With NVGs, I wouldn't be surprised if the helicopter crew had a different plane in sight and never saw the CRJ. We won't know until later whether that's actually a plausible explanation.
On VAS you can hear the controller approve his request for visual separation and tell PAT25 exactly where the CRJ is and was going more than a minute before the collision. I think the controller did everything he could/should have. At the end of the day, the controller is not in the vehicle with them and has to rely on their reports. And PAT25 reported he had the CRJ.
The helicopter PIC is at fault. One question is… did the pilot ever really see the CRJ, or just acknowledge the communication and then start looking. Something that you should NEVER do.
If ATC is legitimately querying either party as to whether or not they can even see the other party in the air, it's already too late for safety to be ensured in that situation and further preventative steps would need to be taken, such as the helicopter grounding itself completely in the midst of several passenger flights ascending and descending
One of the few presenters who has his facts right. We hear about Army aviation accidents from time to time. A helicopter flies into terrain at night, or two helicopters collide. Rarely do we hear about an Army helicopter colliding with a commercial aircraft. This one is going to bring up all sorts of issues. The boys at Fort Rucker had better get their act together. Army aviation can't just go around punching civilian aircraft out of the sky.
The main reason that Reagan National is still in operation, is because Senators and Representatives don't want to be "inconvenienced" with having to drive to Dulles Airport to take a flight home. My mother worked for USAIr at Reagan National for a decade (83-92), several of those years in an office in an old, converted hangar right next to the flightline. She had many friends among the aircrews, and heard many horror stories about flying in and out of DCA. While plenty of big jets fly in and out of DCA safely, the margins of error are much slimmer than many other airports. At least two or three times in the past, reccomendations were made to either close DCA, or limit DCA to general aviation (i.e. no large passenger jets). Each time, Congress pushed back, and even went so far as to fund the EXPANSION of DCA. She had the benefit free travel on USAir, but only flew out of DCA, if she coludn't get to her destination via BWI, and then only during daylight hours.

My wife doesn't like flying and I was teasing her about it because "it's really safe, planes almost never crash".... well her plane was set to land at DCA like 22 minutes after this happened and her flight was redirected to Dulles. It could have been her. What a sad thing for all the families.

ATC should have been more aggressive in separating the traffic. They were using multiple frequencies. This means they are too busy and it is bad for the pilots' situational awareness.
You may be not understand the frequency issue. All civilian aircraft transmit and receive on VHF (Very High Frequency) communications equipment. Military aircraft radios operate in UHF (Ultra High Frequency), and are less susceptible to electronic jamming and interference. ATC (Air Traffic Control) transmits and receives on both. So, the controller can speak to and receive on both VHF and UHF. Both aircrew can communicate with ATC, but they cannot hear transmissions from the other aircraft. Some, but not all, military aircraft simulcast in both frequency ranges. Given this was a VH-60 helicopter tasked with VIP transport, it is likely they communicate on UHF only for security purposes.

This is bad for the situational awareness and frustrates the controllers as well. They get angry at us when we talk when he is monitoring the other frequency, which we cannot hear.
The helicopter pilots are only cleared through a particular corridor along the river at a particular altitude (in this case 200ft). To vector the helicopter out of this means you would have to guarantee them traffic and obstacle clearance outside of this corridor. It is perfectly legal and reasonable to ask a pilot to maintain visual separation. However, lots of factors are involved in this crash: direction of flight, possible use of NVG, pilot error, airspace congestion.
4:42 you are wrong. He specifically asked for VISUAL SEPARATION which was granted. He was essentially saying "Can I handle the separation?" to which the ATC granted. ATC says "affirm".
Nah, this one is easy. Army helicopter was asked if he had the CRJ in sight? He answered in the affirmative and asked for visual separation which was granted. Then he flew into the airplane. Either he was looking at the wrong plane on final or he misjudged his speed.

To be fair, night vision goggles transform the world into green and white, bright and not-bright. In that kind of environment, it can be harder to see a plane that is against the horizon or when ground lighting is in the background. I'm not sure if that's what happened, but this might have been a rare case where NVGs made night flying harder, not easier.

The FAA and Military are at fault for allowing the helicopters a flight path right in front of landing planes. Add to that the ATC and Military are on different radio freqs is absolutely insane.
Other reports show the Apache had ADS-B active, and it appeared on the ADSB Exchange system that does not censor information, whereas it didn't show as present on various FAA linked commercial flight tracking systems, that hide data for military and as a premium cost service, commercial tail numbers trying to evade public notice.
The helicopter reported seeing a different CRJ that had just taken off not knowing the plane ATC was referring was descending right into its path. So this was a SYSTEMIC failure and sadly we have too many other congested airspaces with similar risks to all of us in the flying public.
The problem is sky is full of planes going from point A to B, too much traffic of planes, thousands of them in sky at any moment, not only in US all over the world now-a days.
Main question is why was the military conducting training operations in the class B airspace of the landing zone of Reagan international in the first place?
The zig zagging looks rather alarming but that’s just a data quality issue. ADSB data is very accurate, but without it, the location will appear to jump around. They were likely flying pretty smoothly in real life.
Having been in the biz for nearly 4 decades, I can’t tell you the number of times that I’ve called traffic in sight only to later realize that I was looking at the wrong traffic. While far more critical here, I suspect that this is what happened. Terrible tragedy.

It is not uncommon for Military to fly ADSB off for routine/training flights as it's entirely their discretion not ATCs. Most just keep it off unless the mission requires it.
Do military abide by ADSB? Also do they have TCAS (CRJ does, but as you say is inhibited) I'm curious as I fly with a ton of military guys and they all seem to point that they don't have extensive amounts of "civilian" avionics.

Uh I believe your wrong about the Helo pilots transmitting statement. He replied the he has the CRJ in sight and will (maintain visual separation) That is said on the radio for a specific reason. The Helo pilot is assuming responsibility for separation and its recorded so if this happens there is a record.

What really happened? The controller switched 5342 to runway 33, putting him directly in the path of the Blackhawk. Had he continued to runway 1, like the CRJ-700 in front of him, the Blackhawk would've missed him. Blame the tower, not the pilots who were only doing what was instructed!
No, Blackhawk was in the wrong. it's incredibly normal for small aircraft to land on the short runway at DCA. ATC gave the Blackhawk visual discretion to avoid the CRJ which it failed to do.

Wrong. The chopper reported the crj in sight and would visually separate itself. The controller directed the chopper to pass behind the crj.

I live in the DC area, I prefer IAD and BWI. I try to avoid DCA. It's just too congested there, including car traffic.
The pilot of the jet wasn't to blame . He was cleared to land .
That leaves the pilot of the helicopter or the air traffic controllers ...

I lay this at the ATC. Generally you're not going to deviate from a flight path or fly "ghost" in a congested airspace. THIS IS THE ATC's fault, IMO.
