      Moody GhannaM I was thinking the same thing, but what caused the explosive decompression in the first place?

    • @melvinthedeathless.melvint6727
      @melvinthedeathless.melvint6727 4 года назад +1913

      @@zackplaylists The plane was not pressurized for unknown reasons. At high altitudes, a non-pressurized cabin is suicidal, as there isn't enough oxygen to support human life. There was another case with a rather similar scenario, Helios Airways Flight 522; in that case, the pilots forgot to pressurise the plane, by the time they realised they are passing out and dying due to a lack of oxygen, it was too late.

    I grew up in Spfld mo, and I knew the Stewart family well, and mowed their lawn when I was a kid. I felt really bad for his mother because the military was at her house while the plane was still airborne. She had lost her husband several years prior, and when this happened, it broke her. She passed away within a year or two afterwards. I am sure they were prepaired to shoot the plane down as opposed to letting it go down over a populated area. And sadly...I'm sure they discussed that scenario with her. He was a good guy, never a snob. And his mother was a sweet woman. RIP Stewart family.

      Carson Morris rapid depressurization at 45k feet wouldn’t give much time for anything. I doubt this plane was equipped with drop down oxygen because it was a private jet. All US pilots in in-pressurized/non oxygen equipped planes are restricted from flying above 10k feet for more than 30 minutes because of lack of oxygen at that altitude. 45k feet is usually -50degrees or colder. Rapid depressurization at 45k feet would be like not timing your breath right when you jump off a diving board, and needing to breath at the bottom of your jump. And still having to come up. It would leave maybe a minute of panic and suffocation. Not much time to get down to a safe altitude either. Even if they programmed he autopilot before they passed out to descend they would be dead before the plane got that low.

    On 25 October 1999, professional golfer Payne Stewart was killed in the crash of a Learjet 35. The plane apparently suffered a loss of cabin pressure at some point early in the flight. All on board are thought to have died of hypoxia, lack of oxygen. The plane, apparently still on autopilot, continued flying until one engine flamed out, most likely due to fuel starvation. It crashed near Aberdeen, South Dakota after an uncontrolled descent. The exact cause of the pressurization failure and the reason behind the crew's failure or inability to respond to it has not been definitively determined.[16]

    There was an investigation on this and the conclusion was that there was a rapid loss of cabin pressure. The pilots went for the emergency handbook and started to go through it, however, as it contained a lot of information and everything was crowded in it they struggled to read. The fact that they were beginning to develop hypoxia made this issue worse. The first thing they should have done was go for their oxygen masks, yet the handbook said otherwise, which could have saved everybody on board if it was correctly written. So the plane flew to a high altitude of something I cannot remember and was flying at the highest it could actually go. So high that it was bobbing because it would try to climb but descend due to how thin the altitude was. They had no choice but to leave it.

    • @TheBCSledder
      @TheBCSledder 6 лет назад +86

      Ryan Harte-Stovell I would think they should've known immediately what was happening and should have know what to do, ie descend to lower altitude, put on masks etc without having to flip through a book. A sad incident indeed, but I think the reliance on emergency books rather than the use of common sense can sometimes lead to similar catastrophes.

    • @RyanTheHero3
      @RyanTheHero3 6 лет назад +30

      TheBCSledder unfortunately pilots can’t remember it all off by heart and there are a lot of things that need to be checked. Yet this airline thought that getting a healthy supply of oxygen was not a priority and that it could wait until the rest of the plane was sorted. Even if they knew what to do, it would be following the handbook which was telling them to check unimportant things first.

