    Four years ago I was in massive knee pain, had to use a walking stick, and saw an osteo surgeon. He said as he looked at the scans that both my knees are bone-on-bone with no cartledge left, so I should have both knees replaced. He explained that the pain was from the arthritic swelling around the knees so a very strong anti-inflamatory medication, 15mg of Meloxicam daily, should help initially. It did help from day 1, and I was gradually able to reduce the dose and frequency so that after about 3 months I took a half tablet maybe every couple of weeks when the pain was too much. I was 72 at the time. Now I might take a half dose of Meloxicam once every few weeks if I get pain. I started going to a gym but couldn't use the exercise bike for more than a few minutes and the reclining bike was sheer hell. With Covid-19 closing the gym, I started doing long walks. In the last four months I deliberately changed my diet and reduced my weight from 92kgs to 82kgs and now go for a 4km walk each morning, about 70 minutes, and do at least 10,000 steps a day. I also spend an hour or more almost every day in my pool (I live in the tropics of Australia so it's always moderate to darn hot outside) doing knee and other exercises. This morning I mowed all my lawns, and said to my wife that six months ago I couldn't have done that, my knees and back (I've had two lower lumbar laminectomies and both hips replaced) would have given me massive pain less than half way through. In my opinion, getting to a sensible healthy weight is very important so I'm not putting unnecessary stress on my knees, and gentle but consistent exercise, plus the right mindset, are the three most important solutions to my knee pain. Sure, they're still bone-on-bone, always will be, but I now do not see surgery as a desirable option anymore. At 76 now I'll keep on walking (not furiously, just gentle pace) and exercising. Oh, and I always wear shock-absorbing shoes (New Balance).

      Thank you for your post, I’m in my 40s & I have bone on bone. I was very active before my pain became too intense. I used to walk 3 miles every morning. I think I’m in the severe category. My orthopedic said I have a bucket handle meniscus tear. My doctor said surgery because I’m bone on bone & Nothing is helping. Wish me luck.

    Great advice - Im tired of people telling me that I should walk to help my knee - Im in the severe catagory.

      I have been told total knee replacement for both knees. My stupid insurance company says I've got to do 4 weeks of PT & fail it before they'll allow the surgery to be paid for by them. They nor any doctor knows the amount of pain I'm in 24/7 never stops. They'll only give me ibuprofen 1000mg that does 0 for my pain. I might as well drink freaking water. I'm almost falling & have fallen because of my knee problems. It sucks I really need something that's really going to help with this pain.

      @@DedraAmbroseandSnowthe physical therapy will be good for you because it makes your area around your knee stronger and after surgery it won’t be as bad! I am where you are but I don’t want that horribly invasive surgery! Let me know how it goes. I’m on FB too same name no numbers. Hope all goes well.

    Namaste.... Thank you so much.. i use to walk only 100 to 200 steps at a time.. after watching your video... I kept practicing at home from a week... today I walked 1100 steps at one strech with out any pain..

    Doc, I'm very glad to have found you on yt. 30+ years ago
    I crushed my right knee, on the inside portion. Of course, I was young and didn't have it checked out. All these years, as you've said, aging, loss of synovial fluid, improper exercises have led to limited bending and a lot of pain. I don't think surgeons are always the best answer to what ails you, but, how do you find a doctor who focuses on the mechanics to avoid surgery. As Dr. Peter Glidden, DN says of allopathic doctors, everything looks like a nail to a hammer. Also, Dr. Bergman, D.C., naturopath and researcher for corrective treatments says OTC pain relievers destroy cartilage. I am learning from you and these doctors, and finding improvement in the pain and movement categories. Thank you for dedicating so much time on knee recovery. Wish you were here! 😁

    Omg don't underestimate the pain of arthritis and torn cartilage - both knees ! Thanks to Dr D's knee program I'm about 95% pain free - that's huge ! Still stiff and sore but working towards healing the cartilage with the daily exercises. And I do the Dr D Walk !

