This game is called Phantasm in Japan, and its cover was the ghost with some characters in manga style. They changed it for something more "usa = movies" as they always seemed to believe at this time. The arcade version is more a credit feeder with a lot of stuff hard to avoid. This GB version is probably the better game.

I did, couldn't resist. I got the Hi-Skate shoes with the Nintendo controllers on them (they're even on the aglets) and they will NEVER be leaving my feet.
I bought this game back in 92 or 93 during a trip to Gatlinburg. I remember because I watched SNL with Cypress Hill/Shannom Doherty in our condo that night -- Cypress was banned for life for smoking a joint on TV, and I think Doherty was banned for being a bitch. I loved playing as the Guru simply because I could fly, and it took me until the arcade version to realize that Dracula flashes people when he attacks. And as for that cover. if I didn't already know about the game through Game Pro, I'd have assumed it was about a vengeful mobster.
Oh, and if you miss all the keys. the game ends with the ghost lamenting that he left his girlfriend to die, but keeps smiling anyway. It's a lot harder though because you have to play as a Level 5 enemy.
I saw the cover when the stream was loading yesterday (I only saw part) and I was like why is there a gangster with a thompson on the front? It's a robot game. Then there was a ghost, and I was just super confused by the art. Weird game, but looks kinda fun in that goofy as hell way.
Welp, considering you're already dead, it was going to end bad one way or another. I wish we had more games where we played as ghosts, unlike most games where ghosts are the bad guys.
It's one of the enemies in the game, if I'm not mistaken - so, you could have, like, any enemy from the game on the cover, (e.g. the mech shooting missiles LOL) and would still make about as much sense... unless I'm missing the plot or sth xD
Definitely agree with Mike that that has to be one of the most misleading cover arts for a game ever. Definitely need to recommend to Mike to play this game again to have a more complete version on YT.
