I used and loved tretinoin till my skin became hypersensitive a few years ago from autoimmune issues. Sadly I had to stop. I started using retinol (Cerave) a year ago and switched to adapalene (Avene) a few months ago but the improvements have been negligible. I just got a new prescription of 0.05% tretinoin as I feel that it would work much better for me. Fingers crossed!
I did the exact same thing! 😅 I can remember taking off my mask to eat lunch at work one day and watching the skin fall down like rain 😳 I called it face dandruff because that's what it looked like 🤮 About a year later someone saw my face while I was eating and was shocked at the difference (I hadn't told anyone i was using it). Before long, word got around and people began approaching and asking to see my face 🤷🏻♀️ I'm now 56 and so glad I took the plunge!
I love the SkinMedica retinol products. It’s one of the few brands that specify the retinol concentration and it offers a few strengths (.025, .05, .1%) to allow you to build up your tolerance without guesswork. I’m on Tretenoin now, but I was able to transition onto Tretenoin with minimal irritation thanks to SkinMedica.

I’ve tried the Skinmedica retinol at 0.5. Once I started using it regularly, it really irritated my skin causing redness and stinging every few applications. I finally stopped using it because i didn’t want to aggravate my melasma further. I did notice some improvement in skin texture. Now I just use the Olay regenerist retinol cream though because it’s much gentler. Will see if I get benefits.
I have been using tretinoin for at least 15 years on and off. The past 3 years I have used it every evening. I have no redness, but I did go through of transition and was very persistent. I have no reaction at the highest potency. I’m glad I did. I’ll be 74 next month and still get compliments on my skin!
I have used Tretinoin for over 20 years. I’m a big fan of Skin Medica products and tried the .25 retinol complex then bumped up to 0.5. Hands down my skin irritation is waaaay less and I’m getting excellent exfoliation with the skin medical 0.5. I’ll never go back to tretinoin.
I don’t think that in this case, it would be an issue that Skinmedica funded the study-of course it makes sense that they wanted to prove the efficacy of their product. However, my personal experiece using retinol for 2 years, absolutely no visible positive results. Switching to Differin and using it for 2 years, again, absolutely no visible positive results regarding anti-aging, but positive results regarding acne. Switching to tretinoin- after 7 months, basically new skin, amazing product. Now on year 3, and I will never stop using it.
Preach sister!!! Same experience same product journey!!! I am african american and my skin responded to the products EXACTLY like yours! Only difference is I was also using a 15% vitamin C for hyperpigmentation as well. Still do. Tretinoin for life!!!
Thanks for the detailed description!! I started using granactive retinol 4 months ago and considering switching to Retin-A Micro 0.025%(tretinoin in slow release microspheres). Can you please share the tretinoin percentage and vehicle (gel/cream/liquid) that you used/use? And also your skin care routine (whether you used a moisturizer before applying tretinoin etc.)
@@PercussiveMaintenance Most physicians will start you on 0.025% tretinoin. It's very thick and just takes a dab for the entire face. It takes a long time to work up to the higher percentages. You use it on bare, clean skin. If I get red and scaly, I will skip the tretinoin one night and just apply a good moisurizer.
I have tried Tretinoin several times on the past 2 decades but the dry, flakey, sensitive, results were just so bad that I could never I use it on a consistent basis. I also wax so I would have to stop using it at least a week prior. Some people can use it once a week and increase as they go. Well, good for those people. My skin always looked worse than before I started. I’m happy to try something that is less harsh. I’m going to be 63 and you just get to a point where you’re just happy to be healthy and alive. I’ve always took care of my skin according to my budget. The other thing is I’ve never been a sun person. Also I believe genetics has a part in it as well. Love your content.
I used that skin medica retinol for about two years. It was quite expensive but it made my transition to tretinoin totally seamless. I had literally zero irritation.
I've been using Avene a-oxitive peeling which has 0.1% of Retinaldehyde. I truly love it and it has become my core product of my night skincare routine. So I'm glad to hear about new studies related to vitamin a and derivatives

Great video. The SkinMedica bottle depicted at minute 12:22 is the Tri-Retinol Complex ES but the product bottle shown just below the video chapters is the Retinol Complex 1.0. I suspect the study used the latter product. Anyone know for sure? Thanks! [Can only find the paper's abstract available for free online. However, the clinical trial summary refers to 1% retinol cream which sounds more like Retinol Complex 1.0 than the other one.]
I contacted Skinmedica about this product. They say the product mentioned in the study was discontinued in 2016. It was replaced with their retinol complex which comes in 3 different strengths. They stated “Tri-Retinol Complex and Tri-Retinol Complex ES are no longer available since they were discontinued in 2016. The replacement product is Retinol Complex which comes in three different strengths (0.25, 0.5, and 1.0). Retinol Complex is a separate formulation from Tri-Retinol Complex and utilizes a single form of Vitamin A rather than 3 different forms of Vitamin A (retinol, retinyl acetate, and retinyl palmitate). In comparison to Tri-Retinol Complex, Retinol Complex is stronger even at its lowest strength (0.25). The newer formula was designed to maximize efficacy while also minimizing irritation typically associated with topical retinol products.”
I use skincetuicals retinol 1.0 strength its $92 but my skin is looking great! It has really reduced my fine lines and skin cetuicals also has lots of test done on their products.

