The doctor even killed his own wife when she begged him not to kill her. Shiki weren't undead, they were just transformed into new beings. The anime is supposed to show that the true monsters are humans who don't understand people that are different and don't want to understand that fact. TLDR: The anime is meant to show humanity can be hateful and act with bigotry, becoming the real monsters of this world.