    Your videos are life changing. I have been down for 3 months and listening to your and listening to your videos are really helping me. I am at the irritated knee situation doing physical therapy. I focused on Getting the leg straight and now I’m taking your advice and not walking more than 10 minutes to prevent the irritation

      ​@@epmanualphysicaltherapyI've been told by my orthopedic surgeon that my bone on bone is so bad with both knees. That I need total knee replacement for both. My right is the worst of the 2. I'm having to use a walker now because I've actually fallen. My insurance company is forcing me to do 4 weeks of physical therapy & fail it before they'll pay for the knee replacement surgeries. My ortho said that I have no cartilage left in either knee. I can't stand or walk longer have 3 minutes before needing to sitting down. I've also got chronic back & neck pain & need surgery on both. Any advice I can take to my 1st therapy appointment to talk to my therapist about?

    Very well explained. Thanks! I’m going privately as soon as I can. Been diagnosed bone on bone knee arthritis in both knees. Luckily I just need partial knee replacements so I’m glad. Feels like I’m not alone when I hear you talking

    Good information and well explained. I was told I had bone on bone earlier this year. Was sent to an Orthopaedic Surgeon who said I needed an operation and put me on anti-inflammatories which caused a reaction and in the middle of the night made me think I was having a heart attack. Am now working with a physiotherapist who has given me exercises, and things are beginning to improve. Am working my way through your videos. Wish I'd found them before I bothered seeing the Surgeon.

      Yes! I do this as often as possible, I want to keep as much mobility in my knees as possible. I also stretch out my legs before I even get out of bed in the morning.

    Thanks so much for this information. I'm glad I found your channel! I've started walking the Dr. D Walk but it seems to be causing some discomfort in my knees. I pronate and wear orthotics so perhaps this is part of it. I have a gritty kneecap (femoral patella syndrome). I walk twice a day for about 10-20 minutes each time. Any advice would be welcome. P.S. I've started doing the glute exercises too to build up strength in that area.

      The #1 mistake that leads to knee pain with that walk is over contracting the glutes and making the quads work too much. We discussed exactly this problem in this video ⬇️

    I am in the severe group and it’s a nightmare! I’m currently scheduled for a TKR surgery next week. My other knee is mild grade 2 but my right one is grade 4 and haven’t been able to straighten it for about a year and legs are bowing.

    Thank you so much for this advice. It must help a lot. The arthritis is the one that's irritating my knee a lot. I still could bend my knee but very difficult because of the irritation. It got swollen badly, I guess from the walking I did before.

      You're welcome Teresita!!! Yes, if you're swelling from walking, then you're causing more irritation when you walk. Minimize how much you walk. In the mean time, you need to work on your glute muscle strength so that you can get ready for walking. Start with these exercises:

    I’ve gone from knife in the knee joints pain and feeling like I would end up in a wheel chair. To
    Curing myself by taking 3000mg of Glucosamine, no bread, sugars or grains.
    Now waking 15 miles a week / 60miles a month. More I walk …the better they are getting. Walked straight 3 hrs the other day…

    My patella has no cartilage. It fell off when they performed a microfracture operation 2010. It hurts slightly always when i stand or walk. I still pretend it does not hurt and i can still walk like 5 km without major issues. I train almost daily various physical activities, but its a constant battle not to do too little or doing too much, very thin balance. Best to do various "sports" but in limited amount each time. Walkin stairs (up)is one of the best heavy exercises that almost not hurts at all, at least for me. Long lasting cocktail parties with hours of standing is a killer, at least the next morning:)

      Hey Joe, We're so sorry that you've been struggling with no cartilage in you patella due to microfracture operation!😯 It is good to hear that you can still walk 5 km without any major issues but we would like to encourage you to Not push yourself through any pain, otherwise it can aggravate the knee. We don't want that to turn into a bigger issue. You are right about trying not do too much or too little and the activities that you do should feel good but it's normal to feel tired in the surrounding muscles. It takes self awareness to distinguish when to stop or keep to moving but it sounds like you are doing a great job with that!🎉 It's exciting to hear that you can walk up and down the stairs with little to no pain. The more you focus on your glutes and lower abs it should help the problem as well! Great job! You got this!🙌

The Patella is a bit problematic, in case of a resurface, only option is to do a TKR, even if other surfaces would be in good condition. But will postpone that as long as possible, and after the long lasting recovery of the microfracture, the condition have remained stable, not worsening. BMI is also important, be as light as possible...

