  • Dr. Peter Coleman, who is Board-certified in Addiction Medicine and has been in recovery from opioid use himself since 1984, discusses what causes precipitated withdrawal and what precipitated withdrawal from opioids like Suboxone (buprenorphine) is.
    -Safe, Comfortable Outpatient Detox
The Coleman Institute for Addiction Medicine has been a leading outpatient addiction treatment center since 1998. Our innovative withdrawal management programs, often referred to as The Coleman Method, have helped thousands of people free themselves from addiction to Alcohol, Opioids, and Benzodiazepines. Originally founded in Richmond, Virginia, our network has expanded to cover 14 cities across the US.
    -3 - 8 Day Opioid Detoxification Process
For short-acting opiates like oxycodone, most patients complete their detox and begin Naltrexone therapy within 5 days. Longer-acting opiates like Suboxone and Methadone typically take 8 days. Our standard opioid treatment package in Richmond, VA, Wellesley, MA (Boston area) and Willoughby, OH (Cleveland area) includes the detox and 6 months of case management and Naltrexone therapy. As a non-addictive opioid blocker, Naltrexone dramatically reduces cravings so that patients can focus on their recovery. We have a 98% success rate for completing detoxification and starting Naltrexone therapy. During the detox, we work with patients and their families to develop an appropriate aftercare plan. We also offer a convenient, 3-day outpatient alcohol detox and a Rapid Benzodiazepine Detox.
    -Caring, Empathetic Environment
Our founder, Dr. Peter Coleman, understands first-hand the challenges our patients are facing due to his own personal struggle with the disease of addiction. He has been in long term recovery since 1984. Dr. Coleman has dedicated his career to treating addiction patients and creating a team and an environment conducive to long term recovery for his patients.

When he says switching from 100% to 70% leads to 30% the math aint mathing because the times I went into precipitated withdrawal I did the math and was well over at least 9000% withdrawl. Absolutely brutal, hellish nightmare to endure. After the 3rd time it happened, I waited 6 days and still went into the worst WD ever. The only times that I've actually considered killing myself were times that I was in acute percipitated withdrawl. Makes riding out the regular withdrawl seem like a walk in the park and when you're in withdrawl you won't believe it, you will likely be thinking "there's no way it can get worse then this" but oh boy are you wrong. If you go into precipitated withdrawal there's a good chance it will be the worst experience you'll endure in your entire life and I mean it.

All jokes aside; yeah. Imagine hell. Then imagine the hell of hell. Thank God for methadone.

Now let’s have someone that been through precipitated withdrawal explain it. I’m sure they won’t be telling it like this.

Seriously I don't know where the hell this guy got his degree or certification from but I really feel bad for anyone and his program

Lmao it's because he's explaining it from a pharmacological standpoint not from the physical symptoms associated with precipitated withdrawal

Dr. Coleman and The Coleman Institute for Addiction Medicine, Again I've heard you use the term "comfort meds"...what exactly are you speaking of? Thanks.

Your Opiate of Choice. Thats the only comofort med that exists for precipitated withdrawal, the only thing I'd imagine could possibly help is a good dose of benzo or something but none of the stuff theyre willing to actually give you is going to even scratch the surface. If you go into precipitated withdrawl your choices are to sav out, take more bupe and prepare for the worst 24 hours of your life followed by another day of meh or cave and get on a relatively high dose of your opiate of choice to try and load the receptors back to to get back to normal relatively quick. A lot of people who do self Induction at home end up caving and getting high because it is unbearable - I decided to wait 6 days before taking bupe because after 72 hours I wen into PWD and even after 6 days I still got thrown into the worst PWD again! I couldn't believe it, I had missed the mark after waiting 6 days so I had to cave and get high because I have a life to live but I have decided I won't touch bupe until at least a month because a couple weeks of regular withdrawl seems nice compared to the Precipidated Withdrawl. For 24 hours you'll want to die and then the following day you will wipped out but by day 3 or 3 you'll start coming around.
I guess my advice is if you're serious about quitting wait and use fentnayl wait as long as possible before taking bupe. When you do take it make sure it is on a day where you have at least 48 hours to recover because you'll need at least 2 days before you're doing anything besides crying for help and throwing up and shitting your pants with a restlessness you've never experience, it is truly blinding! Prepare to go into Precipitated Withdrawl and either have your Opiate of choice at hand just in case or make sure it is completely out of reach so you can't possibly cave, I advise you keep it nearby because you will be begging for it. You'll go through it and will decide to try again another time if you have ANYTHING to do besides lay in bed sick as a dog useless to the entire world.

Bad information. precipitated withdrawal is a horror. that %60 %70 example is bad as well.

I don't think this guy's qualified to be in charge of any kind of treatment program. Precipitated withdrawal is not when you go into 10, 20, or 30% wd; precipitated withdrawals when you go straight to 100 within a matter of minutes. Buprenorphine binds to receptors like super glue in fact it binds to The receptors stronger than any other opioid/opiate. So if you are using a full agonist and then take Bupe before that opiate fully leaves the receptor then I bet buprenorphine will strip it and send you into FULL withdrawal. It is by far the worst sing an addict or recovering addict can go through. It is complete torment and it is hard to come out of. Depending on your situation taking more Subutex or Suboxone won't even bring you out of it it will only make it worse.

No., Taking MUCH more will absolutely make it go away or at least level it down.
We’ve been so inundated with the “32 mg ceiling” talk that most addictions specialists believe it. But MACRO dosing (I’m 72 mgs and up) has been used with VERY good results on ERs with patients thrown into precipitated withdrawal caused by the naloxone administered during a fentanyl or herion overdose.
It’s all about getting enough medication onto those receptors. If 8 12 mg strips (or 12 8 mg strips) is what it takes to get there, then that’s what it takes

      AGREE - A TRILLION PERCENT!!!
I have Lupus, Dysautonomia, CRPS, Hashimoto's, and a sprinkling of other Autoimmune Effing Disastrous conditions. Accidentally put myself in PW a week ago, and in abject desperation began to look up ANYTHING to make it stop.
(Short Backstory: I was a pain management patient on Fentanyl patches {125mcg/48hrs} for 14 years. I have organ involvement with the Lupus and suffered a bout of acute encephalopathy during which I was unconscious for 4 days. I got better, went home, and filled the next month prescription with zero problem. Went into my next appointment (wearing my last day of my last patch) and was told to leave. Immediately. This was during peak Covid times, and none of the hospitals were allowing any 'detox' for any reason. Cold turkey? SUCKS!!!)
Anyway, while I was already feeling enveloped in the Freezing Fire of PW I took 4 more strips and it fixed me. I regularly take at least 40mg and my Specialist is perfectly fine with it.
I am beyond happy to read your comment, if I may ask do you do all at once a day because my person has me splitting things up into smaller doses throughout the day. Thank you SO AMAZINGLY MUCH!!!!!

I absolutely agree with you, and have recently begun macro dosing as well.
(I had a relapse and put myself into PW - AND IT WAS HORRIBLE!!!!! The freezing fire, the GI symptoms, anxiety off the chart, etc.) Since I was totally screwed anyway, and NOBODY knew that I had effed up, I found the info I needed and took 4 more strips; knowing that I would have to go to the ER one way or another..... AND IT STOPPED!!!!!
Thank you so very much for your comment! Two weeks down & lots to go. 🤗🙏🏻🥰
