Recurrent UTI (Urinary Tract Infection) Lecture

  • What other pain in the body do we not examine? I argue that the pelvic exam is extremely underutilized in the evaluation of recurrent urinary tract infection.
How do we actually define a UTI?
How to intelligently interpret the UA and only use this as a screening test. The Urine Culture is necessary to confirm the infection and that the presenting symptom is actually due to a bacteria.
What is the recommendation for an acute UTI by the Infectious Disease Society?
What do you do when people have multiple antibiotic allergies?
    Behavioral strategies to reduce UTIs.
How we can change our gut microbe and change our UTI risk.
How your vaginal health affects your UTI risk.
How your microbiome affects your UTI risk.
How the post menopausal state increases the risk for UTI.
Therapeutic interventions for UTI: suppressive antibiotics and the risks associated with antibiotics.
Does cranberry work? How about Vitamin C? Any prescription preventative measures? UTI Options.
When is a cystoscopy a good idea in the workup of a UTI?
    Antibiotic stewardship
Look at the “owie” - Other things that mask as UTI
How Dr. Casperson works up a patient with recurrent UTI

I have recurrent UTI's and I am NOT dehydrated. I drink constantly all day long. The first sign I usually get of a UTI is a strong odor which evolves into burning soon after on urinating, constant feeling I have to go, and a heavy sensation in the pelvis.

dear doctor-- you have very poor training.... You guys prescribe medications only based on a positive urinalysis, a test that is 50 years old and wrong half the time. Secondly you assume that we are getting recurrent utis, when most patients have the very same EMBEDDED CHRONIC uti, because u do not prescribe antibiotics for long enough time...Dogs and cats get a minimum of 14 days of abx, yet humans are typically given 3-7 days. Did you know tht tens of thousands of women are either flying to the UK to see an office that deals with embedded chronic uti's and treats with high dose---long term antibiotics...UNTIL patients are positively bacteria free. Plus asymptomatic patients would never be in your office to begin with... why are you even speaking to it. You are assuming that doctors are throwing antibiotics at patients, when it is the total opposite - u are UNDER PRESCRIBING and thus causing resistance.. I urge u to restructure the way u r treating UTI's...You are saying the same old, same old, meanwhile you know what they say, - 'keep doing the same thing, and expecting a different outcome...not going to happen'. You are right about one thing, just as you have been taught wrongly about the urine not being sterile, you are beung taught INCORRECTLY how to treat a UTI and how to diagnose one to begin with......Check out Dr. James Malone Lee...and google Embedded Chronic UTI...Game changer...

Excellent presentation for a 64 year old female retired nurse with chronic infections every 3 weeks (basically as the bacteria build up from antibiotics). I do all of the proactive strategies but I have a triple pelvic floor prolaps with urethral opening stretched wide and internal tissue exposed. I weigh 112lbs and always do jegal exercises. I get about 3 hours relief from sodium citrate but you can only use it for 2 days at a time. WHAT if anything can I do until covid allows for prolapse surgery? I would love for you to have made it more informative on the menopausal woman. Perhaps a second video? I use estrodial pil l in the vagina twice a week. No use. 😰 The vulval pain and bladder ache is miserable. The freezing and sweating is miserable. The relief after a 7 day course of antibiotics is sooooo lovely. Thanks

If you have not had your surgery yet, you can always be fitted with a pessary. A pessary is a small medical grade silicon device that is inserted into the vagina to support the prolapse. It's a nice interim option while you wait for surgery. Also the "vaginal pill" you speak of is useless. You are quite right. It was a low dose tab to begin with and the company went lower dosing knowing full well it wouldn't treat moderate atrophy. Basically it was a company move to profit more from women. I would ask your doctor for Premarin Cream or Estragyn Cream.

I had 10 UTIs in the last 1.5 years, probably caused by chronic constipation. E.coli culture showed a couple of times, and I got BV (also E.coli) and yeast infections when I wasn’t on antibiotics. After developing a very meticulous washing routine after sex, I saw my symptoms change over the course of the year and got less painful. I was able to manage it with D mannose, probiotics, and now I believe I need to lay off the alcohol as well as reduce flour intake to feel even better.
The pooping on a consistent basis is a great reminder! I also drink lemon water and urine alkalinizers from Uqora. I avoid antibiotics like the plague, just because the pain isn’t so great anymore.