In general, the less irritating an effective product is, the more likely that someone will actually use it more often. A similar product (that is irritating) won't work at all if you buy it and just leave it on the bathroom shelf. 👱♀️❤
This is why I don't use tretinoin. I have tried several times & no matter what I do, I end up with a very damaged skin barrier. There was literally no way to continue. I just don't tolerate it. My rosacea stays very flared through 3 months of attempting even the lowest strengths. I tolerate adapalene with no issues, but only saw acne results as others have stated. The only product I have been able to use with any success (but still only every second night) is retinal (retinaldehyde) by Geek & Gorgeous.

I have been looking for a new Retinol product. I used the prescription from about the ages 11- 55. The one ingredient in the prescription that I believe caused me to have acne on my face which was Isopropyl myristate. I began using it for acne in the mid 70s. It did make me look younger but acne still plagued me for all those year. Right now I have using a product from a pharmacy but needed something stronger to combat wrinkles at age 61. I ordered the product you recommended. Thank you!
I’ve used this skin Medica product before. I liked it. It was $100 though. Now I use retin a micro gel and tretinoin cream. $20 to $30 with insurance in Canada. Use the sandwich method to minimize irritation.

I had to purposefully irritate my skin so I could get razor bumps in order to get full strength tretinoin. I will be using the tretinoin for wrinkles under my eyes. It's a shame that medication is not sold in the free-market. We shouldn't need prescriptions especially when it comes to topical products.
This is one, if not the only, Retinol that I have tried that actually works better than RetinA with minimal irritation! So glad that I tried it. I'm sold.
I started tretinoin 0.01 gel a month ago. I absolutely love it! I haven't had any adverse reactions other than some drying on my chin. I use drunk elephant protini moisturizer on top, then add a sleep mask to lock it in. I also use face oils on top of my sleep mask lol. I'm 38 and I'm so glad I added tretinoin to my skincare. I also get monthly facials which may have helped prepare my skin for harsher products.
I think that OTC retinols absolutely work unless they're poorly formulated by cheap companies. Wherever I use one from credibile brands I get the Same symtpoms and results that I get with prescription tretinoin. I think advantages of Tretinoin is that it's cheaper here than most OTC, credible retinol, it's always fragrance free, it lasts a lot and you know it works. Pros of OTC is that it comes in countless formulas that can suit individual skins better than a tretinoin cream that rarely has something extra going on to prevent irritations like soothing ingredients. A negative aspects of retinol products is that they tend to be expensive and rich in fragrances and useless stuff, so do you research.

Hi, interesting study, but I will stick to retin A, I have been using the 1,1% strength for 5 years, and I do get occasional irritation, especially in our cold canadien winters, but I can’t imagine not using it..unless something better comes along..
I’m sticking to my prescription strength 0.025% Retin A gel. I’ve been using this for about 30 years and it has worked miracles on my skin. Yes, I had to go thru painful times for my skin to adapt, but WELL WORTH IT! So between Retin A, laser treatments, rose hip seed oil and Shea butter, my skin looks radiant. I’m 65 and I’m finally proud of my skin. You would never know that I had bad acne as a teen that made me cry all the time. Also, Retin A has come way down in price for me. I now pay $25.00 for a tube that last many months. 30 years ago it was about $100! Maybe even more than that.
I use about a quarter sized amount of organic Shea butter along with a few drops of organic rose hip seed oil (grown in the Andes Mountains) and rub it between my hands and it instantly melts and becomes sort of milky looking. I rub it into my face, neck, arms, hands, chest, legs and feet. The rose 🌹 hip seed oil is potent and it actually began to fade my dark spots but it was slightly irritating so I began mixing it with Shea butter and together they work . I kid you not, I lived in South America when I was young, and the indigenous Indians used rose hip seed oil thousands of years ago to heal their skin from all sorts of maladies. I can give you the companies that I purchase from if you would like.

