I think the reason the dark brotherhood didn't attack back was because Doug didn't actually attack Astrid, so when he entered the sanctuary the game thought that he was doing the normal questline and then proceeded to skyrim itself so that they didn't care about dying

Since doug never personally attacked astrid, he never hit the "enemies with DB" trigger or whatever so the game saw him walk in without astrid's blood on his hands and said "ah okay so we're supposed to be friends here.." followed by a very confused "wait, what's going on again?"

Astrid dying in the shack triggers that quest regardless of how she dies. I think it's more likely that the AI was bugged bc of how many entities had been spawned.