    Many thanks for this helpful information.. I’ve got medial compartment bone on bone grade 4 (knee) My ortho recommended a partial replacement only. My question is …what happens if down the road , I end up needing a total knee replacement? I’m so confused 🤔

    I have pain sometimes sometimes not I did notice I have alot less stiffness and pain inthe morning I can just get up and not much pain or stiffness like it used to be I take msm, and on low carb diet for 3 months now and the diet seems to becoming comfortable now, no starvation feelings. I notice I still have some edema inmy lower legs but not as bad as it used to be. when I sit in my computer chair I do get pain on getting up but I think it is because the chair digs into my leg right on the spot I injuried a few years ago. scar tissue? if I sit in my lazy boy there is alot less pain and stiffness upon rising. I can walk and do things for about 30 minutes and then myknee starts to ache a bit your site is just so informative I have friends who had knee replacements but I dont want surgury I dont want mybody to be cut up. I want it to heal like it is designed by God/nature.

    Thank you so much Doctor for such an informative and totally helpful irritated bone on bone knee joint video.
    My orthopedic surgeon basically said everything here but you added more layers on coverage.
    I was thinking of getting knee replacement surgery but I will now hold off and follow your excellent advice.
    I would usually walk for 1hr and 30min. Now, I will walk for only an hour and listen to my body during and after the walk. I am 54 and will hold off on the surgery.
    If the condition worsens, then I will get the knee replacement.

    I have severe bone on bone on both my knees. Given Tramadol for the pain but it doesn't even touch the pain. Can't stand for more than 5 minutes at a time.
    Told from the knee Surgeon he won't do replacement until I loose almost 100 pounds.
    How the heck can you exercise when you can't walk.

    I had knee replacement 8months ago but I still can’t walk more than 5 minutes. I can bend my knee all the way , I had good range of motion even before the surgery, walking still hurts, I get swelling on the knee after walking. X-rays show no problems. What should I do ?

    How about a full knee replacement 4 yrs ago. I’ve been left with a straight leg with almost no range of movement. I’m in more pain now than before my surgery , can you suggest anything, please

      KNEE ABLATION: GET A CONSULT. For me, at this time, it has been a MIRACLE. I have been in pain for decades. And NO ONE told me about this. RADIO FREQUENCY ABLATION.

    I’m planning to have knee replacement in January. I was diagnosed with bone-on-bone arthritis 6 years ago. After my last X-ray a couple of weeks ago my orthopedist reported that I have “extremely severe bone-on-bone arthritis.” This week we’re having cold, damp, windy weather and I can barely be on my feet more than a few minutes. It’s hurting even sitting in my recliner with my leg supported by pillows. 😩😩😩 Ibuprofen doesn’t touch the pain.

    I think I have the non irritated knee situation, I appreciate your videos, very helpful. Would you recommend using a stationary bike with no resistance? I've been doing this the last week, it seems to help.

      I have one, I don't use it as often as I should. I think it helps maintain quad strength without weight-bearing, which is so important when you have bad knees.

      @@ponygirl6258 The point is not to "maintain quad strength" but to create glute strength with a special technique of cycling! It is more a pulling movement than a pushing movement with the pedals. Just strong glute muscles will help you with all kind of bad knees. That is my own painful experience.

      @@ponygirl6258 The professional way is to use clic-pedals with clic-shoes. The other way is to drop down the forefoot slightly in front of the pedal, so that you have the chance to perform the pulling work with the forefoot. In both cases it is important to keep the focus on the gluteal work as a prime mover. Maintaining the mind muscle connection will give you the feeling of a real working gluteus maximus. Goal is to fight the quad dominance and to keep the push/pull work in balance.