    When I worked at USAF HQ in Europe, a Soviet jet from Poland infiltrated West German airspace, so two F-15s went up, ready to shoot it down. But there was no driver! The canopy was blown. The MiG-23 kept flying, American pilots in F-15s from Denmark were in position, for 550 miles, several hours. Orders were given: Once it is "feet wet" over the water, shoot it down. But then, the MiG ran out of gas--so the heat-seeking missiles wouldn't find it, and bullets wouldn't explode the fuel tank! So they watched as the jet crashed into a house in Belgium, killing an 18-year-old man.
    A Polish Colonel had ejected after take off when the afterburners failed. Then the MiG maintained flight! The Soviets never once notified the West that a jet was headed our way, and later never answered questions about the possibility of chemical or nuclear hazards. This kind of thing happens more often than you might realize, on both sides.

    I feel like the computer systems should have a safety feature to avoid this. A separate cabin-pressure detection mechanism should be tied into the autopilot. If the pressure drops below some safe level for humans and the plane's controls detect no interaction or input on any switches, dials, or sticks, the system should assume the humans have passed out. The autopilot should then immediately and automatically descend to a safe altitude where a leaky cabin doesn't matter (like 10,000 ft) so that outdoor air pressure is enough for occupants to regain consciousness and divert somewhere. You MIGHT crash into a mountain depending on where you are (but GPS + world Map could determine safe alt?) but it's better than being unconscious and running out of fuel...

    The NTSB determined that:
    The probable cause of this accident was incapacitation of the flight crew members as a result of their failure to receive supplemental oxygen following a loss of cabin pressurization, for undetermined reasons.
    The Board added a commentary regarding the possible reasons why the crew did not obtain supplemental oxygen:
    Following the depressurization, the pilots did not receive supplemental oxygen in sufficient time and/or adequate concentration to avoid hypoxia and incapacitation. The wreckage indicated that the oxygen bottle pressure regulator/shutoff valve was open on the accident flight. Further, although one flight crew mask hose connector was found in the wreckage disconnected from its valve receptacle (the other connector was not recovered), damage to the recovered connector and both receptacles was consistent with both flight crew masks having been connected to the airplane's oxygen supply lines at the time of impact. In addition, both flight crew mask microphones were found plugged into their respective crew microphone jacks. Therefore, assuming the oxygen bottle contained an adequate supply of oxygen, supplemental oxygen should have been available to both pilots' oxygen masks.
    [A] possible explanation for the failure of the pilots to receive emergency oxygen is that their ability to think and act decisively was impaired because of hypoxia before they could don their oxygen masks. No definitive evidence exists that indicates the rate at which the accident flight lost its cabin pressure; therefore, the Safety Board evaluated conditions of both rapid and gradual depressurization.
    If there had been a breach in the fuselage (even a small one that could not be visually detected by the in-flight observers) or a seal failure, the cabin could have depressurized gradually, rapidly, or even explosively. Research has shown that a period of as little as 8 seconds without supplemental oxygen following rapid depressurization to about 30,000 feet (9,100 m) may cause a drop in oxygen saturation that can significantly impair cognitive functioning and increase the amount of time required to complete complex tasks.
    A more gradual decompression could have resulted from other possible causes, such as a smaller leak in the pressure vessel or a closed flow control valve. Safety Board testing determined that a closed flow control valve would cause complete depressurization to the airplane's flight altitude over a period of several minutes. However, without supplemental oxygen, substantial adverse effects on cognitive and motor skills would have been expected soon after the first clear indication of decompression (the cabin altitude warning), when the cabin altitude reached 10,000 feet (3,000 m) (which could have occurred in about 30 seconds).
    Investigations of other accidents in which flight crews attempted to diagnose a pressurization problem or initiate emergency pressurization instead of immediately donning oxygen masks following a cabin altitude alert have revealed that, even with a relatively gradual rate of depressurization, pilots have rapidly lost cognitive or motor abilities to effectively troubleshoot the problem or don their masks shortly thereafter. In this accident, the flight crew's failure to obtain supplemental oxygen in time to avoid incapacitation could be explained by a delay in donning oxygen masks; of only a few seconds in the case of an explosive or rapid decompression, or a slightly longer delay in the case of a gradual decompression.

