Yeah the shadow figure is Aqua's previous life, the doctor called Gorou.
Note Aqua and Gorou have different VAs, so it's clear on this when the ending credits did show Doctor himself in the cast.
It might be a split personality deal. Where part of the "doctor" is still attached to his new life as Aqua.

There's a pun here that matches this anime. Incest and six places. They have the same pronunciations. Incest = Incesto. Six place = Em sexto. Pronouncing six place = Incesto, you say incest place
I saw some comments on Twitter, saying that this is the last good arc of the anime. Really? I don't know.
