AJ and Mario have LONG since conceded that it was a broken halfshaft. It's stupid fans that don't get it. I have Foyt on record multiple times as saying the halfshafts back then were fragile. This just happens to be the most high profile case of several that happened. It was a very real problem that was exasperated with the fact that the failures were rarely identical. And it wasn't until the late 80's that we see that it's resolved. Crew chief Derrick Walker also stated "We didn't come to Kevin's defense like we should have.
Such an injustice that Cogan took all that crap for all those years for that one accident which likely wasn’t even totally his fault. He didn’t deserve that
Cogan screwed up but man look at how slow they’re going. That may have contributed to the wreck. Apparently, Rutherford, Unser, and Johncock complained about Mears bringing the field at such a slow pace. Also, Johncock went on to say that Andretti could have avoided the wreck if he hadn’t jumped the start.

Nothing broke! Cogan got on the gas to hard when shifting on cold tires! Sam Posey was also the worst announcer ever at Indy, he said some stupid stuff.
Pretty much the same thing that happened to Roberto Guerrero at the start of the 1992 Indy 500. Too much power on cold tires and just lost it. Nothing broke.

If Mario would have been in his starting position, even with the second row, he probably could have missed Cogan. Mario was a few car lengths ahead of his row.