I use curology .14% tretinoin for 40 bucks every 2 months. Yes, stronger than the strongest of .1%. It also has 5% azelaic acid and 4% niacinamide. I think it's a great deal. Been using it for years.
I use all over my face but avoid the area inside the eye socket bone, not because of dryness, etc. but because I don’t want to take the chance of the adapalene migrating into my eyes.
Great video! For three years I tried all different Brands and strengths of otc retinols. I kept having issues with them and then taking breaks. My skin just did not like retinol. It would burn and be irritated and get rashy and red and no matter what I tried, my skin never adjusted. Adapalene worked well but didn't completely get rid of my adult acne. I used it for a long time until I could finally afford Retin-A. I didn't think I was going to be able to do Retin-A because I couldn't do retinol but ironically retin-a is the best thing that happened to my face. It completely cleared up the acne and after 6 months I started seeing what they mean about anti-aging with reducing fine lines and wrinkles and just looking more plump and less saggy. I'm glad I stuck with it because I started out only being able to use the tret once a week and then work my way up until now I can use it three times a week. It's really nice because it made my skin less oily like I can't believe at age 45.

Love this style video! The way in which you present these studies so we regular folk can understand them is so very appreciated too, Dr. Dray. This is exciting news! I am able to get my 0.05% tretinoin Rx from my GP, and fortunately my insurance covers it: my doctor provided authorization for its use by including my hx of basal cell and tret's efficacy in cellular turnover, and my insurance company okayed it, $3.95 for a 20 gm tube is my actual cost. I don't have acne, at age 67 I'm an anti-aging kinda gal, and am very interested in the exciting discoveries being made behind such reputable studies. That you bring us such such interesting and new data in such a "user friendly" way is just one of the many reasons we, your fans, think you're just great!
How is your insurance covering it? They don’t cover anything cosmetic. I’m curious to know what diagnosis code your GP used when submitting the RX claim.
Better be careful the cream did nothing for my basil cell cancer, I had to have 3 in hospital surgeries it ate my nose up, the basil cell cancer. I thought the tretinoin would have helped it didn't basil cell cancer is nothing to play with. I learned that the hard way. But good luck and I wish you well flanamom.
SkinBetter has a retinoid molecule combined with lactic acid molecule. It actually made my skin feel plump in the morning since the lactic is hydrating. I think this is a superior retinol to use. Great ingredients in it. I hope you analyze one day
Dr Dray actually did a video on the skinbetter science alpharet cream. Her analysis of it was that the product itself showed evidence of helping smooth fine lines and wrinkles, but not the active ingredient itself, which was the retinoid+lactic acid. She also said the major issue with their studies is that they did not double blind the study, meaning testing one group with the product that has the active ingredient in it, and the other group with the vehicle with no active ingredient in it, so it's really hard to tell what in the product is causing the anti aging effect. Another thing she brought up is that there are other ingredients in the alpharet cream that have been known to help with skin aging, such as glycolic acid and niacinamide. I don't use alpharet personally, but a lot of people anecdotally report seeing better results with it than regular OTC retinol, so I hope one day alpharet does an unbiased extensive trial so that we know if the alphret technology is actually worth considering.
I have been using the Skin Medica 1.0 for over ten years (I am 79). Prior to that I used tretinion for many years, but it became a hastle because of insurance. recently I considered goint back to tretinion. I guess I'll stay eith Skin Medica-no flaking. Thanks for the video-great timing.
Interesting! I was using Skinmedica retinol .50 and felt I had pretty good results. Then asked my derm for tretinoin because it is the “gold standard”. I’ve only been on tretinoin a month but so far feel the skinmedica retinol was better for me. Will give the tretinoin more time but may switch back to the retinol.
It's important to give it time! At first I felt the same, but it has really made a difference in my 68 y/o skin! The texture is so smooth. I get compliments every day!
I'm currently working my skin up to prescription tret. I started with an OTC retinol, then graduated to adapalene alternated with retinol every other night. Next, after my skin acclimates, I will begin adapalene every night. Eventually I will go in with tretinoin every other night ,,,etc. It's just a gentle way to get there. ❤
I just got back from Guadalajara on Monday & while I was there I bought some Retin A Tretinoina .05% & also some 4% hydroquinona crema, both OTC. I am using The Route on my face but spot treating 1 tiny brown spot with the 2 others that I purchased in Mexico.
I began with Agency at the lowest level and thankfully had no redness and flaking on my rosacea skin. Gradually increased in three month increments. I also recommend using a emollient moisturizer to lessen 😢any reactions

Honestly, if I were to buy a OTC product it would be Avene, retenaldahyde. Costs less, takes fewer conversion steps to become effective and has peer reviewed clinical data as well.

I use the neutrogena retinol in the grey jar. Fragrance free It works amazing on my skin and I have a lot of lines on my forehead and I’m very sensitive. My skin looks great. Just my opinion. I like to tell people if I use something good
It’s weird I’m black and I really don’t notice any irritation with tretinoin, I use the 0.05 cream but of course I pair that with a moisturizer. The results have been amazing
I’ve used Skinmedica retinol 1% for 10 yrs . I tried Musley trentinoin for 3 months and I find the Skinmedica to be BETTER! It makes my skin peel more and it’s smoother.

