I think we do a disservice by referring to these as "back injuries" (which sounds like "oh a pulled muscle") when in fact most people who have what we call "back injuries" have what should be called "spine injuries" because that is what it is -- I find that when I tell people I have a "bad back" they shrug, when I describe it as a "bone spur pinching my spinal cord" they look horrified and have infinitely more sympathy for the situation

Years ago i had severe back pain. I had a broad based disc herniation at L5S1 that was causing bilateral sciatica, i had treatment here in the US going from doctor to doctor and nothing helped. Anyways i knew i needed surgery and paid out of pocket in Guadalajara Mexico. I didnt do major surgery but i got Ozone intradiscal injections directly into my herniated disc which reduces the size of the herniation thus relieving nerve pain. I paid 6k for a total of 4 injections and also the doctor removed nucleus material using striker decompressor. Pain wise im at 2-3 from being at 10+ pain for 4 years. I hurt my back in 2010, in 2014 i got my procedure done in Mexico. Its 2021 and its still holding up. I was able to go to college and been working a full time job. I had to stay in Guadalajara for over a month for the treatment but it was worth it. The intradiscal ozone injections were done weekly.
at 29:25, Dr McGill says: "do not flatten your back on the floor". What about in bed? I usually sleep in various positions during the night, which are both sides and on the back. In that position, the back is always flat, even with a pillow under the knees, and that hurts/spasms. Any hints?
As you comment positions in sleeping is a topic in itself.. And you want control it as you s.l.e.e.p A conondrum.. Can lead to injuries in REM sleep as the muscles are paralyzed... I am trying the belly position and noticed that training my neck muscles have made this position safer in Rem-sleep

Dr McGill, you'll probably never read this, but I'm reaching out anyway. My situation is that I have a lower back that was injured a few years ago, helping to pick up a patient in the hospital where I was working . I saw a local Otrho Doctor and received steroids and PT. After the last few years I've dealt with off and on horrible back pain. Approximately 21 days ago, I somehow reagrivated it again. This time the pain was different. Excruciating pain and numbness on my left side when walking, sitting and lying in bed. Spoke with a Ortho surgeon in Nashville, but my chances of getting back surgery are months out. Trying to manage the pain with over the counter meds and the PT advice I've found on RUclips. I worry about permanent nerve damage if the nerve pain doesn't subside. Any advice on how to manage this would be so greatly appreciated! Thanks. Gary from Tennessee
Very interesting video. I had a diagnosis of tethered cord syndrome and had surgery with excellent results. However the problem reoccurred 23years later. The "neurosurgeons" looked at my MRI and never saw me. I got turned down. Today I have no bladder and bowel function and I'm in a lot of pain. I'm bitter and feel sad about the whole thing.

Watch Dr McGill’s interview with Brad from Bob and Brad; he explains it with greater detail. Essentially this movement is intended to teach your body how to employ better core stability and technique when moving, lifting etc.
Doctors have no intention of finding the source of back pain. Their STANDARD OF CARE is a cookbook approach as approved by the College of Physicians, the Ontario Medical Association and the Canadian Medical Protective Association. Any doctor who deviates from this Standard of Care is chopped liver and lives in fear of retribution. The best book to read and essential to read on this is "Medical Nemesis: The Expropriation of Health" by Ivan Illych. Much more sophistication on the part of the public is needed to survive the Medical Industrial Complex. Every citizen should buy, read and apply Stuart McGill's book, "The Back Mechanic". It's a life-saver.

