      Hi medicosis perfectionalis, i have a doubt
      Why is ascitic protein >2.5 in post-hepatic portal hypertension (budd-chiari, CHF) even though it's a transudate ?
      If water flows out into peritoneum, shouldn't "ascitic protein" get diluted and become low ?

    lymphoma & TB military will have SAAG = 0.8 (A)
    Budd Chiari, Cirrhosis w/ increased hydrostatic pressure, & CHF all will have the SAAG of 1.3 (B)

    This concept always boggled my mind until you put it into terms of exudate and transudate. Not sure why I was overcomplicating it! Thank you!!!

    Patient 1 and 2 ➡️exudate (saag=0.8). Patient 3, 4 and 5 ➡️transudate(saag=1.3)

    I don’t understand one think why nephrotic syndrome is exudate ? I read it in Davidson 22th edition that nephrotic syndrome is transudate ascites while in 23rd edition Of the same book that NS is exudate?

      bcs of losig the protein (albumin) in the urine so its exudate

    What if i have a SAAG of 0.8 which favours exudate or transudate of a low oncotic pressure orgin but the fluid albumin /serum albumin is 0.75 which suggests an exudate what is the discriminant indicator in this case

    Dear doctor, how we can stop ascites,after tips procedure and replacing a stine in the liver?
    We did the tips procedure and our patien still has ascites. Doctors tell us, it is due to lack of albumin.
    Will she be OK?

      @@MedicosisPerfectionalis I heard reference to it as it passed in impatient conversation. I was very much hoping that 'it' was a 'solution' to a condition of acities, - one which demands attention now, - almost every-other day.
      It's my wife, and I grow more worried and anxious with each passing day.
      Question: would a 'shunt' at least help with this ever-
      growing accumulation of fluid in her abdomen ? Her appearance is becoming LITERALLY very frightening

    Why does oncotic pressure decreases in hypoprotinemia?? Somebody pls explain

      Because the oncotic pressure is caused mainly by the albumin protein (because albumin is the most abundant plasma protein, and osmosis is a colligative property that depends on the number of particles).

    Hi! I got a quesiton here. Why is that decreasing oncotic pressure (probably transudate) belongs to the category of SAAG

      Well, I believe if I was following right, its because the decreased oncotic pressure (like in nephrotic syndrome) has to do with loss of proteins from the plasma and so the serum albumin on its own is already low, in a nutshell you are trying to measure what's not even there 🤷🏾‍♀.
