I remember this crash very well. I was taking my then wife to the terminal where she worked for Delta, and returning home, I watched the debris on one of the 18R/L runways, and then noticed the flipped tail of a 727, which I knew well, having worked on many 727s for another company. As I drove by airport road westbound, I saw the airplane burning in the distance, and no emergency vehicles on site yet. Very weird, and seems like it was yesterday, and having seen Delta 191 as well. All the people that died were trying to open the L2 door near #1 engine, but the fire was concentrated there. Only later it hit me that I was watching people die. So sad. RIP.

I knew Larry Davis. I was his Flight Engineer a few times before this crash. Good pilot, and a very nice gentleman. So tragic. Insofar as his back injuries from this crash, I know he already had back problems. I don't where he is today..... It's my understanding that the NTSB never actually proved the flaps were not extended, but they do have excellent circumstantial evidence. I just retired from my Delta career 2 years ago, flying my last 9 years as Capt on the 777, flying mostly to Sydney, Australia from LAX in the last few years. And I can assure you, Delta cockpits are very disciplined these days.
That’s so sad the crash is not entirely because of the pilots distraction although it plays an important part in it. I hope captain Larry Davis and first officer Wilson Kirkland and the other flight engineer are all well today. They all suffered pretty serious injuries from that crash and their flight career probably was over since delta fired them.( except the flight Engineer Steve Judd I think delta gave his job back)

@@777OutriggerI tried to looked them up and I think after the crash captain Larry Davis worked at Alaska airlines? I am not quite sure about that but Copilot Wilson Kirkland definitely never flew commercial flights again I think he was working at various aviation institutions and stuff like that? But I really hope they are all well today they made a small mistake and really did their best to save the plane.

Most people have no clue about aviation. Most people would be scared off from flying if they think a pilot cracking a joke 2 minutes before an accident has much of anything to do with the safe operation of an aircraft.
@@joesmith389 No, it isn't anything like that at all. There are two reasons that CVR recordings are handled delicately. One, is that an airline cant use the recording as a political tool - e.g. using it to fire a pilot that is, say, a union agitator. They would if they could. However, as far as releasing them in full to the public, why? The public has no idea what's going on on a CVR. All that does is allow jackasses with no idea what they are on about to comment on videos like this - as if they didn't do that already. There would be no contribution to safety. None. Zero. Nil. Zilch. The square root of fuck all.
prophetsnake I’m a pilot and still think they should be public after the investigation is completed. You’re paying for a service. If that service injures you, you should have full access to all evidence. Just like public records are available to anyone through their respective county for all crimes committed.
I’m pleasantly surprised by the number of survivors. Could’ve been way worse.

Cthight I never said ‘proud’ and the fact is I’m NOT commenting from the perspective of having lost my parents in the crash. I’m someone who knows that catastrophic plane crashes so often result in 100% fatalities, so it could’ve been a lot worse. It’s not that deep. Relax.

The CVR should be public record in almost 80% of cases, if not more.

I was a B-727 flight engineer from 1998 to 2001, sterile cockpit and checklist were a must! CRM matters even more today with only two crew members, the captain is usually a high timer paired with a medium to low time first officer. Checklist discipline rules!
The pilots' gossiping was recorded and shown to be the distraction that caused the crash. And the result of the investigation was to protect pilots from CVR's being made public. Great job, everyone!

Jonathan Fischbach Contact with the ground is always my favourite bit! Particularly crashing into the ocean for some reason. ‘Aircrash Investigation’ (or ‘Mayday’) on National Geographic channel do those CGI scenes magnificently. Awesome TV series.
He can't take credit for it because the animation is taken right from the show Air Crash Investigation. It's the only thing about this channel I don't like; the crash animations are all taken directly from Air Crash Investigation without any credit. It's only a matter of time before Patreon or the copyright holders find that he's profiting from other people's work.

Unions are there for a reason and without their bargaining workers would be screwed big time all the time, by big greedy corporations and small businesses altogether. When I discovered that my employer had defrauded me by underpaying my work, I was only able to sue them through the Unions, because otherwise I could not afford the litigation.
2 employees from the company I used to work for, and who were in Dallas on business, arrived at the airport about 20 minutes behind the accident to board a Delta flight back to Ohio. After seeing the aftermath of this, one of them took the bus back instead.

