DEMON'S SOULS: How To Get The Makoto Magic Sword.

Magic Sword "Makoto" is a rare katana in Demon's Souls. It can be looted off of a corpse in 4-2, found in the underground cave underneath the first Reaper room. This is the same underground cave Patches the Hyena kicks you into, and the same room where you rescued Saint Urbain. The corpse is only accessible if World Tendency is Pure White.
Magic Sword "Makoto" special property is that it drains the user's health slowly whenever it is equipped. It has very high base damage and long range, but no scaling with any stat. It is a great weapon for a weaker character, but will not become any more powerful as you level up.

Fun fact: you only need to kill 2 of the bosses for you to get enough tendency to get the sword
Just make sure you DO NOT die at all or else this doesnt work

I agree but the point of not die at all is only applied when you start the world in human form or continue playing as human when kill a boss but it's always good to get back to the Nexus and kill yourself when you get revived after killing a boss. That would make it a lot easier than being stressed by not getting killed in human form. Appreciate what you said thoug.

To be honest.... No! It drains your health as soon as you start using it. I'm using a different sword which is a lot better. Great game though.

When i equip the mokoto my health bar starts decreasing how can i solve that

It's a side effect unfortunately. Maybe a ring will counteract the effects???

Regeneration ring or adjucator shield counteracts it

OK, so basically you need to go through the game in spirit form. Every time you defeat a demon, go back to the Nexus and kill yourself and don't forget to pick up your souls. That way you should keep your Tendency white.

Have you killed all 3 demon's in this world? If so, try leaving and then going back.

If you still don't see him then most likely you need to have white tendency
