    Thank you for this video. I wish you were in Australia. Do you think placing the implants in your bra gives an accurate depiction of final result? I did this at my consult and was alarmed at how large all of the implants looked... it made me question how accurate this method of trying sizes is. Would really value your opinion on this. Thanks in advance!

    For patients after weight loss surgery, how do we know which implant to select? I lost my breast mass, i was cup C, but it is kind of flat now . What do you recommend for patients that gad weight loss surgery?

      The fullness up high comes from the implant, but it is not dependent on the PROFILE of the implant. I didn't discuss the low style implants because I don't use them. They look too flat in my opinion and give a really kind of pancake look to the breast which I do not think is aesthetically appealing. Some patients like that look and some surgeons specialize in that wide and flat look but I don't like that look so I don't use them.

      I don't do breast reconstruction anymore, but that is where the expanders are used. I don't have any to show or make a video. I am sorry.

    If someone had a BA 30 years ago under the muscle in an under the arm procedure. What surgical procedure would ideally be preformed to remove the old implants and insert new implants? The ones she has are saline not silicone. I have heard silicone feels more natural is that true? Would the new implants need to be quite a bit larger now than originally because the pockets around them have now been stretched out? Which do you prefer asthetically under the muscle or over the muscle, just curious?

    Can I please ask a question related to reconstruction surgeries, specifically the exchange surgery (replacing expanders with implants)? I just wanted to know if it's always true that the actual implant will be a bit smaller than the expander volume... I'm at 550 ccs on both sides (130 lbs, 5'7") and they look to me about 60 ccs larger than what I would prefer but my doctor isn't really discussing sizes with me; he says he needs to make the "pocket" as large as my skin will allow.

      I would love to make that video but I don't do breast reconstruction anymore so I am not really the one to make them. Good luck with your exchange. There is also a lot of fat grafting that gets done with breast reconstruction and that can change the look and volume as well.

    I'm not interested in a BA, but I am curious; do the implants that are HIGH PROJECTION come in different sizes? I imagine that they do. My sister had a BA back in 1996, should she change them out? She's not concerned at all, but I am. THX! You are such a great surgeon. you should be CLONED! lol

      Yes all the implants regardless of the profile come in different sizes from the 100 cc all the way to around 800 cc.

    Are all implants spherical? I’ve seen teardrop shaped ones- when would they be used?

      Tear drop implants aren't used anymore because they are textured and textured implants have been linked to a very rare cancer so at least in North America we use smooth, round implants.

    I have had my breast implants for 18 years ! I am healthy and have no problems with them ….
    I DON’T WANT another operation ; is it safe to KEEP them forever ?

      I advise my patients to perform breast self exams and get mammograms as suggested by their primary care physician and as long as their implant is not on the recalled list or a textured implant it is safe to have implants forever. If an implant is not causing problems than there is nothing to do. The idea that implants have to be changed out every 10 years is a myth.

    I have hp right now but wish to get a lift with smaller low profile..I no longer want a large size or large projection.
    but it sounds like you don't recommend that?

      I typically use a high profile for a lift, I think most of us do, but you can always select the style of implant you prefer. Not an issue.

      I didn't talk about low profile because I personally don't use them. I don't like the appearance of a low profile implant as I think it makes the breast look very flat but some surgeons who prefer that look certainly do use them.

    Hi Im currently at a 495cc high profile. I would like to get a lift with a lower cc with a lower profile. Would you reccomend that?

      Certainly if you want to have a smaller volume that can be done. In terms of the profile I would have to see you in order to comment but in general that can easily be done. I would sit down with your surgeon and discuss the look that you want and that will help determine the implant profile.

    Good explanation, thing is the high profile is more narrow at the sides so moderate plus profile xtra suits me better but I’m afraid that it will be too flat. I’m gonna do lift aswell

    What is it about a lift that makes high profile the best option? I'm 45 yrs old 5'8 123 pounds and have had 4 children. I am planning a breast lift with augmentation and was going to do mentor BOOST moderate plus profile, but now I'm re-thinking that. Thank you for your videos, by the way. Extremely informative and helpful!!

      It's usually because patients that will need a lift really don't have very much projection and so we use a high profile implant to give them more projection. For patients that have a mastectomy who have had breast cancer we actually use an extra high profile because now there is no breast tissue on the chest, and they need to have even more projection. Sometimes patients think that high profile means it's higher up towards the clavicle, but it's really not. It's just a projection from the chest outwards.

    Hi, I have a BA scheduled and I’m stuck on what cc to go for. I’m currently 32A in bra size, I’m 5’1, 108lbs and considered petite. I want to be a full C after my BA. I can’t decide if I should go with 385cc or 405cc. I like the look of 405cc though. Do you think 405cc will be too large for my body frame? What are your thoughts?