My questions are: 1. Is the bladder supposed to be an acidic organ or alkaline? 2. Is having UTI issues during ovulation give me a clue that my estrogen levels are bouncing too high? So do I see an urologist or my gyno? 3. How often is medication the issue to occurring UTIs? (I have RRMS and take Vumerity). 4. Is low bile function possibly linked as well? And I ask because you did talk about poop a bit. Low bile can cause a fattier, greasy stool.

I have also found that using estrogen cream on the vulva has truly helped my skin heal where a lot of the infection started in the first place. I hope this may help you.

The only test the has consistently been accurate for me is the PCR/Next Generation sequencing from MicrogenDx. Also, I hate to break it to this woman, but having a sensitivity to an antibiotic can be just as bad as having an actual allergy. I’ve almost died from antibiotics that I wasn’t allergic to. And I have chronic gastritis, so taking anything that hurts my stomach can trigger it and then I’m in serious pain, and honestly-on top of everything else-it’s really just not worth going on for me. Also, she’s not addressing what happens when somebody is doing everything right and still dealing with chronic or recurrent (or both) UTIs.

uhm...I'm no doctor, so don't take my word for it and ask your doctor, but I don't think YOU are be coming resistant by taking antibiotics a lot. The problem with you is more your overall health because yoi continously wipe out all your good bacteria in your gut.
The problem with antibiotic resistance is that the BACTERIA gets resistant, not you. If bacteria mutates to be resistant to antibiotics, that is a problem in our world because we then don't have a way to treat it anymore. make sense? so if you treat a stem of bacteria with antibiotics over and over again it can become resistant..

What an amazing urologist. And I found simple things to try, because my microbiome wishes me to avoid antibiotics! So, lots of water, vitamin C, perhaps vaginal estrogen, and then I'll see how things go. Thank you so much.
Update: my infection eventually got worse and I was becoming alarmed. I eventually asked my doctor if I could try nitrofurantoin, an older antibiotic that works primarily in the bladder, and has minimal effects on the gut microbiome. And it worked!! Such a relief. And my gut seemed unaffected. Apparently it's prescribed less often because you have to take it twice a day for up to a week. But it's well worth it for me

    • @judithenglish
      @judithenglish 2 года назад +5

@@Olivia-bq9rk Hi. I think the estrogen is probably more important for those of us whose bodies no longer produce much. Topical estrogen stops the shrinking of the inner vaginal lips in post-menopausal women, since those lips help protect the urethra from bacteria getting into the bladder. At 15 the causes of your UTIs could be very different. I remember the pain of that and I feel for you!!! One thing that might apply is that I'm working hard to avoid antibiotics as much as I can, particularly Cipro if possible. But I understand trying anything that might work. I hope you find a solution.

I looked up uva ursi, and it seems interesting. But I'm scared off by possible side effects to the liver. So I'm sticking to the d-mannose and making sure I drink 1.5 litres of water every day. I'm also taking 1000mg of vitamin C every day So far this seems to be working. I just found this, First off Merry Christmas, But let me say, this, I have been going to Dr after Dr, then to urologist here in Anniston, Al, (2 of them) and I kept telling them over and over again my problems, had 3 scopes sent me for 3 CTS within 2 years, I mean yes I usually have 3/5 UTIs a year ( PCR Urine Culture) I have been put on SO MANY ABX in the past 3 years, FINALLY started going to UAB GYNOUROLIST, finally put on Estrogen Cream and Tablet as well, but they still put me on an ABX before getting culture back and even if it's neg they tell me to finish it. My last Culture came back with Entercocussus F. And it was only sensitive to 2 ABX ( Macrobid and Cipro and Vanco IV) Macrobid worked. BUT my thing is and I KEEP expressing concern of not not putting me on AB if I truly do not need it. I ALWAYS feel like I have a UTI, BC I Pee so much, sometimes burn, and I'm , on Macrobid now although my culture came back neg this week and was told to finish it anyway, I was on cefidinr 2,5 wks ago for sinus infection, I'm so scared Abt all these abx. I'm 50 years old, was told by Blood work I'm Post Menopausal, and have Crohns as well. All I know is I listened to you and let me tell you I'm gonna take your advise and I'm actually gonna ask my Dr to listen to this video. I feel like they get so tired of me, bc I feel like I have a UTI so much. Like it's so much stress and I cry so much bc I get no help. Just listening to you made me feel like there is still hope for me. Ty so very much. I wished you was here to be my Dr, bc I would absolutely be ur patient and I would follow ur orders with no hesitation. I'm just setting here wondering if I need to finish this Macrobid.,,And they also tell me to take Macrobid 100 mg for a prevention. They said they may change me to CIPRO for a prevention. But I have been. On it alot as well. Idk anymore, I'm gonna ask my Dr to listen to you, I'm pretty sure she will give me the evil eye. Lol..... I'm thankful I understood this video and the medical terms .