On the recording, you hear them talking about the Continental crash, and what they would've done. I believe the answer the one gave to the other, after a chuckle, was something along the lines of kissing his ass goodbye.

I remember this well. At the time, I worked for one of the fire departments that is near DFW, and we were part of the disaster plan for the airport. I had just gotten off work and was napping on the couch, when they broke in on the TV and reported the crash. I called an assistant chief (we didn't have cell phones or pagers then) and asked if I needed to return to work. He said to stand by for activation, but we were not called to assist. It could have been much, much worse. Great video, thank you for posting.

The crew also didn't respond to the stall well. The F/O thought that there was an engine failure, however all 3 were functioning fine. Flaps should have been the first thing to come to mind. The idle talk not only resulted in missing major checklist items, it also placed them in a poor mindset for the critical takeoff phase.

Yeah but you're not accounting for the malfunction with the alert system. If the flaps weren't set, the alert should've been there, but wasn't. Either way, they were distracted and that's why they crashed.
Jonathan Fischbach this is true, because even if the checklist hadn't been carried out, the moment a plane starts to go into stick shake, the first thing you would do is to look down at the flap setting and engage immediately, with slats too, in the hope that there is enough power and speed to get the plane off the ground and into the air.
I would have thought at the very least, jamming the engines to absolute max thrust, whilst engaging max flaps and slats, might just, just, just have let it limp into the air slowly.
If I'm wrong, then you can put me straight - I'm not an airline pilot. I don't know how long it takes for flaps to engage. I'm thinking though, watching this video back twice, the second that your stick starts shaking, and grabbing flaps within let's say, two or three seconds of realising that there is a serious problem.. and the flaps engaging in three or four further seconds.. that gives you still a handful of seconds before the plane hits the ground.
At maximum thrust, bearing in mind that the 727 is slightly overpowered, and with a generous wing size for the fuselage (according to Bruce Dickinson), with excellent climb rate..
..would this be possible? Even theoretically?

I have 10 years on the 727 and I doubt they could recover in time by extending the flaps. They extend slowly. They were already at a high angle of attack just to keep the plane flying which caused the compressor stalls and the loss of power caused the plane to descend.

That's absolutely ridiculous. The cockpit is a persons workplace, pilots are humans not machines, they deserve some shred of privacy. If there is no incident or accident then there should be no reason for anyone to listen to a CVR.
@@SomeDutchGuy NTSB is taxpayer funded. As the video says, they can release any video related to safety. And they should. Not sure why non-safety-related videos would need to be released though.
TheAmericanCanadian I completely agree. As an American commercial river boat captain, I am stuck in the wheelhouse 12 hours a day. In many ways, the wheelhouse is like the cockpit of an airplane. The USGC and the NTSB mandate voice and data recording. There is even a toilet in the wheelhouse and a remote for steering the 1200’ long, 50,000 ton vessel from the “throne”. I would not like the public to have access to THOSE recordings.

Is it weird that your Channel can cure the fear of flying? Ive been binge watching your Videos... And inspecting each accident i realized that the accidents happening there are really more unlikely to happen. 75% of your Videos are crashes from the 80's or 90's and from back then until now we learned from our mistakes and Pilots are way more trained than back then. Not saying that this completely cures it. But it gives you a clear message that the Pilots and Co Pilots know what they are doing with Auto Pilot on and the ATC Tower giving you informations about pretty much everything.

A lot of accidents at takeoff took place due to flaps not being configured properly. Enough of them happened, to where I am sure it is thoroughly drilled into today's pilots. The most likely bet if anything were to happen today, would be a mechanical failure such as the decompression that happened to MH370, causing the death of all occupants before the plane ran out of fuel and crashed into the Indian Ocean. Decompression would definitely be today's biggest fear.
I fly a lot and get somewhat nervous on every flight, but the thing I think about every time is that me driving to the airport is 5000 times more dangerous than flying. A drunk, or a terrible driver can kill you instantly whereas there needs to be a disastrous malfunction to the plane to die or get hurt.
Even walking on a sidewalk is more dangerous.

I have no doubt of this. Even a piston-powered aircraft flown under a light-sport certificate is safer than most any car on the highway.
The difference to me though, which makes me hate flying despite the statistics in its favor, is that in the car I'm in control and down on the Earth -- not in a tube six miles up with a hundred other people and nothing I can do about it. It's the lack of control. The chances of crashing are super unlikely, but if you do crash in a plane you probably aren't just breaking some bones.