Hmmm .. disagree with the title to a degree. We can't quantify the components of a placebo effect, but we don't state there is no placebo. Likewise, we can rarely identify the specific cause of back pain, even imaging has shown to be largely unreliable in this instance, only aggs/eases/lifestyle/directional preferences and "calm things down" then "build things up". Therefore, Non-Specific Low Back Pain is all we have (~90% of back pain), in the research, this is where we are at. McGill points out some interesting things, but also some things I disagree with coming from a research background. Pain can very much be caused by non-physical mechanisms, pain is a danger reaction to stimuli, if an initial cause is not managed effectively with cofounding psychosocial factors, then the threshold for "pain" neuropathways decreases overtime. "Neurotags" become more active. Many of these tests done by McGill should not form your clinical reasoning, only part of the picture pragmatically as aggs and eases, they have not been quantified in research, the closest thing was Hodges work showing abdominal bracing in shoulder movement and this was grossly misinterpreted during the trans abs fad, even then the difference was 200ms. Again, interesting view and great to see a clinician with years clinical experience rationalising some common problems, but you can't help feel there is a large part of the pie missing in this approach. I guess you gotta learn from McGill & O'Sullivan and take the best from both. They clearly both have their own biases, but I feel O'Sullivan is more evidence informed than McGill.
I'm going to make two guesses: You haven't read much of Dr. McGills publications and you're a physiotherapist. I've read both McGill and O'Sullivan and I find McGills work has far more compelling evidence than O'Sullivans. I would also assume that the 90% of non-specific back pain reported in research, as you stated, is due to the fact that the researcher or clinician did not have the insights that Dr. McGill provides to diagnose the specific problem.
Simon why is my profession pertinant to this debate? How much do you clarify as adequate? I have read into what he discusses in this lecture hence my initial opinion. Just clarifying here, you believe Dr. McGills research or points are superior due to his specific insights. If this is correct and I haven't misinterpretted your sentence. What "insights" are you referring to that Dr. McGill has that other researchers/clinicians do not?
Because most physiotherapists I came across follow O'Sullivans approach. That gives me a feeling you're biased. McGill combines qualitative and quantitative study, in vivo and in vitro, to form an insight. To then test these insights on different population groups. The data and results have more weight to me than that of O'Sullivan. Why do you think O'Sullivan is more evidence based? An example of a misinterpretation by professionals and clinical research groups: The overestimated importance of Transversus Abdominis. Research based on this assumption could influence results. You deleted your other comment, maybe for the best. This is a comment on a RUclips video, not a debate.
Simon blending several low quality methodologies is still low quality. O'Sullivan and his research group do utilise qualitative research, you may not be as well read as you believe. Trans Abs fad is over, many Physiotherapists who are taught to critique evidence including myself are fighting against this approach. O'Sullivan has revoked a lot of what he wrote about stability and follows a new BPS evidence based thought process called Congitive Functional Therapy. Not afraid to compromise reputation, O'Sullivan came out of a rabbit hole which he made his name on, in sight of progression. McGill makes overstatements about how he can specifically Dx LBP by finding aggs and eases essentially? This is a pretty inaccurate report. Pain correlates poorly with damage, pain is not nociception. McGill makes sweeping statements with poor weight in evidence. A LOT of what he has to say is along the right lines (from what research tells us). Doesnt mean some things are not hence my original comment. A retort and discussion about a topic on a youtube video is not a debate? I'll admit I deleted my comment. Originally compromising my professional attitude to stoop to the level of generalising a profession or inferring degredation of opinion/expertise when someone does not agree with your own narrow thought process.
Low quality methodologies? I find objective emperical data gained through a biomechanical approach of much higher quality than data aquired by subjective questionnaires like in Vibe Fersum, et al.(2013). Efficacy of classification‐based cognitive functional therapy in patients with non‐specific chronic low back pain: A randomized controlled trial.). The study even states that the pain was provoked and relieved by posture, movement and activities. All mechanical. Since the cause of pain is mechanical, does it not make sense to pursue a biomechanical approach? I do not disagree with the impact of cognition and behaviour but I miss the biomechanical data to support O'Sullivans claims. Take a look at the exclusion criteria of the study I cited. One of them is this: Acute exacerbation of LBP at time of testing in order to avoid regression to the mean. So every person that experienced pain because of treatment was cut from the dataset. No shit it was succesfull. Other exclusion criteria just show that this approach is not fit for serious back disorders, only a specific group. What are you going to do when have patients who are not in this group? "O'Sullivan has revoked a lot of what he wrote." This does not give me great confidence. You are trying to hard to be the better man. You insulted me in your deleted comment and now by calling by thought process narrow. I just guessed your profession, and man, you do not like to seem that at all.
Obviously Dr McGill thinks sitting at a desk causes back pain. But even if I retire, how can I avoid sitting at all? What am I supposed to do for the better part of the day? Stand all day? Sitting in a recliner causes more backpain. So, I am not getting this answer from Dr McGill's videos, what are we supposed to do all day if not sit. How about just lying on bed? does it help?
Not sure but I'm just finishing his book Back Mechanic, he gives you a plan in the book and goes into the stuff in this video in lot more detail. The core plan is the Big 3 Exercises, an exercise called camel (or cat-cow) pose and walking. Then he has some more advanced stuff to try later on, like working on your hips and some specialized exercises for certain conditions. I definitely recommend the book, it is full of useful advice.
@@ObscureStuff420 I purchased it with the videos a few years ago as well. It makes sense but I think every back issue is different. Some exercises hurt some don't. People have to figure out what works for them. But it all comes down to finding the real cause of your pain and I think McGill's approach is one of the best and surprised most back doctors and surgeons don't do the same.
Dr Stuart. I'm 40 years old. I have degenerative disc since I was 16 my last MRI was 2 years ago. In waiting to talk to a surgeon. I'm not sure if other part of my body a weak couse my back more pain,like the Clute min.degenerative disc can heal if seft?
Dear Prof McGill, I had severe lower back pain and admitted to hospital. According to my surgeon my MRI showed I had cervical spinal stenosis and must have major surgery immediately. Is cervical spinal stenosis a specific name for lower back pain or another name for the my pain? May I have your kind expert advice, thank you.
Wow, you literally did no research on your diagnosis to ask this question. Severe spinal stenosis can cause permanent disability leading to paralysis. The techniques he shows here would not address it or prevent the progression of stenosis.
@@rooscous8657 Of course I know cervical spine is neck (C1-C7) Thoracic is T1-T12) and Lumbar is L1-L5). My complaint was lowewr back pain (Lumbar L5-S1) but my surgeon's diagnosis was cervical spinal stenosis and the surgery was on the neck which I suffered no pain at all. That's bullshit. It had been six years ago and I'm experiencing no pain on my spine at all. I am 84 years old and still as fit as a fiddle. I am allso practicing Intermittent Fasting 16:8 protocol and Wim Hof Method of breathing exercises.
I think tms is canceling all typsed of doctors and physio therapists, so I believe it goes other way around too. But in my personal case tms has much more sense. With all respect to all these wonderful materials and if they help people, to me it doesn't make sense for a human body to be in so much everyday pain because of I don't know what, since we all have different bodies and habits, Drs often mention sitting long whilst I have not been sitting more than hour a day for years, always active, worked with different trainers etc and actually had Mc gills exercises as part of my workout routine. But as I say, whatever helps you to get out of thebpain is great.

