It is a challenge added on top of the hardest difficulty that exist on Ascension. Each trial level increases hp and damage further than the previous one. For example here, Trial 10 is scaling the regular Ascended difficulty values with 43% healing reduction, 260% hp increase, 345% physical damage increase and 245% magic damage increase to the boss. Some bosses still don't benefit much from such buffs while some others like C'thun become really hard (even tho the video doesn't make it look like it). We spent multiple weeks trying to kill the boss.
