same amount of drinking. Also if you prefer to level with one hander and shield - the idea that flame tongue is optimal has been debunked unless you stack tons of +dmg/healing
It's more about what gear you have at any current time. An awesome 2h dropped from a dung, then use a 2h. You got an awesome dagger from a quest reward, then use a dagger.

A common misconception is that fade is a thread reduction spell. While that isn't entirely false, the way it works is more like a reverse taunt. Because it reduces the threat only for it's duration. Once the effect is gone, the threat is back. In other words it's useless to reduce threat and as far as I remember most of the time the effect is to weak to actually serve the purpose it's made for. Popping something like a limited invulnerability potion will do a lot more than fade.
It is useful on the pull. If DPS hasn't done anything and you have any healing going on at all, it puts you on the threat table. A quick fade can get the extra second or two for the tank to pick up threat. Yeah, in an ideal situation you would never need to do this, but hey, it happens. You are chain pulling a dungeon and got renews up on targets in between pulls, things like that. Just that extra level of convenience as a healer. I've played both a druid and priest and while fade is certainly not needed, it was definitely noticeable when I didn't have it on my druid.
