Unfortunately I have SCD😞 I come to videos like this for comfort when I'm in pain. I know it seems weird but it helps me relax 😴. It also doesn't help I'm a Colorado native, because of our higher elevation. Also please keep in mind, I can only watch videos and such with lower pain levels because unfortunately sickle cell can be debilitating with high pain levels.

While in hospital, giving 50% oxygen unsickled reversible sickled cells, but it did not reduce pain, in a study. I think though that's only because it takes a while to decide and get to a hospital and receive treatment. If you tried to maximize oxygenation (breathing etc) at onset of crisis, probably most sickled cells could still be reversed. Acidosis leads to right shift of the bohr curve. Two ways to prevent it, one right and one wrong: decrease in co2 eg by hyperventilating reduces acidity, BUT (god knows why*) less co2 directly reduces cerebral blood flow through vasoconstriction, which can lead to strokes or seizures in already compromised sickle cell blood flow. So the other way to reduce acidity is by increasing bicarbonate, which is something that can be done by eating baking soda and has already been shown to be beneficial in ckd (ckd is common in SC). I say baking soda should also help in crisis onset. Not only does it make oxygen stay in HbS by left shift of release curve by lowering ph, it also allows more CO2 to be in the blood, which increases brain blood flow. *actually the weirdest thing this is: there is this big hole in the literature that on the one side acknowledges that co2 directly regulates amount of brain blood flow 'through some unknown mechanism'. So I figured it must mean the brain actually needs co2 somehow. What I find is animals actually fixate co2, especially the brain, ie co2 is turned into some other carbon molecules. Nobody talks about that at all since 1960 or so. Is this some sort of conspiracy??

No they’re different diseases. Thalassemia causes reduced synthesis of alpha or beta hemoglobin. Sickle cell, is caused from a mutation in the gene that encodes for beta hemoglobin so they produce the wrong type of hemoglobin.
Thalassemic face .. they have maxillary hyperplasia and frontal bossing ( prominent frontal bone) due to extramedullary expansion of the bone marrow and this is seen in thalassemic pt rather than sicklers .. in a pt with thalassemia you may find spleenomegaly but above 5 years you almost will never find spleen in sickle pt due to auto amputation and this is exclusively for sicklers .. also there is another clinical feature could differentiate between them

How can v distinguish sickle cell and beta thalassaemia based on clinical features ?
The demographics are different! because part 1 of physical exam is an overview of the patient’s “look”.
