When your doctor clears you to drive
Good information. I’m pre TKR surgery and am having difficulty getting in and out of our low sedan. I have to lift my legs out one at a time using my arms, probably due to weak quads and painful knees. No problems when I’m actually driving but hoping the surgery will restore ROM and PT will strengthen my quads.

Good topic. In my case the surgeon told me the physical therapist would be the one to clear me to drive. Of course, the physical therapist told me that the surgeon would be the one to make that decision. So I compromised and made the decision myself. Fortunately I live in a rural area with little traffic. My first drive was to my 4 week physical therapy appointment. I managed fine. Part of that decision was that it was quite difficult to get into and out of my wife's car which is much lower than mine. So I drove my car and had no issues getting in or out largely because it sits much higher than my wife's car and the doors are bigger. I had a right knee replacement and was completely off narcotics at that point. Another factor is that even after the right knee replacement, I still had a left leg that could be used for braking if I needed it.

How long did it take you to get back to driving? And which knee did you have replaced?
My first knee replacement (left knee, late August 2023) I was cautious and had a person drive me to rehab two weeks. The second knee replacement (right knee, late August 2024) I didn’t have anyone staying with me so I only went out for physical therapy and had drivers to and from rehab for a week and half).
I followed my surgeon's instruction to wait 6 weeks before I drive after my right TKR last January. I will have my left TKR in about 5 weeks and, again, I'll take my surgeon's recommendation.

I am doing the exercises-not sure I am getting any better-I am one month and start using a cane so hopefully 🙏 I improve but my knee is totally stuck-Stay well!I see people at therapy place and they are doing great not sure why I am so behind the others at some point doesn’t make sense I practice my exercises every day .
Unfortunately, cries for help often go unnoticed in the hustle and bustle of life. Even the people who care about you most can write it off as a small thing. You have to be adamant in your self advocacy, which is very difficult when you are considering suicide.
I had a partial knee replacement that lasted seven months. I suffered for two years with a loose tibial component. I had a a revision to a total knee replacement. I was discharged twelve hours after surgery. My pain relief at home was totally inadequate for the first week. It was pure hell triggering a depression and PTSD. BTW I also developed Erythomelagia post-op. It’s an incurable neuropathic disease without any good treatment. I traded one form of pain for another. Just think really hard, and consider if it’s for you. One way or another it will change your life.

I had my right hip replaced first week in November, on the 4th day I almost started crying because I was pain free. Best decision ever. Hope all goes well for you as it did for me.

Had my TKR on Halloween 24, doctor prescribed ROMTEC it’s a similar device .Insurance covered it. It was the best thing I could have done, My range of motion at PT on December 9th was flex 145, straight -2. I know everyone is different, but the pedal bikes should be used if available.I’m walking without any assistance like a cane. I believe it was the bike that sped up my recovery.

I had s RTKR in June, 2024. I'm actually really starting to feel much better pain- wise now and have great ROM, although there is still some stiffness at times. The pain is very slight now but i swore I'd never do this again, it was one of the worst pain i ever felt. I had a cortisone shot in the Left knee mid October that really helped with the pain tremendously. S side effect i experienced was that my BP readings spiked ( as they did with celebrex taken for a month post -op as ordered). My meds had to be adjusted as needed after this. My cardiologist said i was sensitive to the cortisone and if I needed to get it again at some point to be aware of this so i could plan ahead. I know this is a long post but it may be helpful to someone else. Thanks for all your helpful videos and your book too!😊

How about insufficiency fractures ?? Are they treated differently?
It depends. Some stress fractures of the neck are protected with screws to prevent it from displacing. If it is an insufficiency of the head without collapse many of those can get better with time and protected weight bearing.

I am 2 years out from TKR on my Left Knee and 1 year out on the TKR of my Right Knee. I wore Offloader Braces for approx 4 years before the Left TKR and continued the brace on the Right up to the 2nd Surgery. Although I still have some residual stiffness, flexion approx 112-116 degrees, the positives outweigh the negatives. 1) The TKR’s addressed knee conditions that would have only gotten worse if left untreated. The very noticeable VARUS deformity in my left knee pre-TKR X-Ray is completely gone. Nerve pain from Right Knee to Ankle is gone. My knees no longer wake me up at night. I handle stairs better going up & down. 2) Lower back pain and sciatica, that I suspected may have been from a bulging disc, may actually have been from spine alignment issues related to the knee arthritis. I experienced several lower back flare-ups during my right knee rehab. Each flare-up moved a little higher up the back each time and decreased in intensity. My back now feels better than it has in a long time. 3) I think the braces used over 4 years may have led to some muscle atrophy of the quads, calves, & shins. Some slight weight gain(approx 10 lbs) has accompanied the rebuilding of muscle & tissue in those areas. I have workout routines that focus on Total Body & Core 3 nights a week. I’m mindful not to overdo anything, yet I expect steady improvement in the way my knees and related areas, especially back & hips, move as compared to my pre-surgery days. No Regrets.