Another idea look at the SI joint.. this is sacrum region.. Doing things for the SI.. Ex. Hinging at the hips and alternatively full extend one knee, letting the other bent: the Hams pull at the SI sides Doing scissors with straight legs in supination.. (I got twice pain at my SI so I have experimented these things. Am also 68 of age) May be right or not right for you Each person has different issues. I have experienced that stretch is not helping at all: it works as a stressor. Better massages

I've hurt my rib sneezing. I recently hurt my back. prior to my injury I had been DLing and Squating for 1.5y. I think the most interesting thing so far (not finished yet) has been him describing hip hinging, using a valsalva, and the bow to do things to avoid pain. when I injured my rip I naturally knew to use those movement patterns. they were already ingrained into my body, and the fact that I could DL and Squat with a rib injury showed me that those movement patterns were safe and helpful. with my current back injury, hip hinging to get stuff off the floor is crucial. so is putting my foot on a chair, or opening my legs very wide while sitting so I can keep a neutral spine while leaning forward. I brace against my knee with my elbow while lifting plates off the floor. unfortunately I've lost a lot of strength, but I'm still DLing and Squatting, and working back up. I just think it's interesting that these movements patterns were so intuitive and natural for me. And as far as sitting goes, sometime after learning to squat I realized I changed the way I sat in chairs. it made me wonder if anyone else noticed that I was squatting to the chair instead of flopping down on them. hope to be pain free again, though I'm kinda saddened. I understand that once you've injured something, you'll eventually heal, but it'll likely become a reoccurring issue.

My wife has osteopenia in her spine. No curve in neck vertebrae. c5-6, T9 vertebra fracture compression. Near normal intervertebral spaces and nrve channels. MRI revealed bone density eas not strong enough to place screws. How do we increase bone density? Eats lots of cheese, yogurt, Calcioum with D3 600 mg daily. Starting Magnesium to increase Calcium absorption. Is this aqpproach right?
Well my wife has ostheopenia, which makes her spine bones more fragile. She has stem cell treatment of upper back and neck disks. Little reduction in disk spacing between vertebrae. T9 has fracture by compression. Spondyliosis was diagnosed and possible spasm in neck muscles to sustain vertebrae. Has no curvature upwards in neck.
Don't just rely on nutrition alone, if possible add some type of movement like walking or resistance training. I wouldn't recommend cheese due to the fat content. Low fat dairy would be better
She has a breakfast of two to three ounces of cheese plus the 600 mg of Calcium, plus D3 4000 IUs, Magnesium 400 mg. The maximum absorvable calcium at single time is 600 mg. So she is wasting the excess. She also takes Omeprazole 30 mg to reduce acidity needed to digest calcium. She doesn' t walk too much either on regular basis. Thanks for your advise. Blessings

