Hey! Thanks for putting these videos out. I take my NCLE on the 17th of December, and I've been studying for months. Reading everything, and writing most of what I read so I remember it. These videos kind of help me cap it all off and sum it up! I really appreciate the detail you go into. The one thing reading it and writing it doesn't always do is help me visualize it. Very helpful video, thank u. I have aquestion though, you mentioned the case of 3 and 9 o'clock staining in the WTR astigmatism. So how can we act in case we have that? What modifications we should do to a lens so we avoid this?
Great question. The answer is - you can’t avoid it! Wife the cornea is steeper along the vertical meridian Fluorescein will create this pattern. What you CAN control is any unnecessary pooling!

Hi. Thanks for the videos, they’ve really helped. Can I please ask, when/how do you choose what type of fitting to do out of interpalpebral or lid attached fit? Is it simply determined by the patients lid position?
Hey, Allan. That will generally be up to the contact lens fitter to choose which type of fitting philosophy that they would like to pursue. On the test this will be noted for you.
By default, as you stated, one’s lid position (interpalpebral fissure) will dictate how the lens acts in situ.

I have a question that’s not related to the video. I passed the ABO but after learning more about contacts, I have found that I’m way more interested in the contacts! Im in a license required state but my question is what type of work could I go into that I get to work more with contacts over glasses?

Yes - Contact lenses are overall, interesting!
You could work at a larger ophthalmology group as a contact lens technician, or scribe. Many times you can work alongside OMD’s and ODs... You could also be a sales rep for one of the major contact lens brands, or you could work for a GP or speciality lens company as a product/fitting specialist (the ones the docs call and give K readings, power, etc. to for guidance on fitting)

The best advice I can give you is to familiarize yourself with the simulations that are available on the ABO-NCLE.org website. Congrats, and good luck!
For medial and lateral will they refer it as adduction or abduction?
Good question.
Adduction is towards the midline but it refers to the “movement”
Abduction is away from the midline and again refers to the movement.
It could say “The patients left eye abducted…” not likely though 😊

Would it ever be appropriate to send a pt home with a gp lens empirically fit without checking the lenses with a slit lamp and nafl, .

Even if the data suggests a lens would be a perfect fit, no two eyes are exactly the same so it’s always best practice to check the fit to make sure it’s not tight, flat etc… on the cornea. Thanks so much for watching 📺
Sam . , in the in the Contact lens Review book on pg 71 .. WTR and ATR are opposite. Please help I take my NCLE in 2 days
WTR is where the strongest power is near the 90
ATR is where the strongest power is near the 180
In a minus cyl Rx WTR will present itself around the 180 (this seeming opposite) but if you transpose you can see that the greatest power is along the 90

I’m referring to the fluorescein patterns . 9-3 dumbbell the NOA contact review book shows a picture and it says it’s with the rule .?