As physical therapists we disliked bilateral replacements. To get to standing the patient can not stand up on their own. We literally had to lift them from sitting and they many times could not bend either knee enough so their feet are out in front. And it seemed they would be 250 lbs or more. It was scary as if they buckled it could be catastrophic. These patients did not have a good leg to stand on. Stairs were usually a no go.
Thanks for sharing. Yes, we have scoring systems on who to pick for outpatient surgery but it was always a judgement call for doing bilats. And some surgeons were more in favor. I saw what you saw and rarely did bilaterals except in super fit, young, strong motivated patients or those who had severe angular deformity.
@ Thank you for your response. A scoring system would have been nice. A patient that was sedentary ie lift chair to bedside commode and 300 lbs probably should score low? Not all PTs can deadlift 300 lbs.

Your discussion on the evolution of knee prosthesis design is fascinating. How do you see advances in material science influencing future developments in implant longevity and performance?
I don't see how it could go otherwise. It's a savage operation that doesn't even last very long. They chop off your thigh bone and attach a steel rod with a plastic knee attached. How's that supposed to work? Your leg looks like the leg of a mutant humanoid. I've heard that it's the most painful operation a person can have. I think anyone who has this operation is crazy.

So at a week and a half im 5 degrees and 116 bending im walking going up and down stairs. No cane ever walker for 3 days. I work construction so my muscles were strong ahead of the surgery. Yea pain is there but getting better. My goal is full bending of my knee !

@dradamrosen I went in fully ready to get it taken care of , no fear well informed about all parts of the surgery. Plus self employed carpentry business that i have to run so no babying myself. 10 day out im driving. And no more home Pt. now 2 days a week outside pt. One thing i do have is nerve firing on and off. I guess they are trying to wake up and find there way home!
Sir,Which knee implant is best whether stryker or meril maxx freedom?
We did a small study between 4 implants and found no difference in outcome, range of motion or pain. I am not as familiar with the maxx but no one has proven a 'best' implant. Stick with the big manufacturers.

My right knee looks like it has been run thru a shredder! Hurts and makes my leg swell.
I had so much soft tissue inflammation from the tourniquet. My thigh turned black from the amount of bruising . I was in perpetual hell !!!!!! I've never been in so much pain . I do not think I will ever agree to this surgery again. I was not told anything about what to expect. I know that's my fault . Now i know everything . I'm 5 weeks out, and I'm finally feeling better. Those first few weeks were off the chart pain wise . Nothing could touch it . I couldn't sleep because I hurt too bad and going to the bathroom was excruciating !!!! PT was horrible and I really wasn't sure about the future. I am now able to go half way up my stairs and I'm getting stronger everyday but it's been one hell of a ride . Good luck to anyone about to have surgery you are in my thoughts and prayers 🙏 ❤️

I do not regret having both knees replaced. Both were brutal at first and my knees were wrecked per my surgeon. PT and faithfully doing it was the key. Also had dental implants and that too was rough - the first ones had to be redone several years later even after being told they would last a lifetime - so yes I feel for you on your journey
Dr. Rosen: Prior to my TKR’s, I found your videos extremely helpful in preparing me for the surgeries and the recovery …I’m 11 months out from the left knee and 8 months from the right…in each case, the first 4-5 weeks were challenging especially at night, but not unexpected…PT and using the bike as you suggest in your videos helped improve knee flexion…I no longer have the arthritic pain and climb stairs normally…by the summer, I was recovered enough to get back into the yard work routine and other activities that we get into around the house…as time goes on I’m sure the knees will continue to improve…TKR’s, of course, will never be equal to our natural knees, but I would say it’s an 8 of of 10 in my case…thanks again for sharing your knowledge and helping everyone deal with all aspects of a TKR!

I had a serious injury that eventually led to replacement (TKR). So my recovery doesnt look textbook. And I'm younger than most. Hopefully I will get there.
My left TKR was done a year ago . I have no issues with it now, and I am very glad I opted for the surgery. Now, if you had asked me that question at week 2, I would of told you I will never have the other knee done. Well, I had the right TKR sept 15th and am so glad I had the right knee done also. I had a lot of stiffness but last week it finally subsided. Thanks for sharing you informative videos, Dr. Rosen.

When cleared for WBAT, and patients experience excessive pain during continuous stepping (that which therapists persistently instruct to do)--(as opposed to offloading the leg, one step at a time), could this be contributing to persistent pain & contributing to delayed discharge? Put another way, do post op knees need sufficient offloading for proper healing, and for more adequate pain control.
I had my TKR on October 22. Today I walked 2 miles at 2.9 miles per hour rate. Going down stairs is still a struggle. Overall, I'm much better than before. I've had over 5 surgeries on that knee and this was the last resort. The first 2 to 3 weeks was brutal but I worked hard to get my flex to 130 and 0 today. It is still achy though.
My surgery was Oct 29 Congrats to you. Did you walk the 2 miles without a cane? What exercises were u doing
@tsmith3745 No cane. I haven't used a cane since week 3. I walked 2 miles in 44 minutes. I was very active before the surgery up until 2 months prior. I did yoga, walking and riding a stationary bike before and I am doing them again.
Im 6 weeks out from TKR and had 130° by 5 weeks. I did the exercises several times a day from about day 5. Prior to that, it was too painful. I used an ice compression sleeve for 4 weeks. Called Game Ready it truly is a game changer. They put the sleeve in in hospital post op and i had virtually no swelling at all. Its amazing how much heat the knee is generating . That icevand compression combined was amazing for pain relief as well. I feel so lucky to have had no swelling, which has helped me achieve a good ROM so quickly