You had me at skin biopsies! Sadly, my super sensitive facial skin does not play nice with any of the retinols I have tried. I switched to the Avene retinaldehyde 0.1% and trained my skin over a 5 month period starting at once a week and very gradually increasing frequency to nightly. I did not experience redness, peeling, stinging or hotness during or after the 5 month introduction period. So I have my Vitamin A skin product and am not likely to experiment further.
Boots (UK) have just brought out a 1 percent retinol version of their 0.3 percent product that you reviewed previously so I was really interested to hear about this study. I have only just started using it so can’t comment yet on its effectiveness. They have also brought out an accompanying retinol soother to help to combat the redness and dryness.
I love Skin Medica retinol! I’ve been using it for 2-3 years and have seen a big difference in my skin. I started with the 0.5 and now am up to 1.0. I’ve never experienced irritation, just a tiny bit of dryness in the beginning.

Yes I use that retinol for 3 years now ! It has taken my line expression away from my forehead I love it and I am obsessed with it z I put it every night . You have start at .25 bc you will peel off u til your skin gets used to it ! I am 40 years old and everyone thinks I’m on my 20’s is not only this retinol . I have been using Avene Retrinal 0.1 Intensive Cream which I tolerate very well. I chose to use this product because my understanding is that as a retinaldehyde, it only takes 1 step to convert into bioavailable retinoic acid, rather than the 2 step conversion for retinol. Given this study, you refer to in Dermatology JAMA, would you recommend reinaldehyde over retinol for those who are concerned about the side effects of Tretinoin?
I have very sensitive skin with rosacea and my dermatologist recommended the skinmedica 0.25% retinol because I really wanted to try some kind of retinol for anti aging. And all the OTC products I tried previously irritated me a bunch. I got up to using it twice a week and it never caused any irritation. I always notice the next day my skin looks bright and bouncy, but I haven't noticed any changes in dark marks, and don't have noticeable wrinkles yet. I also use medik8 bakuchiol serum every day and I love it!
I actually was and have gotten my prescription Tretinoin from my regular primary and have never had a problem, dry for a day or so but that was it. My prescription insurance is terrible so I use just a discount card. For about $90 which lasts very long and that's just cash. Cheeper compared to the cosmetics I had been buying for no results.
I’m going to try the skin medica retinol 0.25 on the neck. I can’t tolerate even retin a micro 0.06% on neck. But I want to incorporate something for my neck.
Wow, this is a really thorough study, albeit its limitations! I guess it would be nice to try Skinmedica's retinol complex, but not if I have to spend more than 100 bucks for the product (product + shipping) when I can buy OTC adapelene (15~30g) for 12~24 bucks. There are also cheaper retinol products which I could buy for around 25~50 bucks. I would just buy those and use the rest to buy sunscreen and a couple of sturdy jeans (because those are good at blocking sunlight for a non upf clothing).
Oof yeah that price is rough. Wonder how long the bottle lasts. I get tretinoin + azelaic acid formula from Apostrophe and a $75 bottle lasts me like 4-5 months.
Great study although small with robust evidence behind it. Question: Skin Medica Retinol issue for most users is to graduate to application on areas such as under eyes, neck and around mouth. Most can graduate to higher strength but can’t get over the sensitivity in these areas
The product shown in the video, SkinMedica Tri-Retinol Complex ES is not sold SkinMedica’s website. In the written part of this video it has a link to buy another retinol from this company, but that product wasn’t used in the study.
The Golden Rule by The Route is the best. It is a next generation retinoid & can be used everyday by anyone. I know one of the 2 founders. She is my N.P since 1999.
Ever since you mentioned the advanced clinical retinol cream for the body, I’ve been using it on arms and legs regularly for a year, and it has improved texture of my body skin, though not as dramatically as tretanoin on my face. I think it’s probably a very low concentration. Of course, sun protection before and since starting the retinol. I hope it suppresses pre cancer cells.
I tried skinceuticals retinol 0.5 and my face peeled a lot and felt pain when applying other products till I got use to it. Now trying their retinol 1.0, the peeling was not as bad but I also used less frequently.
I appreciate the information on the study. My question concerns the strength of tretinoin that was tested. I understand the tretinoin was .02%. Hello Dr. Dray, thanks for your very informative videos. After watching this I did a little research and I'd like to know your opinion about Paulas choice 1% retinol or the ordinary 1% retinol. Paulas choice in addition to peptides also contains vit C which skinmedica doesn't but the latter has niacinamide. That's the major difference I noticed. what do you think?
I use the PC because it's just more cosmetically elegant than TO and I use a lot of her products. I use it on my neck as my neck doesn't tolerate my tretinoin. I'd say it goes down to budget personally, because the price differential is huge. You can always use a serum or a moisturiser with peptides and niacinamide in it.
There's other products of PC that are low to mid-strength and cheaper than mine. I use the Clinical line which is the most expensive (purple packaging). 1 day of the 1% Clinical Retinol on the neck, 1 day of the Clinical Ceramide-Enriched Moisturiser on my neck (0.3%). Keep alternating. That moisturiser is hella expensive. I wait for a 20% off sale.
As someone with the most sensitive skin (I am 55 with the usual number 11 between my brows) I tried a prescribed tretinoin and no matter how I used it, it was just too strong.. I tried the Olay 24 which was fine but it always pilled off my skin and I didn't like that. I bought the Garnier Retinol Berry (green bottle) and I absolutely love it. I used it every 4 nights at first and sometimes less but now I can use it every night. I definitely see results and feel a lot better about my skin. I highly recommend it if you have sensitive skin and it's at a very affordable price point. I was happy that Dr. Dray had recommended it in one of her videos as well.
Thank you to Dr. Dray for pointing out this study. There is something really weird about it. Something we have never seen before with a clinical study. The actual study was done more than 10 years ago and they only decided to publish it now. That weirdness is underlined by the fact that several well known academics on the publication are funded by SkinMedica.
it struck me as fishy that the chosen tretinoin concentration was low on the scale of available concentrations. more useful would be a follow-up comparison that uses the .1% tretinoin.

