Kneeling after Total Knee Replacement

  • Many patients have pain when they kneel after a total knee replacement. A study by Dr. Rich Berger that was published in the Journal of Arthroplasty in 2019 shared a simple protocol that helped get many patients the ability to kneel. I share this protocol with my patients and it has helped them.
    Each day you will spend 10 minutes doing the kneeling on different surfaces starting with soft and ending with hard.
    Week 1 - Kneel 10 minutes per day on the sofa
    Week 2 - Kneel 10 min/day on a sofa cushion on the floor
    Week 3 - Kneel 10 min/day on a thin pillow
    Week 4 - Kneel 10 min/day on a rug
    Week 5 - Kneel 10 min/day on a hard floor
    It is not to be started right away since you need time for your incision to heal. You must check with your surgeon to see if this is safe for you.

    I watched this video several times before I had a LTKR 1/15/ 2024. I was very concerned because I work a job that requires kneeling.
    About 2 and 1/2 weeks post op, I dropped something on the floor and it made a mess. My wife had already gone to bed, and I did not want to leave a mess for her to clean later.
    Without thinking I dropped to my knees and cleaned up. About 3/4 of the job completed I realized what I had done.
    I was pleasantly surprised my surgical leg wasn’t screaming at me and it wasn’t a huge deal. The lack of feeling was a bit weird.
    It wasn’t that bad getting back on my feet either.

    Hello Dr.Adam.Thanks for your advice. Im about to have double knee replacement and im 50yr old concrete placing male. Im a work horse with anxiety about everything. The good thing is their will be no more pain. God Bless Dr..33

      Oh ... I'm not sure about no more pain but less for sure and the secure feeling that you won't be dropping to your knees as you are walking.

    This gives me hope where I had previously just been told I won't be able to kneel. Thank you for making it clear that I won't have to start this exercise on Day One! My TKR is scheduled for six days' time (May 2023). I am pretty nervous of the pain to come! So glad my husband found your video; I'm going to catch up with the rest of them, pronto!     My surgery is coming up in around 3 weeks and I’m trying to do different exercises pre-surgery. I only found you, Dr Rosen, a few minutes ago but you approach to post rehab sounds doable and good to me! I trust my surgeon here in Kingsport TN. My pre-op is coming u soon and I wanted to watch some videos in order to know IF I need to ask him some questions plus understand what I can expect post surgery!! Your videos are very helpful. I might also add that I am an amputee and my only knee is shot, kapoot,etc so a bit of extra apprehension involved. So thank you for your straight forward, clear and concise advice!!! Barbara

    My surgeon told me it's a "use it or lose it deal". After the staples were removed, after I felt safe trying and after my oven element decided to "blow" I decided to try kneeling. I decided to try kneeling in bed. My thinking was if the pain is unbearable I could tuck n roll with no problem. It worked and I survived. It felt weird but all went well. I used a small cushion knelt behind the stove and fixed it. Even today 18 months later it feels strange but the Doc. may have been right. So far so good. Dave

    This is gold because, as a Muslim, it is my fervent prayer that i will be able to prostrate (with both knees on the floor) after my upcoming tkr. Prostration (with knees, face and palms on the floor) is a part of the Islamic prayer. Thank you for clarifying that week 1 of kneeling practice should not start until 3 months out with the surgeon's approval.

    Thanks so much mate. After a double TKR I asked my surgeon and he responded in that ' No Arguments' voice no kneeling for a year. This led me to just avoiding kneeling for two years, except lately and gently on a soft yoga mat. You give me confidence - and a good plan.

    I’m at almost 12 weeks and I use a thin pillow still. It’s fine! I’m going to start working on it daily. I got clearance to kneel by my surgeon and PT at 6 weeks. 😳

      I have heard so many negative thoughts about how my knees were going to respond to my active life style. I really welcomed reading thus. Shortly I hope my right will be done. In the meantime I am exploring what I can do with my new left. I hear constantly that I will sink to the bottom of the pool and I will never swim again. Please put forth your info on swimming. Thsnks

    Great video. Yes, Dr. Berger is renowned in Chicago. He did my friends hip replacement and had fabulous results. Interesting, apparently he doesn't take medicare and some other insurances and must be of proper BMI to even consult with a patient.
    He is allegedly the top hip and knee surgeon in the "world".

    Thanks for this video. I'm 6 mos PO LTKR and find it very uncomfortable to kneel, esp on a rug on a hardwood floor, and need to readjust or put most of the pressure on my non-surgical knee. On the floor, I can manage just a few seconds of pressure while, e.g., on a bed, I can kneel for a longer period of time but it feels strange and I get worried if I kneel for too long.
    For the weekly protocol, is there a specific form for kneeling we should use, e.g. on all fours or kneeling as if in a church? Thanks!

      Talk with your surgeon. For most of my patients it is usually around 3-4 months due to sensitivity, pain and wound healing.

    I had both knees done 6 mos apart 6 yrs ago. It is so painful to put any pressure on both knees, otherwise all's good. Never heard of this desensitization process. It's changed what I can and want to do. Could this still work now? I'll try but very nervous. Thank you for this info

    I was not able to kneel back before surgery, I am interested to see if I can someday. I have dreams that I am doing childs pose! I am 14.5 weeks postop of a tkr on left side. My quads as well as hamstrings and it band feel really tight when bending. I am currently at 116 range of motion. Also thank you ever so much for your videos. My surgeon told me when I saw him at 10 weeks I did not need to see him again, unless I had a problem. 😊

    I couldn't kneel on a hard surface before surgery. Can kneel on a rug with the left knee but not the right. The difference is the implant.

    Wish I knew this. My knee replacement is 5 years old. I can’t kneel as it’s very painful. I wonder if 6our 8th a would work on an old knee rep.

    I had my right knee TKR on 20th May 2022.
    My Surgeon is happy with my 90° bending and wants no more bending,Reason for his comment is this, ' the implant may loose its longivity '!!
    I feel i could kneel.
    My thighs muscle became week before surgery as my knee suffered acl injury for 26 years. degeneration is in the MRI report.

      That’s contrary to everything I’ve been told. My TKR was April 13, 2022.
      Get a second opinion. Or a 3rd. Dr. Rosen seems like a great 2nd!

    Hi Dr. Rosen, by kneeling, I wonder if you refer to the Japanese style of sitting called Seiza 正坐 whereby the body sits on the calves and the angle of knee flexion is almost zero. I wonder if that's achievable at all, with practice? I am considering getting a TKR for one knee which has a bone-on-bone medial condyle. I can kneel with genuflexion of 90 degrees and I am having stability problem but not much pain except after extended walking or on getting up after sleep in the morning. It's more stiff than painful. Do you think I need a TKR right now or I should wait? Also, I read that a partial knee replacement on the affected side is better than a total one. Is that correct? Thank you.

    At 6. weeks, I was kneeling on a gardening mat. Recaulked the shower. May have to try these exercises. Kneeling feels odd and tender. I’m 10 weeks out now. Can walk 3 miles.

      Well ur amazing. My scars /incisions still hurt. I'm I'm 2 and a half months out on one knee and 4 + weeks out on the other knee. Can't kneel yet.

    In to my 14 years and I hurt worse than ever., He was never u front with me😅. He is known as the best.I bet if they ask the tight ones.

    When you say 10 minutes a day, is that 10 minutes all at one time, or can it be divided into two 5 minute sessions?

      I have some patients that can only do it for 5 minutes and others that break it up into two sessions.

      I disagree. It is simply a matter of desensitizing your new knee. I have done it. I didn’t use this method,but it sounds like good device.