I mean I am serious I’m asking this question because I’m doing a lot of physical therapy right now because for six months I’ve been in pain and couldn’t move for three months out of that six months and I’ve been doing a lot of physical therapy at first and physical therapy hurt me and I did what you said I just laid down and took it easy for a few months when I felt like the inflammation was under control I started doing stretches that are calledThe V stretch the W stretch the tea stretch in the L stretch and I believe that has helped me tremendously to put my body back in better posture by trying to put my shoulder blades down /back It really has seem to help me with my neck issue believe it or not

When you are 48 years old and you have 4 discs that are virtually non-existant, almost completely gone, with absolutely no reason whatsoever (no overuse, no accident, no discernable cause).... and you are in extreme pain, what do you call it ?
Read the publication of his paper with a colleague on back pain during coitus (females & males studied); the quadruped position ("plank" on elbows; female coitus position) may help us inform as to whether plank position is stabilising or not. Hope that helps - I think it it was in the Eur Journal Spine.

Back problems are sorta like the common cold. The medical establishment hasnt made much progress in dealing with the problems. Every few years they come up with something like fusion,or little rubber inserts but they get disproved eventually Still in 2021 surgery is 50 50 not much progress. Like the cold

"when you're flexed forward, it pushes the gel (nucleus pulposus) out the back" This kind of outdated information is not only proven false, but is also a major part of promoting fear avoidance behavior in CNSLBP. The back is our strongest bodypart and there has not been found any correlation between physical activity/conditioning/back posture and chronic low back pain. McGill has a lot of good and interesting points, and give good guidelines on how to avoid pain in your everyday life. But he is also creating more fear of movement and fear of pain, which really only enhances the pain in the long term. He even claims that it is proven that forward flexion causes CNSLBP, when in reality this claim has been dismissed by overwhelming evidence. However his way of working with patients appears caring and patient centered, and the most of his points are well founded. It's can just be very damaging to spread this kind of misinformation.

If by no such thing as non-specific back pain one means there is always a physical/mechanical mechanism, not sure anyone would argue. However a situation in which one's physical response( e.g. chronic guarding) to emotional/psychological stress creating a soft tissue environment that triggers pain, that is entirely different from suggesting one's pain is all in their head. Huge distinction, imo. Great, highly informative, presentation otherwise.

I think we do a disservice by referring to these as "back injuries" (which sounds like "oh a pulled muscle") when in fact most people who have what we call "back injuries" have what should be called "spine injuries" because that is what it is -- I find that when I tell people I have a "bad back" they shrug, when I describe it as a "bone spur pinching my spinal cord" they look horrified and have infinitely more sympathy for the situation

Excellent info. I've personally discovered bending from the hip to brush my teeth greatly relieved my lower back pain. This video help me undersrand that it should be incorporated in my everyday movements. I work in a warehouse lifting an average 30 pound boxes. So far so good.

Years ago i had severe back pain. I had a broad based disc herniation at L5S1 that was causing bilateral sciatica, i had treatment here in the US going from doctor to doctor and nothing helped. Anyways i knew i needed surgery and paid out of pocket in Guadalajara Mexico. I didnt do major surgery but i got Ozone intradiscal injections directly into my herniated disc which reduces the size of the herniation thus relieving nerve pain. I paid 6k for a total of 4 injections and also the doctor removed nucleus material using striker decompressor. Pain wise im at 2-3 from being at 10+ pain for 4 years. I hurt my back in 2010, in 2014 i got my procedure done in Mexico. Its 2021 and its still holding up. I was able to go to college and been working a full time job. I had to stay in Guadalajara for over a month for the treatment but it was worth it. The intradiscal ozone injections were done weekly.
at 29:25, Dr McGill says: "do not flatten your back on the floor". What about in bed? I usually sleep in various positions during the night, which are both sides and on the back. In that position, the back is always flat, even with a pillow under the knees, and that hurts/spasms. Any hints?