It’s one of the few brands that specify the retinol concentration and it offers a few strengths (.025, .05, .1%) to allow you to build up your tolerance without guesswork. I’ve tried the Skinmedica retinol at 0.5. Once I started using it regularly, it really irritated my skin causing redness and stinging every few applications. I finally stopped using it because i didn’t want to aggravate my melasma further. I did notice some improvement in skin texture. Now I just use the Olay regenerist retinol cream though because it’s much gentler. Will see if I get benefits.
I have been using tretinoin for at least 15 years on and off. The past 3 years I have used it every evening. I have no redness, but I did go through of transition and was very persistent. I have no reaction at the highest potency. I’m glad I did. I’ll be 74 next month and still get compliments on my skin!

I have used Tretinoin for over 20 years. I’m a big fan of Skin Medica products and tried the .25 retinol complex then bumped up to 0.5. Hands down my skin irritation is waaaay less and I’m getting excellent exfoliation with the skin medical 0.5. I’ll never go back to tretinoin.

That's great to hear. Other studies have shown that retinols don't cause the same pro-inflammatory response that retin-A does.
I don’t think that in this case, it would be an issue that Skinmedica funded the study-of course it makes sense that they wanted to prove the efficacy of their product. However, my personal experiece using retinol for 2 years, absolutely no visible positive results. Switching to Differin and using it for 2 years, again, absolutely no visible positive results regarding anti-aging, but positive results regarding acne. Switching to tretinoin- after 7 months, basically new skin, amazing product. Now on year 3, and I will never stop using it.

Thanks for the detailed description!! I started using granactive retinol 4 months ago and considering switching to Retin-A Micro 0.025%(tretinoin in slow release microspheres). Can you please share the tretinoin percentage and vehicle (gel/cream/liquid) that you used/use? And also your skin care routine (whether you used a moisturizer before applying tretinoin etc.)
@@PercussiveMaintenance Most physicians will start you on 0.025% tretinoin. It's very thick and just takes a dab for the entire face. It takes a long time to work up to the higher percentages. You use it on bare, clean skin. If I get red and scaly, I will skip the tretinoin one night and just apply a good moisurizer.

I have tried Tretinoin several times on the past 2 decades but the dry, flakey, sensitive, results were just so bad that I could never I use it on a consistent basis. I also wax so I would have to stop using it at least a week prior. Some people can use it once a week and increase as they go. Well, good for those people. My skin always looked worse than before I started. I’m happy to try something that is less harsh. I’m going to be 63 and you just get to a point where you’re just happy to be healthy and alive. I’ve always took care of my skin according to my budget. The other thing is I’ve never been a sun person. Also I believe genetics has a part in it as well. Love your content.

I've been using Avene a-oxitive peeling which has 0.1% of Retinaldehyde. I truly love it and it has become my core product of my night skincare routine. So I'm glad to hear about new studies related to vitamin a and derivatives

Great video. The SkinMedica bottle depicted at minute 12:22 is the Tri-Retinol Complex ES but the product bottle shown just below the video chapters is the Retinol Complex 1.0. I suspect the study used the latter product. Anyone know for sure? Thanks! [Can only find the paper's abstract available for free online. However, the clinical trial summary refers to 1% retinol cream which sounds more like Retinol Complex 1.0 than the other one.]
I contacted Skinmedica about this product. They say the product mentioned in the study was discontinued in 2016. It was replaced with their retinol complex which comes in 3 different strengths. They stated
“Tri-Retinol Complex and Tri-Retinol Complex ES are no longer available since they were discontinued in 2016. The replacement product is Retinol Complex which comes in three different strengths (0.25, 0.5, and 1.0).

In general, the less irritating an effective product is, the more likely that someone will actually use it more often. A similar product (that is irritating) won't work at all if you buy it and just leave it on the bathroom shelf.