    I have bone on bone knee pain, due to runners knee and hip diplasia, does keniesiology tape help to prevent bone on bone pain? FYI knee cap high up and to the right on the leg to almost dislocation potion about 1/4 on the right space before dislocated. Hope that helps appreciate your feedback.     Is cycling good for people who are having cartilage degeneration in the inner side? There are cycles which can be used sitting too. Is it ok for any part of knee degeneration. ? My doctor has advised me viartril S sachets to drink does this help sir ? Thank you

    My problem is that my left knee wants to go sideways for just a minute, but it only hurts a little; it just scares me every time. I never have warning; but it scares me to walk a lot; I am 79, though!!

      Great question Linda, it really depends on your muscle imbalance. If you have quads that are too strong, then yes it will hasten your bone on bone knee problem. But it might actually help if you’re muscles are in the proper balance.

    Doctor told I am with varicose veins. Though there is no manifestation
    Suggested walking. With stocking , I am walking two times . But my right leg is bending. I feel pain, but , still at tolerance level.
    Can u suggest exercise. I am doing as per physical medicine doctors and physiotherapist.

      You may have the beginnings of knee arthritis. Possibly a meniscus tear. It's hard to say without seeing you in person.
      You might start off with glute exercises to make sure that your walking will go well for you. Here is a suggestion for a starting point.

    My knee is bone on bone. It hurts. I can walk 1.5 miles. It hurts but I push through it. I have good range of motion. I have been told I need to. I am not sure where my knee falls in your descriptions here. How can I know for sure?

      Hey Michael! It sounds like your knee is heading toward irritated. It probably is irritated when you're pushing through pain during walking. You've got to keep the irritation down and find the right amount of walking/exercise you can do that won't irritated your knee. Also be sure that the way you're walking is helpful for you - using glutes properly. In case you haven't seen this video about using glutes when walking, here it is:

      Hey Pratibha! Great question! Yes water aerobics can help to offload your knee joints, but if you're exercising in the water and feel your quads working too much, then you're going to make your muscle imbalance worse that way. Even if you're in the water. The key is strengthening the glutes. You've got to make sure you make your glutes work in the water to get the best effect out of the water aerobics!

      Hey Jeannie! Using an elliptical will be helpful if you can get your glutes to work during the exercise. If you feel you are using your quads more than your glutes while using the elliptical, then you will likely make your knee problem worse :-(

      Standing is hard for me, too. I keep shifting my weight if I have to stand, like when I'm in the line at the grocery check-out. I also flex them periodically, it helps a little.

      It sounds like your knees are very irritated right now. You've got to get the irritation down so you can better tolerate standing and moving. Rest up and do exercises to begin to make your knees feel better.

      You've got to get the inflammation in your knees down by reducing the amount of pressure going through your knee joint. That should allow you to rely on painkillers less.

      Hey Linda! Sorry you got worse from walking. You might have needed more glute strength before you go on walking, just like we mentioned in the video. Stretching might feel good for a short while, but it doesn't improve joint health like strengthening does.

    The lame horse med MSM stopped ALL my knee pain, every so often it flares up and I take a teaspoonful a couple days and it goes away for 4-6 months. and it's safe. It's given to 10 million dollar racehorses, they wouldn't prescribe it to them if it didn't work, they are more valuable than people so you can know it's safe lol. I've told many people about it and they often call me and thank me profusely.

      I'm sorry you're hurting. I hurt every day, too. There are lots of natural remedies that can help many people, but they don't help everyone. Have you tried supplements like glucosamine, collagen, turmeric, ginger, calcium, boron? A lot of people say cutting out processed sugars helps, there is some evidence that sugars promote inflammation. I've tried the supplements and dietary modifications, they don't make the pain go away, but they do seem to help. Just some ideas. I hope you feel better.

      Great suggestions pony girl!!! You've got to find the right mix of solutions to get traction on a knee problem and heal. Start trying different things and see what works, stop what doesn't. Ultimately if you can settle on 3-4 things you can keep up consistently, then you should begin to take your knee health in the right direction. Correct exercises is usually a mainstay for people with bone-on-bone knee problems.