Dr McGill, you'll probably never read this, but I'm reaching out anyway. My situation is that I have a lower back that was injured a few years ago, helping to pick up a patient in the hospital where I was working . I saw a local Otrho Doctor and received steroids and PT. After the last few years I've dealt with off and on horrible back pain. Approximately 21 days ago, I somehow reagrivated it again. This time the pain was different. Excruciating pain and numbness on my left side when walking, sitting and lying in bed. Spoke with a Ortho surgeon in Nashville, but my chances of getting back surgery are months out. Trying to manage the pain with over the counter meds and the PT advice I've found on RUclips. I worry about permanent nerve damage if the nerve pain doesn't subside. Any advice on how to manage this would be so greatly appreciated! Thanks. Gary from Tennessee

Very interesting video. I had a diagnosis of tethered cord syndrome and had surgery with excellent results. However the problem reoccurred 23years later. The "neurosurgeons" looked at my MRI and never saw me. I got turned down. Today I have no bladder and bowel function and I'm in a lot of pain. I'm bitter and feel sad about the whole thing.

Watch Dr McGill’s interview with Brad from Bob and Brad; he explains it with greater detail. Essentially this movement is intended to teach your body how to employ better core stability and technique when moving, lifting etc.
Doctors have no intention of finding the source of back pain. Their STANDARD OF CARE is a cookbook approach as approved by the College of Physicians, the Ontario Medical Association and the Canadian Medical Protective Association. Any doctor who deviates from this Standard of Care is chopped liver and lives in fear of retribution. The best book to read and essential to read on this is "Medical Nemesis: The Expropriation of Health" by Ivan Illych. Much more sophistication on the part of the public is needed to survive the Medical Industrial Complex. Every citizen should buy, read and apply Stuart McGill's book, "The Back Mechanic". It's a life-saver.

Hmmm .. disagree with the title to a degree. We can't quantify the components of a placebo effect, but we don't state there is no placebo.
Likewise, we can rarely identify the specific cause of back pain, even imaging has shown to be largely unreliable in this instance, only aggs/eases/lifestyle/directional preferences and "calm things down" then "build things up". Therefore, Non-Specific Low Back Pain is all we have (~90% of back pain), in the research, this is where we are at.
McGill points out some interesting things, but also some things I disagree with coming from a research background. Pain can very much be caused by non-physical mechanisms, pain is a danger reaction to stimuli, if an initial cause is not managed effectively with cofounding psychosocial factors, then the threshold for "pain" neuropathways decreases overtime. "Neurotags" become more active.
Many of these tests done by McGill should not form your clinical reasoning, only part of the picture pragmatically as aggs and eases, they have not been quantified in research, the closest thing was Hodges work showing abdominal bracing in shoulder movement and this was grossly misinterpreted during the trans abs fad, even then the difference was 200ms.
Again, interesting view and great to see a clinician with years clinical experience rationalising some common problems, but you can't help feel there is a large part of the pie missing in this approach. I guess you gotta learn from McGill & O'Sullivan and take the best from both. They clearly both have their own biases, but I feel O'Sullivan is more evidence informed than McGill.
I'm going to make two guesses: You haven't read much of Dr. McGills publications and you're a physiotherapist. I've read both McGill and O'Sullivan and I find McGills work has far more compelling evidence than O'Sullivans.
I would also assume that the 90% of non-specific back pain reported in research, as you stated, is due to the fact that the researcher or clinician did not have the insights that Dr. McGill provides to diagnose the specific problem.
Simon why is my profession pertinant to this debate? How much do you clarify as adequate? I have read into what he discusses in this lecture hence my initial opinion.
Just clarifying here, you believe Dr. McGills research or points are superior due to his specific insights. If this is correct and I haven't misinterpretted your sentence. What "insights" are you referring to that Dr. McGill has that other researchers/clinicians do not?
Because most physiotherapists I came across follow O'Sullivans approach. That gives me a feeling you're biased. McGill combines qualitative and quantitative study, in vivo and in vitro, to form an insight. To then test these insights on different population groups. The data and results have more weight to me than that of O'Sullivan. Why do you think O'Sullivan is more evidence based?
An example of a misinterpretation by professionals and clinical research groups: The overestimated importance of Transversus Abdominis. Research based on this assumption could influence results.

