I've got a write-up regarding gear in my discord, but for M+ it's enduring dreadplate(from kazzara) + stormeater's boon(from dathea) if you're willing to play double on use and aren't afraid of self rooting for 10s. Pretty risky, so might be better off running dreadplate + cataclysmic signet(from smolderon). If you don't plan on raiding at all, I would go for bottle of spiraling winds(nokhud) + erupting spear fragment (neltharus).
For raid/ST always elementium pocket anvil(amalgamation chamber) + cataclysmic signet if you can. With S4 tier set puzzlebox is a decent alternative.