      Despite what you hear you really can't pick out any size of implant you want. I know that sounds terrible. Let me explain.
      You have to have an implant that will fit your breast. It is like buying a shoe. If you are a size 7 you can't go to a store and say you want a size 5. You could not get your foot into the shoe. Same with the breasts. The implant has to be able to fit in your breast.There are things we can do during the operation to allow for placement of a larger implant, but there are limits. The size of the implant we can place depends on your breast width, the softness of your muscle, the distance from your nipple to your crease under the breast, and the softness of your tissue and a lot of other factors. As an example, a patient who is 40 years old, has had 4 children, and has a naturally wider breast will take a larger implant to fill her breast than a 22 year old patient with a narrow breast who weighs 102 pounds, has never had children, has very tight skin and a very well developed muscle from working out.

    Hi Dr. William, I have a question about butt cheek dents? Is they any way to fix that? What would you recommend for me to get done to fix that? I’ve had them for over 20 years and I’m 34 years old and I work out and do all my butt exercise but nothing seems to work I’ve tried wood therapy also and nothing I have a lot of dents like cellulite on my butt cheeks and behind my thighs. Please point me to the Direction or even instructions of how to get rid of them or even fix them or what to get done for them to be smoothed out. Thanks in advance much love from Brownsville Texas 🏝

      I would need to evaluate you to really give you a good answer. Hip dips are filled in during a BBL but it depends on the tissue and how soft it is--soft tissue will allow for good correction but patients with very tight tissue can have a difficult time getting their hip dips filled in.

    What is the maximum cc volume that can be used for a breast lift with implants, I have some ptosis about a grade 1 and loose skin after pregnancies and weight loss, I have deflated by about 5 bra cup sizes. From all the information I can find about breast implants it says that the diameter should be the same as your breast width diameter or 0.5 to 1cm less, I have a very wide breast diameter of 16cm each breast (I’m currently a D cup size), I prefer the look of high profile implants, but from looking at the size charts of various implant manufacturers,, mentor and motiva, it appears to fit my breast width correctly the volume is over 700cc. Is it safe to have implants with that many cc if I also need a lift. Thank you in advance for any advice

    I have permanent metal screws in my ankle as a result of a foot surgery due to fracture. Am I a good candidate for TT or BBL or breast lift?

      No I only do a breast aug with a BBL; not a lift or a reduction. It's ok to lay on your tummy after a breast aug, but not after a lift or reduction (blood supply).

    I was suggested extra high profile during one of my consultations because I have thin breast tissue after having 3 kids and a narrow rib cage … now I’m hesitant

      Talk to your surgeon and figure out what is best for you. Remember this is just my opinion, and what I prefer, and the look I like, but everyone is different. Talk to your surgeon and figure out what is the best option in your case for the look you want.

      I can't say in your case but what I tell my patients is the same as your surgeon is telling you. I have my patients change to round and smooth if they have anatomical implants.. The anatomical implants are textured and we know textured implants have been associated with ALCL. I have not used textured implants now for probably about 8 or 9 years.

    Do you have any suggestions for trans girls who have more broad shoulders? What would look best and most natural? Im assuming a smaller volume and profile.. honestly i kinda just want them to help with the shaping etc

      Breast augmentation for trans patients has really become a specialty now and I have not done a breast aug on a trans patient for many years. I would find a board certified plastic surgeon in your area who specializes in trans patients because there is a lot to consider in terms of implant type, location, style and volume and that is best discussed with an expert who performs a lot of breast augmentations in trans patients.

    do you deal with patients that previously had augmentation with several complications, revision, and then finally removed. It left me looking cosmetically awful. Mostly due to a revised permanent crease that looks terrible. I had double bubble, bottoming out, capsulary contracture (these complications did not happen over time.... but were present directly after surgery in recovery room and was sent home like that besides the contracture). Had a revision in which the IFC was sutured up... left me with fatty folds on the sides and are at an angle. I have lived with this ugliness for the last 13 years and so much want to look normal and cosmetically beautiful with a new augmentation. It has been a nightmare having to look at myself like this for all these years... not to mention trying to be intimate. TMI i am sure, but that's my story.

      Gee. That sounds like you have been through the ringer! I would have to see you in person to see what options are available for you, but I certainly do perform revisional breast surgery. At my stage of my career I enjoy challenging breast cases and it sounds as if you would fall into this category.

    Can someone with breast reconstruction (mastectomy) receive a bbl and fat transfer to the breast from a physician who doesn’t perform breast reconstruction? I just wonder if I would need to see my physician for the fat
    transfer since she specializes in reconstruction then see another Dr for the cosmetic surgery?

      Definitely stay with your breast reconstructive doctor for fat transfer to the breast and that surgeon may or may not also perform a BBL but I would not suggest having fat grafted to the butt and the breast at the same time.

    What profile would you use for a benelli lift for tuberous breast? (To consider: I am totally flat chest with tuberous nipples)

      There is such a spectrum to the tuberous breast category that it's hard to say for sure but I will use either a moderate or a high profile depending on the patient. If there is really no breast tissue than for sure a high profile to get as much projection as possible but for women with much more volume and projection I will use a moderate or moderate plus implant.

    You say ultra high profile is really only appropriate for women who completely lost their breasts due to cancer, but what about women who never had them to begin with? It's rare, but some women are less than an AA cup and have virtually no tissue to cover an implant. Are those women just screwed and have no other choice than to look unnatural with implants? I guess the best option in this scenario would be implants covered with fat transfer.