Simon blending several low quality methodologies is still low quality. O'Sullivan and his research group do utilise qualitative research, you may not be as well read as you believe.
Trans Abs fad is over, many Physiotherapists who are taught to critique evidence including myself are fighting against this approach. O'Sullivan has revoked a lot of what he wrote about stability and follows a new BPS evidence based thought process called Congitive Functional Therapy. Not afraid to compromise reputation, O'Sullivan came out of a rabbit hole which he made his name on, in sight of progression. McGill makes overstatements about how he can specifically Dx LBP by finding aggs and eases essentially? This is a pretty inaccurate report. Pain correlates poorly with damage, pain is not nociception. McGill makes sweeping statements with poor weight in evidence. A LOT of what he has to say is along the right lines (from what research tells us). Doesnt mean some things are not hence my original comment.

Originally compromising my professional attitude to stoop to the level of generalising a profession or inferring degredation of opinion/expertise when someone does not agree with your own narrow thought process.
Low quality methodologies? I find objective emperical data gained through a biomechanical approach of much higher quality than data aquired by subjective questionnaires like in Vibe Fersum, et al.(2013). Efficacy of classification‐based cognitive functional therapy in patients with non‐specific chronic low back pain: A randomized controlled trial.). The study even states that the pain was provoked and relieved by posture, movement and activities. All mechanical. Since the cause of pain is mechanical, does it not make sense to pursue a biomechanical approach? I do not disagree with the impact of cognition and behaviour but I miss the biomechanical data to support O'Sullivans claims.
Take a look at the exclusion criteria of the study I cited. One of them is this: Acute exacerbation of LBP at time of testing in order to avoid regression to the mean. So every person that experienced pain because of treatment was cut from the dataset. No shit it was succesfull. Other exclusion criteria just show that this approach is not fit for serious back disorders, only a specific group. What are you going to do when have patients who are not in this group?

Obviously Dr McGill thinks sitting at a desk causes back pain. But even if I retire, how can I avoid sitting at all? What am I supposed to do for the better part of the day? Stand all day? Sitting in a recliner causes more backpain. So, I am not getting this answer from Dr McGill's videos, what are we supposed to do all day if not sit. How about just lying on bed? does it help?
How can someone meet with Stuart McGill and have him look at their back and prescribe a plan?
Not sure but I'm just finishing his book Back Mechanic, he gives you a plan in the book and goes into the stuff in this video in lot more detail. The core plan is the Big 3 Exercises, an exercise called camel (or cat-cow) pose and walking. Then he has some more advanced stuff to try later on, like working on your hips and some specialized exercises for certain conditions. I definitely recommend the book, it is full of useful advice.
@@ObscureStuff420 I purchased it with the videos a few years ago as well. It makes sense but I think every back issue is different. Some exercises hurt some don't. People have to figure out what works for them. But it all comes down to finding the real cause of your pain and I think McGill's approach is one of the best and surprised most back doctors and surgeons don't do the same.
Dr Stuart. I'm 40 years old. I have degenerative disc since I was 16 my last MRI was 2 years ago. In waiting to talk to a surgeon. I'm not sure if other part of my body a weak couse my back more pain,like the Clute min.degenerative disc can heal if seft?

Your cervical spine is your neck. So this is not lower back pain.
Wow, you literally did no research on your diagnosis to ask this question. Severe spinal stenosis can cause permanent disability leading to paralysis. The techniques he shows here would not address it or prevent the progression of stenosis.
@@rooscous8657 Of course I know cervical spine is neck (C1-C7) Thoracic is T1-T12) and Lumbar is L1-L5). My complaint was lowewr back pain (Lumbar L5-S1) but my surgeon's diagnosis was cervical spinal stenosis and the surgery was on the neck which I suffered no pain at all. That's bullshit. It had been six years ago and I'm experiencing no pain on my spine at all. I am 84 years old and still as fit as a fiddle. I am allso practicing Intermittent Fasting 16:8 protocol and Wim Hof Method of breathing exercises.

Do you have any opinion on tms or mind body syndrome that dr john sarno and dr david hanscom talk about in regards to back pain?
I think tms is canceling all typsed of doctors and physio therapists, so I believe it goes other way around too.
But in my personal case tms has much more sense. With all respect to all these wonderful materials and if they help people, to me it doesn't make sense for a human body to be in so much everyday pain because of I don't know what, since we all have different bodies and habits, Drs often mention sitting long whilst I have not been sitting more than hour a day for years, always active, worked with different trainers etc and actually had Mc gills exercises as part of my workout routine. But as I say, whatever helps you to get out of thebpain is great.