This is why I don't use tretinoin. I have tried several times & no matter what I do, I end up with a very damaged skin barrier. There was literally no way to continue. I just don't tolerate it. My rosacea stays very flared through 3 months of attempting even the lowest strengths. I tolerate adapalene with no issues, but only saw acne results as others have stated. The only product I have been able to use with any success (but still only every second night) is retinal (retinaldehyde) by Geek & Gorgeous.

I have been looking for a new Retinol product. I used the prescription from about the ages 11- 55. The one ingredient in the prescription that I believe caused me to have acne on my face which was Isopropyl myristate. I began using it for acne in the mid 70s. It did make me look younger but acne still plagued me for all those year. Right now I have using a product from a pharmacy but needed something stronger to combat wrinkles at age 61. I ordered the product you recommended. Thank you!

I had to purposefully irritate my skin so I could get razor bumps in order to get full strength tretinoin. I will be using the tretinoin for wrinkles under my eyes. It's a shame that medication is not sold in the free-market. We shouldn't need prescriptions especially when it comes to topical products.

This is one, if not the only, Retinol that I have tried that actually works better than RetinA with minimal irritation! So glad that I tried it. I'm sold.
I started tretinoin 0.01 gel a month ago. I absolutely love it! I haven't had any adverse reactions other than some drying on my chin. I use drunk elephant protini moisturizer on top, then add a sleep mask to lock it in. I also use face oils on top of my sleep mask lol. I'm 38 and I'm so glad I added tretinoin to my skincare. I also get monthly facials which may have helped prepare my skin for harsher products.
I think that OTC retinols absolutely work unless they're poorly formulated by cheap companies. Wherever I use one from credibile brands I get the Same symtpoms and results that I get with prescription tretinoin. I think advantages of Tretinoin is that it's cheaper here than most OTC, credible retinol, it's always fragrance free, it lasts a lot and you know it works. Pros of OTC is that it comes in countless formulas that can suit individual skins better than a tretinoin cream that rarely has something extra going on to prevent irritations like soothing ingredients. A negative aspects of retinol products is that they tend to be expensive and rich in fragrances and useless stuff, so do you research.

Hi, interesting study, but I will stick to retin A, I have been using the 1,1% strength for 5 years, and I do get occasional irritation, especially in our cold canadien winters, but I can’t imagine not using it..unless something better comes along..
I’m sticking to my prescription strength 0.025% Retin A gel. I’ve been using this for about 30 years and it has worked miracles on my skin. Yes, I had to go thru painful times for my skin to adapt, but WELL WORTH IT! So between Retin A, laser treatments, rose hip seed oil and Shea butter, my skin looks radiant. I’m 65 and I’m finally proud of my skin. You would never know that I had bad acne as a teen that made me cry all the time. Also, Retin A has come way down in price for me. I now pay $25.00 for a tube that last many months. 30 years ago it was about $100! Maybe even more than that.

I use about a quarter sized amount of organic Shea butter along with a few drops of organic rose hip seed oil (grown in the Andes Mountains) and rub it between my hands and it instantly melts and becomes sort of milky looking. I rub it into my face, neck, arms, hands, chest, legs and feet. The rose 🌹 hip seed oil is potent and it actually began to fade my dark spots but it was slightly irritating so I began mixing it with Shea butter and together they work
. I kid you not, I lived in South America when I was young, and the indigenous Indians used rose hip seed oil thousands of years ago to heal their skin
from all sorts of maladies. I can give you the companies that I purchase from if you would like.

I use curology .14% tretinoin for 40 bucks every 2 months. Yes, stronger than the strongest of .1%. It also has 5% azelaic acid and 4% niacinamide. I think it's a great deal. Been using it for years.

I use all over my face but avoid the area inside the eye socket bone, not because of dryness, etc. but because I don’t want to take the chance of the adapalene migrating into my eyes.

Great video! For three years I tried all different Brands and strengths of otc retinols. I kept having issues with them and then taking breaks. My skin just did not like retinol. It would burn and be irritated and get rashy and red and no matter what I tried, my skin never adjusted. Adapalene worked well but didn't completely get rid of my adult acne. I used it for a long time until I could finally afford Retin-A. I didn't think I was going to be able to do Retin-A because I couldn't do retinol but ironically retin-a is the best thing that happened to my face. It completely cleared up the acne and after 6 months I started seeing what they mean about anti-aging with reducing fine lines and wrinkles and just looking more plump and less saggy. I'm glad I stuck with it because I started out only being able to use the tret once a week and then work my way up until now I can use it three times a week. It's really nice because it made my skin less oily like I can't believe at age 45.
This happened to me too - couldn't tolerate Retinol on my neck but Tretinoin is just fine.

