  • Dr. Rich Blake discusses the overall success rate that he has seen from sesamoid removal. He warns that the area must be protected forever, due to only one sesamoid remaining, post operatively.

Hi Dr, Thank you for sharing your knowledge.
I've injured my sesamoid in november 2021, and after nearly 2 years the pain has been debilitating and has worsen. I've had two times cortisone shot in 1rst MTP at the hospital, and the 2nd time past february, there has been an infection which took MONTHS to be finally treated because the evolution caused a cellulitis twice this summer in my 1rst MTP joint. The pain is still present and I had to stop pretty much all my activites (sport is my life...I used to do lots of running cycling etc.)
Now even at rest I have lots of pain. I need further scan to make sure there is no infection still going in my joint, and to see if I have AVN because I am at high risk (I only have 1 artery in my left leg, and the cortisone shot caused a vascular trauma, I have no dorsal pulse and have severe Raynaud syndrom. The orthopedic surgeon I saw today was quite pessimist about the treatments options. I tried already wearing a boot for 3 months with no improvement (that was 8 months after the initial beginning of my symptoms).
She told me that even if she operates that 50% of people are unhapppy with the surgery.
What do you suggest? It is very depressing😭I wish I can find a sports orthopedic surgeon who will understand the importance of my foot for my athletic life and passion.

Sorry to hear what you're going through. I'm going through pretty much the same thing. Will be two years next month. Just wish I had a time machine to go back before I injured it...

Hello Doctor, I’ve had sesamoiditis in the fibular sesamoid of my left foot since 2019. In September 2022 it evolved into a stress fracture. After 6 weeks in an orthopedic boot and plenty of physio, orthotics and better footwear, I was back to 80% of my normal capacity. Unfortunately I developed tendinitis in my right foot over this summer and that has caused a flare-up of the inflammation… I’m considering surgery and will see my surgeon on 1 September. Last year the surgeon talked about screw fixation, but I’m afraid that the problem may reoccur when the screw is taken out as my patience has already been tested for a long time with these multiple injuries. At this stage, would you recommend going for a removal as opposed to screw fixation? Is there a way to replace the bone with another material? Thanks.

Hello Dr Blake, I'm from the Netherlands. And I have an sesame bone fractures. My orthopedic surgeon don't want to operate on my feet. A year ago I broke it. I had a cast, special shoes made but nothing helpt me. I'm out off options. Why would they not operate me ?

Manon, sorry to hear! It is a common surgery! You try conservative treatment and if the patient still disabled, you remove the bone! Are you stuck not getting 2nd opinion?? Rich

Hi Dr Blake, I am scheduled for surgery next month for a sesamoid removal and am very nervous I know that you had already talked about success rates but wanted to ask your opinion on weight lifting after surgery i know it would take some time and recovery but I am afraid that the surgery could make my problem worse and I would not be able to workout how I would like but I want to get rid of the problem if possible I am a 28yr old male and was told to try to avoid surgery by one doctor then another doctor said she was very confident in a good recovery I would appreciate your opinion on the situation, Thank you

Hi Dr. Blake, Is this possible to get in touch with you for a consult? I have been experiencing pain in feet and legs (both) for 3 years now. I would really like to get your expert opinion, you seem to have an expansive knowledge in this area. I am hoping I can get few answers. I live in California.

Hi Dr! I had my left foot lateral sesamoid removed on September 26,2022. I have had a wonderful recovery up until today, a year and month later. When i raise my foot I have bad pain shoot through the removal area. Is it normal for after a year of surgery to start feeling pain sharply again? Hurts slightly when I walk on it but hurts mostly at rest and when pressed.

Could be many things! Sometimes it is scar tissue that needs some PT. Could be some nerve issues off the back! Best to ice 5-10 minutes twice a day, spica tape if you can during the day, wear most protective shoe gear, and limit activity? These are for only 4 days to see what happens! No painful massage is always good. Rich

Hi Dr. Blake, Have you performed or know of other podiatrists who have performed bilateral medial sesamoid removal?