Can I have UTI without any pain?

Im a male who got uti that lasted on and off 1 month. I was taking biologic humira for mild psoriasis. It wasn't worth the side effect. I quit humira and will probably as a male, never have another uti

I took biologics 7 years and dealt with UTI's for a very long time, but it was worth dealing with side effects because i have like 10 years since i got my disease in remission.

Throwing my own thoughts in, here. I STAY hydrated. Period. My thought is that instead of looking to answers from the medical world, it would be so much better on our health if we look for natural remedies. Example, see what Dr. Eric Berg has to say on the subject. He is correct about 95% of the time. 1-10-2024.

10 years ago when I started seeing my then boyfriend I got regular UTI’s because of sex. It was so tough to deal with. What I learned was prevention is better than cure. If you can find a routine and diet than can help prevent UTI’s you can avoid antibiotics.
For me going to pee after sex is vital. An I also wash myself down with water to flush away any bugs.
I try to take D mannose before an after. I sometimes take it throughout the day too as precaution.
Less caffeine, sugar and alcohol if you have had a recent infection.
    Vitamin C an probiotics daily.
I’ve gotten my second UTI this year, again related to sex. I have to restart the program I made for myself 10 years ago. Hopefully il be back with good results in no time.

This video made me grab a mirror after a UTI every 2 weeks after sex (STI free) and now I see my vagina is very raw in places and shedding. I must have a yeast infection!😮 either from the antibiotics or this is the cause of my pain!!!! Waw.

Thanks so much brilliant video. I have a question I understand what urgency, frequency and burning is but when you have a genuine infection (pathology lab tested)what do you call the feeling you get towards the end of your wee that makes it seem like the bladder is trying to push out some thing more out. At times I have given in to the sensation and actually allowed my body to strain and I noticed what looks like fresh blood in the urine after. I try not to do it but its like your body takes over pushing a child out it seems like it’s necessary. It feels so much like a punch in the bladder or something twanging on your urethras… It literally makes me groan out loud at the end of each wee, for me it’s the worst symptom of UTIs. It takes so long to explain that symptom over and over and know there must be an easier way to say it. Or just say if that feeling isn’t normal for UTIs, just like burning isn’t something that happens to me with UTis.

Amazing info TFS it. I looked to see if there was a video about chronic pelvic pain, couldn’t see it. Can some one point me to that video if you know of it please. I have chronic pain from several conditons not normally pelvic related, so would there be a higher propensity for this condition in someone who has other chronic pain. At the moment I understand these are path lab tested UTI but I have other pain at other times that may not be.

I thought urine was supposed to be acidic to prevent UTIs?

    • @Me5534
      @Me5534 3 года назад +1

She said vit c with methanimine which makes acidic urine. Methanimine works best in acidic environment so that would go against her alkaline theory.

I'm the problem patient lol. I usually get a zpak or macrobid because I'm allergic to everything else. But I've been taking dmannose when I get symptoms and it usually helps flush it out within 3 to 5 a days unless it is a severe case.