Love this style video! The way in which you present these studies so we regular folk can understand them is so very appreciated too, Dr. Dray. This is exciting news! I am able to get my 0.05% tretinoin Rx from my GP, and fortunately my insurance covers it: my doctor provided authorization for its use by including my hx of basal cell and tret's efficacy in cellular turnover, and my insurance company okayed it, $3.95 for a 20 gm tube is my actual cost. I don't have acne, at age 67 I'm an anti-aging kinda gal, and am very interested in the exciting discoveries being made behind such reputable studies. That you bring us such such interesting and new data in such a "user friendly" way is just one of the many reasons we, your fans, think you're just great!

Better be careful the cream did nothing for my basil cell cancer, I had to have 3 in hospital surgeries it ate my nose up, the basil cell cancer. SkinBetter has a retinoid molecule combined with lactic acid molecule. It actually made my skin feel plump in the morning since the lactic is hydrating. I think this is a superior retinol to use. Great ingredients in it. I hope you analyze one day
Dr Dray actually did a video on the skinbetter science alpharet cream. Her analysis of it was that the product itself showed evidence of helping smooth fine lines and wrinkles, but not the active ingredient itself, which was the retinoid+lactic acid. She also said the major issue with their studies is that they did not double blind the study, meaning testing one group with the product that has the active ingredient in it, and the other group with the vehicle with no active ingredient in it, so it's really hard to tell what in the product is causing the anti aging effect. Another thing she brought up is that there are other ingredients in the alpharet cream that have been known to help with skin aging, such as glycolic acid and niacinamide.
I don't use alpharet personally, but a lot of people anecdotally report seeing better results with it than regular OTC retinol, so I hope one day alpharet does an unbiased extensive trial so that we know if the alphret technology is actually worth considering.

Interesting! I was using Skinmedica retinol .50 and felt I had pretty good results. Then asked my derm for tretinoin because it is the “gold standard”. I’ve only been on tretinoin a month but so far feel the skinmedica retinol was better for me. Will give the tretinoin more time but may switch back to the retinol.
It's important to give it time! At first I felt the same, but it has really made a difference in my 68 y/o skin! The texture is so smooth. I get compliments every day!

I use skinmedica’s highest strength. Using it for over 6 yrs. Worked wonders on my acne and pigmentation.
I'm currently working my skin up to prescription tret. I started with an OTC retinol, then graduated to adapalene alternated with retinol every other night. Next, after my skin acclimates, I will begin adapalene every night. Eventually I will go in with tretinoin every other night ,,,etc. It's just a gentle way to get there. ❤
I just got back from Guadalajara on Monday & while I was there I bought some Retin A Tretinoina .05% & also some 4% hydroquinona crema, both OTC. I am using The Route on my face but spot treating 1 tiny brown spot with the 2 others that I purchased in Mexico.
Interesting results! I use Agency tretinoin but I mix it with my moisturizer to avoid flaking and peeling.
I began with Agency at the lowest level and thankfully had no redness and flaking on my rosacea skin. Gradually increased in three month increments. I also recommend using a emollient moisturizer to lessen 😢any reactions

Honestly, if I were to buy a OTC product it would be Avene, retenaldahyde. Costs less, takes fewer conversion steps to become effective and has peer reviewed clinical data as well.

I use the neutrogena retinol in the grey jar. Fragrance free It works amazing on my skin and I have a lot of lines on my forehead and I’m very sensitive. My skin looks great. Just my opinion. I like to tell people if I use something good

You had me at skin biopsies! Sadly, my super sensitive facial skin does not play nice with any of the retinols I have tried. I switched to the Avene retinaldehyde 0.1% and trained my skin over a 5 month period starting at once a week and very gradually increasing frequency to nightly. I did not experience redness, peeling, stinging or hotness during or after the 5 month introduction period. So I have my Vitamin A skin product and am not likely to experiment further.

Boots (UK) have just brought out a 1 percent retinol version of their 0.3 percent product that you reviewed previously so I was really interested to hear about this study. I have only just started using it so can’t comment yet on its effectiveness. They have also brought out an accompanying retinol soother to help to combat the redness and dryness.
I love Skin Medica retinol! I’ve been using it for 2-3 years and have seen a big difference in my skin. I started with the 0.5 and now am up to 1.0. I’ve never experienced irritation, just a tiny bit of dryness in the beginning.