Excellent videos, and give a real insight into what actually happened in these accidents. Better than news media. WX channel, " Why do planes crash", Discovery Channel and 60 Minutes, National Geographic, 20/20 and the NASA Channel all have done quite well over the years, but your videos are very good and informative. I have been in Aviation 40 plus years and have seen some terrible stuff. As always, most could have been avoided if folks involved paid attention to detail. I have had some flying experience, but I am a mechanic, line and depot trained. Well done sir, hopefully the kids learn more about this from you. Keep em flying!

Thank you for mentioning the flight attendants by name. They always seem to be forgotten in these crash videos. One thing that contributed to quick evacuation of the aircraft was the several off-duty flight attendants headed to SLC for annual recurrent training.

They had 2 minutes waiting time for recheck T/O configuration. No checklist, full relax. Routine kills.
They weren't following procedures.
Slats and flaps are a must to take off. Lift is very important. If they followed the book, this wouldn't have happened. Rules are made for a reason. Even if an engine had failed, the plane would have climbed, if slats and flaps had been set properly. 100% pilot and crew error.
Well, partly maintenance issue too - the takeoff configuration warning failed to sound. So not 100% pilot error.
No, it's 100% pilot error. The fact that a warning system designed to tell you that you failed to properly perform major checklist items did not work properly, does not excuse the fact that you still failed to perform those functions. Having a failure of a warning system designed to tell you that your suspenders aren't working properly doesn't excuse the fact that you are the one who forgot to put your pants on. At least they were up to speed on the dating habits of their flight attendants though, so that's good.
6omega2, there was significant pilot error, no question, but you should know that it is human to err. That's why warning systems are designed in the first place. They are a redundancy to reduce error rates on part of the pilot. They are as much a part of the system as the rest of the instruments. If that redundancy failed on its own accord, then the system is also to blame.
Contributing factor, yes. Root cause, no.
If "To err is human", don't become an airline pilot. Your first mistake is often your last too.

An in-flight fire is about the worst thing that can happen to an aircraft -- even if you lose one or all of your engines, often you can glide to a safe landing, not good, but survivable if you are lucky. (We love the X Pilot's videos -- thanks for posting.)

Great job, very realistic.
This crew demonstrates failure to keep a "sterile cockpit". The best thing you can do before take off and landings is to only focus on the tasks at hand and keep non-critical chatter between crew to a minimum. Non-essential chatter can lead to serious distraction and danger as we see here.
During WWII in was common for B-24 pilots keep the flaps up during the takeoff role until they got close to V1 then put the flaps down. This reduced the drag and decrease the takeoff distance. It is also common on some seaplanes to keep the flaps up until they get on the step.
But flaps on the 15° setting produces negligible drag, and significantly increases lift, thereby leading to a faster takeoff...

This reminded me to the LAPA 3142 flight. Take off with no flaps set, non-essential conversations on the cockpit, warnings dismissed because of the constant false alarms... it was one of the deadliest aviation accidents in Argentina's history.
I knew a guy who was on that plane. Thankfully, he survived the crash.
I knew one of the passengers on this flight. She and her husband survived. Hearing her account of the crash, then ensuing chaos while escaping a burning aircraft was riveting and surreal. To this day, she suffers from PTSD. Also disappointing was how Delta treated the crash survivors in terms of compensation. The flight crew made a rookie, fatal, and totally preventable error that caused death and destruction.

I was a 727 pilot for a different airline then; and we departed DFW the day after this accident on the same runway. The Delta wreckage was clearly visible from the right side of our aircraft. After reaching cruising altitude, the senior flight attendant came into the cockpit and told us the passengers are usually pretty quiet during take-off and landing; but during this take-off the cabin was utterly and completely silent. R.I.P. Delta 1141.

I was on a MD-80 that hit the brakes mid takeoff roll because the tower had seen the flaps were in the wrong position. They told us a faulty indicator light made it happen. Went back to the gate and switched planes.

Interesting, I can see the KJAN ATC tower from my office window.
I worked for Builder's Transport and had to drive a reefer unit out to the crash scene so they could load the bodies up. Told me to stay in the cab. What a sad wait that was.
After watching a number of these videos I automatically thought "flaps". Bang on the money!
