    I love u! please come out with youre own program, like physeo, pathoma, first AID, lectures and questions, with question videos. You are the BEST, youre multisystemic, its what lacks in todays education. Professors have a chip on their shoulder, they should be helping students not let them fend off on their own. My medical school knowledge and integration has purely been because of your videos!

    I have sickle cell disease SS. I'm currently in a crisis which has been getting worse but they won't admit me into the hospital claiming they are at capacity due to C-19 patients. I get how severe C-19 is but so is sickle disease.
    I need an oral surgery which is 3 years in the making because I can't afford it. I've had 2 hip replacements due to Avascular Necrosis which has ruined my front teeth. It's so humiliating.
    I've postponed my graduation for medical billing and coding because I can't smile.
    If I made a GoFundMe for the surgery it probably won't get any support.
    Trying to manage a crisis at home is so hard.

      You're not alone dear, I have sickle cell disease too. I have major crisis every month and minor crisis every day. I repeated a class in medical school, because I was too sick to write my professional exams. I literally die and resurrect anytime I have to write any professional exams. I just want to let you know it's not the end of the world for you, you're meant for great things. Just see yourself as God's special creation, cos everything changed for me once I saw myself in that light.
      PS: I'm a doctor now😎

    Thank you for this very thorough series. Very proud that a person from my country is doing a great job. Makes me feel proud to be an Egyptian. But I do have one question. Doesn't hemolytic anemia cause acholuric jaundice due to the unconjugated bilirubin not being able to produce yellow pigment unlike that of obstructive jaundice? Thanks in advance and great video as always

    12:23 um no autosplenectomy does not occur at 12 months of age for everybody. I just checked a study on it and half the scd patients did not have autosplenectomy, mean age 28 years.
    15:17 g6pd only kicks in in vitamin D deficiency.

    1-, infection. I think children have weak immunity and with sca its even weaker.
    2- acute chest syndrome.
    3- s.pneumonia, >70% cause of death in children with sca
    4- also s.pneumonia
    5- in sca, salmonella, but generally staph.aureus

    Sir does coca cola like urine discoloration occur with indirect hyperbilirubinemia ? I'm confused about this point, can you please clarify it ?

      It occurs with obstructive and hepatocellular jaundice, not with hemolytic jaundice.
      So, the answer is no.

      Probably, but patients are different. That’s why you need lab results to know the stage that the patient is in.

    Isn't unconjugated bilirubin insoluable in water?? So how can it be excreated in urine and give it cola cola like colour??

      What actually happens is that the liver works overtime to conjugate as much bilirubin as possible (converting unconjugated bilirubin to conjugated bilirubin)…The conjugated is water soluble and will show up in the urine.
      In all cases of hemolysis, as long as the liver is normal, it will conjugate more and I will have high levels of conjugated bilirubin.
      Moreover, the hemoglobin released from RBC hemolysis is toxic to kidney tubules.

      Sickled RBCs go to spleen-> phagcytosed by splenic macrophages--> extravascular hemolysis--> Hemoglobin is broken down into heme and globin. Heme is broken down into iron and protoporphyrin--> unconjugated bilirubin-> liver--> conjugated bilirubin --> dump it into GIT, since the excess of liver secretion is stored in gall bladder and since the liver is working harder to keep up with the excess hemolysis-> you have higher risk of forming gallstones. (Especially the bilirubin subtype of gallstones, not the cholesterol subtype).