I have been using Avene Retrinal 0.1 Intensive Cream which I tolerate very well. I chose to use this product because my understanding is that as a retinaldehyde, it only takes 1 step to convert into bioavailable retinoic acid, rather than the 2 step conversion for retinol. Given this study, you refer to in Dermatology JAMA, would you recommend reinaldehyde over retinol for those who are concerned about the side effects of Tretinoin?
I have very sensitive skin with rosacea and my dermatologist recommended the skinmedica 0.25% retinol because I really wanted to try some kind of retinol for anti aging. And all the OTC products I tried previously irritated me a bunch. I got up to using it twice a week and it never caused any irritation. I always notice the next day my skin looks bright and bouncy, but I haven't noticed any changes in dark marks, and don't have noticeable wrinkles yet.
I also use medik8 bakuchiol serum every day and I love it!
I actually was and have gotten my prescription Tretinoin from my regular primary and have never had a problem, dry for a day or so but that was it. My prescription insurance is terrible so I use just a discount card. For about $90 which lasts very long and that's just cash. Cheeper compared to the cosmetics I had been buying for no results.
I’m going to try the skin medica retinol 0.25 on the neck. I can’t tolerate even retin a micro 0.06% on neck. But I want to incorporate something for my neck.
Wow, this is a really thorough study, albeit its limitations! I guess it would be nice to try Skinmedica's retinol complex, but not if I have to spend more than 100 bucks for the product (product + shipping) when I can buy OTC adapelene (15~30g) for 12~24 bucks. There are also cheaper retinol products which I could buy for around 25~50 bucks. I would just buy those and use the rest to buy sunscreen and a couple of sturdy jeans (because those are good at blocking sunlight for a non upf clothing).
Oof yeah that price is rough. Wonder how long the bottle lasts. I get tretinoin + azelaic acid formula from Apostrophe and a $75 bottle lasts me like 4-5 months.

Question: Skin Medica Retinol issue for most users is to graduate to application on areas such as under eyes, neck and around mouth.
Most can graduate to higher strength but can’t get over the sensitivity in these areas
The product shown in the video, SkinMedica Tri-Retinol Complex ES is not sold SkinMedica’s website. In the written part of this video it has a link to buy another retinol from this company, but that product wasn’t used in the study.
The Golden Rule by The Route is the best. It is a next generation retinoid & can be used everyday by anyone. I know one of the 2 founders. She is my N.P since 1999.
Ever since you mentioned the advanced clinical retinol cream for the body, I’ve been using it on arms and legs regularly for a year, and it has improved texture of my body skin, though not as dramatically as tretanoin on my face. I think it’s probably a very low concentration. Of course, sun protection before and since starting the retinol. I hope it suppresses pre cancer cells.

I tried skinceuticals retinol 0.5 and my face peeled a lot and felt pain when applying other products till I got use to it. Now trying their retinol 1.0, the peeling was not as bad but I also used less frequently.

I appreciate the information on the study. My question concerns the strength of tretinoin that was tested. I understand the tretinoin was .02%. Aren’t most tretinoin prescriptions written for .05-.1%? Perhaps they chose the lower strength because they were comparing it to an OTC retinols. Thank you for your honest and educational videos!
I think your a little confused. The retinol was .02%. You cannot get tretinoin for more than 1% which is what I use.
and the lowest tretinoin you can get is 0.25%.

Hello Dr. Dray, thanks for your very informative videos. After watching this I did a little research and I'd like to know your opinion about Paulas choice 1% retinol or the ordinary 1% retinol. Paulas choice in addition to peptides also contains vit C which skinmedica doesn't but the latter has niacinamide. That's the major difference I noticed. what do you think?
I use the PC because it's just more cosmetically elegant than TO and I use a lot of her products. I use it on my neck as my neck doesn't tolerate my tretinoin. I'd say it goes down to budget personally, because the price differential is huge. You can always use a serum or a moisturiser with peptides and niacinamide in it.
There's other products of PC that are low to mid-strength and cheaper than mine. I use the Clinical line which is the most expensive (purple packaging). 1 day of the 1% Clinical Retinol on the neck, 1 day of the Clinical Ceramide-Enriched Moisturiser on my neck (0.3%). Keep alternating. That moisturiser is hella expensive. I wait for a 20% off sale.
Oh I just looked at the price of Skinmedica LMAO. It's double the price of PC.
As someone with the most sensitive skin (I am 55 with the usual number 11 between my brows) I tried a prescribed tretinoin and no matter how I used it, it was just too strong.. I tried the Olay 24 which was fine but it always pilled off my skin and I didn't like that. I bought the Garnier Retinol Berry (green bottle) and I absolutely love it. I used it every 4 nights at first and sometimes less but now I can use it every night. I definitely see results and feel a lot better about my skin. I highly recommend it if you have sensitive skin and it's at a very affordable price point. I was happy that Dr. Dray had recommended it in one of her videos as well.

Thank you to Dr. Dray for pointing out this study. There is something really weird about it. Something we have never seen before with a clinical study. The actual study was done more than 10 years ago and they only decided to publish it now. That weirdness is underlined by the fact that several well known academics on the publication are funded by SkinMedica.
it struck me as fishy that the chosen tretinoin concentration was low on the scale of available concentrations. more useful would be a follow-up comparison that uses the .1% tretinoin.
@@eee3400 to be fair the strength of tretinoin used in anti-aging studies is 0,05% so that would be an ideal concentration for comparison
