I Would say Miljenko uses much more of a head voice than Adam and Lee, for me most powerful singing in fifth octave for male is Sebastian Bach, but if he was more consistent and could have sing live what he was singing in studio.
the most powerful, consisent and have proper technique singing in 5th octaves male singers for me is David Phelps. But look like Karo is talk about only rock genre, so i agree with you ^^. Mike from steelheart is the headiest "high voice" male rock vocalist i know. Adam and Hyuk use very heady mix too ^^

Also I am with everyone else and agree that for a tenor, especially one with issues with support, their mixed voice range should be started much earlier. A4 is too high and starts him off with vocal strain already. A4 ~ Bb5 is just straining, at least for the most part. B4 ~ Bb5 is definitely all pure strain.

Yes. It does. You don't start a tenor on A4. That's like starting a mezzo on E5 or having a soprano lower register end on E3.
