Aren't there two types of fear with mobsters, those who are unpredictable and those who are calculating. Carlo Gambino was by far the most successful and cunning mobster there is, you don't achieve that without inspiring fear in other mobsters.

I worked at a Pizzeria in Florida for years that was owned by a nephew of Edward Lino. Their last name is Lino as well. He passed though and now his wife and daughters run the restaurants. Just didn’t have the white collar understanding and approach necessary for the top level. Castellano lacked the street level connection, Gotti lacked the white collar style.