I am really interested in the idea of tuning abdominal bracing. I would like to learn more. Any suggestions
I've hurt my rib sneezing. I recently hurt my back. prior to my injury I had been DLing and Squating for 1.5y.
I think the most interesting thing so far (not finished yet) has been him describing hip hinging, using a valsalva, and the bow to do things to avoid pain.
when I injured my rip I naturally knew to use those movement patterns. they were already ingrained into my body, and the fact that I could DL and Squat with a rib injury showed me that those movement patterns were safe and helpful.
with my current back injury, hip hinging to get stuff off the floor is crucial. so is putting my foot on a chair, or opening my legs very wide while sitting so I can keep a neutral spine while leaning forward. I brace against my knee with my elbow while lifting plates off the floor. unfortunately I've lost a lot of strength, but I'm still DLing and Squatting, and working back up.
I just think it's interesting that these movements patterns were so intuitive and natural for me. And as far as sitting goes, sometime after learning to squat I realized I changed the way I sat in chairs. it made me wonder if anyone else noticed that I was squatting to the chair instead of flopping down on them.
hope to be pain free again, though I'm kinda saddened. I understand that once you've injured something, you'll eventually heal, but it'll likely become a reoccurring issue.

My wife has osteopenia in her spine. No curve in neck vertebrae. c5-6, T9 vertebra fracture compression. Near normal intervertebral spaces and nrve channels. MRI revealed bone density eas not strong enough to place screws. How do we increase bone density? Eats lots of cheese, yogurt, Calcioum with D3 600 mg daily. Starting Magnesium to increase Calcium absorption. Is this aqpproach right?

Well my wife has ostheopenia, which makes her spine bones more fragile. She has stem cell treatment of upper back and neck disks. Little reduction in disk spacing between vertebrae. Don't just rely on nutrition alone, if possible add some type of movement like walking or resistance training. I wouldn't recommend cheese due to the fat content. Low fat dairy would be better
She has a breakfast of two to three ounces of cheese plus the 600 mg of Calcium, plus D3 4000 IUs, Magnesium 400 mg. The maximum absorvable calcium at single time is 600 mg. So she is wasting the excess. She also takes Omeprazole 30 mg to reduce acidity needed to digest calcium. She doesn' t walk too much either on regular basis. Thanks for your advise. Blessings

I mean I am serious I’m asking this question because I’m doing a lot of physical therapy right now because for six months I’ve been in pain and couldn’t move for three months out of that six months and I’ve been doing a lot of physical therapy at first and physical therapy hurt me and I did what you said I just laid down and took it easy for a few months when I felt like the inflammation was under control I started doing stretches that are calledThe V stretch the W stretch the tea stretch in the L stretch and I believe that has helped me tremendously to put my body back in better posture by trying to put my shoulder blades down /back It really has seem to help me with my neck issue believe it or not

Read the publication of his paper with a colleague on back pain during coitus (females & males studied); the quadruped position ("plank" on elbows; female coitus position) may help us inform as to whether plank position is stabilising or not. Hope that helps - I think it it was in the Eur Journal Spine.

This kind of outdated information is not only proven false, but is also a major part of promoting fear avoidance behavior in CNSLBP.
The back is our strongest bodypart and there has not been found any correlation between physical activity/conditioning/back posture and chronic low back pain.
McGill has a lot of good and interesting points, and give good guidelines on how to avoid pain in your everyday life. But he is also creating more fear of movement and fear of pain, which really only enhances the pain in the long term. He even claims that it is proven that forward flexion causes CNSLBP, when in reality this claim has been dismissed by overwhelming evidence.
However his way of working with patients appears caring and patient centered, and the most of his points are well founded. It's can just be very damaging to spread this kind of misinformation.
And don't get me started on the orange seed metaphor....

Thats the case with me. Worked with McGill clinician made me fear of every movement, and not helped me at all. Things are only getting worse..

Have you had spinal surgery? There you might not have needed.
If by no such thing as non-specific back pain one means there is always a physical/mechanical mechanism, not sure anyone would argue. However a situation in which one's physical response( e.g. chronic guarding) to emotional/psychological stress creating a soft tissue environment that triggers pain, that is entirely different from suggesting one's pain is all in their head. Huge distinction, imo. Great, highly informative, presentation otherwise.
