I decided not to venture or risk her health and my practice when there is a anaphylactic allergy to bees considering my clinic location and emergency facilities. Would you recommend doing a hyla patch test every patient before injecting filler? The purpose being to determine if they are allergic before receiving the filler treatment, so that we can fully educate the patient that dissolving isn’t a future possibility if they are allergic.
I had a test patch today for hyalurondaise to try and resolve some lumpy filler but I had a positive reaction. Are there any other alternative for me to remove the filler? Thank you
Liporase! The Korean dissolver is particularly tricky and I find that while it does dissolve, it causes severe swelling in people that are not even allergic to Hylaurandais. As an American, in doctors offices I have only used Hylanex Recombinant which is a fantastic dissolver, causes absolutely zero swelling, if any so minimal it's barely noticeable. Results are apparent within seconds and full results with in probably 6 hours, possibly 10% drop off after 24 hours. Some say longer but I don't believe it. As for the liporase which I have done myself at home, I find I swell significantly, and though I'm not going to die, I cannot stand it. I don't understand why all hylaurandais isn't from bovine or dna recombinant.
my eyes almost swelled shut.... what I went through was hell , how I looked, how others looked at me, the injection telling me to go to emergency cus it could make me blind...the swelling was for Months .. we then tried Hyaluronidase alone on my arm to see if I would react, and I reacted ( thinking the Hyaluronidase together with the filler in my face caused the reaction )
Hi Katreen, The path for this is that you do your best with other measures, so massage, warm compress and try to get hyperbaric oxygen if available to preserve the tissue that remains.
What happens if the dosage for emergancy it's used for corection? My face is melting. Are we ever gonna be back to normal or out tissue is distroyed forever?
Hopefully they will have got hold of the dissolving agent. But possibly if it’s got to the point of hospitalisation debridement of dying tissue ? This is extreme of course. Hyperbaric oxygen and intravenous antibiotics too. Regarding allergic reaction though they would be ensuring that the allergic reaction was being treated if it included systemic response such as airway risk. Kind regards Mary. If you do have a patient who has had a VO and you’re going onto the reversal procedure, which I am aware can take sometimes an hour or longer before the blood flow is back , would you reapply numbing cream before injecting the hyalase, as it can wear off through out that time? Please let me know . Thanks
I am a cosmetic I injector and I never used to have any reaction to Hyalase however now I get severe swelling- however when I had some Hyalase my practitioner tried it with adrenaline and I didn’t swell at all. Strange that it is something that has developed over the years though- and annoying. Let me know if you want pictures!

I had a severe reaction to Hyalase, not full blown anaphylaxis but my entire face and neck swelled. I was having lumpy filler dissolved in my lips and the doctor only injected a tiny amount but he didn’t perform an allergy test whatsoever. Now i’m terrified to have any more filler incase i get a VO 😭 . Would it be an option to dilute hyalase with adrenaline instead of saline?
No this would not be safe. I personally would not inject hyalase again, but you may if you are in certain parts of the world get hyelenex which is DNA recombinant copy of human hyaluronidase you may not react too.
Apologies but info is absolutely useless. I have an under eye angioderma caused by hyaluronic and had a strong allergic reaction when hyaluronidase was injected in the lip. HOW do I treat my under eye angioderma. Your answers focus exclusively on the one side of hyaluronidase use , namely PRIOR to the hyaluronic injection. I have no choice. WHAT IF ANY OPTIONS are open to me to remove my angioderma?
I have never been allergic to anything in my life and had an allergic reaction to the dissolver when a patch test was done. Was it likely a false positive? I have pretty sensitive skin and tend to get hives etc when anxious (which I was when we did the test).
This is exactly my situation too, i’ve got nasty filler left in my face after over 3 years as it hasn’t dissolved by itself due to migration. I had previously had dissolving sessions however because the of the exposure i’m left with an allergic response to the solution. It baffles me how there’s still no answer or alternative for people who are allergic to hyalase, infact it’s extremely disappointing.

Hi Sara, No because the chances of it being relevant are low and you are exposing them to an allergen without a good reason really.

What would you do if you don’t inject hyalase on a VO? What is the care path for this?

Yes- It's wise to take what ever steps you can to make it possible to keep injecting hyalase for as long as is needed to restore blood flow.