Hypoxia, you just go to sleep and never wake up. This is so, so sad. I grew up a Catholic, but I'm agnostic. The air masks for passengers only last about 3 minutes, if they are activated at all. They don't use air tanks they use oxygen candles, basically when you tug on the tube it starts a chemical reaction that creates oxygen. The pilots have actual oxygen tanks however they are not automatically deployed so if they didn't know there was no pressurization then they wouldn't put them on.

    This just gave me an interesting idea - thinking of this and the later, very similar Helios Airlines incident. The problem in both was that the autopilot didn't know or care that the pressure had been lost, it just kept flying at it's set course and altitude until the aircraft ran out of fuel. A very simple device that could have saved both would be a pressure switch (extremely simple, $10 component) installed inside the autopilot box, set so that if cabin altitude goes above say 14,000 feet, the altitude hold will automatically set to 10,000. As this simplest version I think it would be hardly any additional cost. A more complicated version, which would work well in more modern aircraft where all the avionics are tied together, could first sound an alarm, and if the pilots don't respond will set the descent, send out an automated radio call on both the currently set and emergency frequencies saying that cabin pressure is lost and pilots are unresponsive, and set transponder to 7700. The next step after that, which I think many planes would already be capable of and could be integrated into all this if the pilots don't come to and intervene, would be for the plane to land itself. Self-landing if incapacitated pilots could be useful in a variety of circumstances, including this one. Maybe also activate if pilots don't do anything or respond to alarms for a certain amount of time, the aircraft version of alerters used on locomotives, though more sophisticated as all those do is stop the train.

    • @xponen
      @xponen 6 лет назад +1

      This idea is cool, but for whatever reason some people will dislike automation & oppose it. I also thought like you after watching a video about another crash, this one is caused by a pilot forgetting to lower flap during landing because he was busy fighting crosswind or something, I said "why not automate the flap? just lower flap automatically when landing, there's no reason not to" then people seems to not like automating stuff. There's even Lockheed L-1011 (an airliner) in 1970s that can land itself! but it fell out of favour. I think people are more scared of autopilot than pilot error!

    • @quillmaurer6563
      @quillmaurer6563 6 лет назад +2

      You make some good points, people seem to fear automation. Taking this a step further would be to simply automate everything - make a self-flying plane. There's all this talk of self-driving cars, I believe that's possible but extremely difficult. Self-flying planes would be vastly easier, and I would argue safer than human-piloted in most cases. Most crashes are caused by pilot error, not malfunctions, so a self-flying plane would, statistically speaking, probably be safer. The only problem is that automated systems are only good at dealing with things that are "normal," if it's not in their programming they don't know what to do, so they are less able to think of non-standard solutions to problems. A self-flying plane never would have landed in the Hudson River after loosing both engines, it surely would have crashed. But combining automation and humans, that could do quite a lot. They're already moving in that direction, look at Airbuses in particular.
      As for this auto-descent auto pilot, I don't think it would be too controversial, as it would only activate in abnormal circumstances, and the pilot would be able to cancel or override it. The idea is that it would activate if the pilot isn't conscious to do so. Auto-flaps I'd be a bit more concerned about, as if the flaps were to deploy when the pilot doesn't want them to that could cause problems. Same is true for gear, I've heard of a small plane with auto gear, it was deployed when airspeed got too low - thus when the pilot was struggling to get over a mountain pass, the auto-gear deployed, the last thing they needed in that situation, and from what I vaguely remember I think they crashed. Maybe a warning horn, similar to that used for landing gear, but people still land gear up occasionally even when there is such.

    • @woo9914
      @woo9914 3 года назад

      Aircraft rarely actually use 100% of their fuel tanks.
      They dont pull fuel out from the very bottom of the tank afaik because of potentially stuff getting into the tank.
      If they pulled fuel from the bottom of the tank, anything that got into the tank would be pulled into the engine, or block fuel flow.
      So generally, some fuel is left in the tanks.
      On smaller aircraft this will only be a few gallons.
      On a small jet, it could be a decent amount of fuel.
