Surgical Transition: Underlying Issues, Ethics & Regret (Dutch Documentary - English Narration)

- Transgender people are becoming more widely seen and accepted. There are many positive and celebratory stories about medical and surgical transition in the media. What is less commonly seen are the stories of individuals who transition and then realize that it was not the right choice for them after major and irreversible changes have been made to their body.
In many cases, these regretters originally presented with multiple complex underlying psychological issues - however the affirmation-only model of treating these individuals leads to their gender issues becoming the priority, rather than first addressing such co-morbid conditions as depression, PTSD, anxiety or body dysmorphic disorder among other considerations.
Stories of regret and detransition should provide important information to doctors and institutions about the ethics of these interventions and the criteria for recommending patients to surgery without first addressing underlying issues. This type of reflection and revision of clinical criteria, protocols and policies does not appear to be happening, however, possibly due to strong pressures made by activist forces both inside and outside of the medical profession.
In 2018 a Dutch documentary aired which looked into one such story of regret and examined the medical institutions and clinical culture which led to the tragic outcome for this individual. Also examined are the influences of social media and the phenomena of regretters who disappear from the view of clinics and the public due to the irreversible harm they experienced as well as ethical considerations in selecting patients for surgery and even the ethics of allowing euthanasia for patients harmed by this therapeutic pathway.
The documentary is presented here with english voiceover based on the original captions.

I know this is a very serious topic, but the way the Serbian doctor talked about collecting penises cracked me up. It just shows how utterly bizarre the world of transgender care is. I wonder what else goes on in the medical world that we aren't aware of.
Same here. It's not normal. What would be normal is a situation where no surgeon would agree to remove healthy body parts, because that would go against his work ethics. But I guess work ethics isn't something very trendy among plastic surgeons.
God gave us a sense of humor to help us deal with difficult things. Don't be hard on yourself for being human 🙂

Exulansic covers a lot of horror stories regarding SRS, phalioplasty has especially high complication rates with absolute horror stories of repeated "corrective" surgeries that go on for YEARS, colostomy bags, MULTIPLE catheters, constant leakage of pee, they go in ABLE to pee, with the goal of standing up to pee, and end up NEVER being able to pee again (without catheter and pee bag) and at the end she says "SUICIDE PREVENTED".

Per a study about people who are post gender affirmation care and still committed suicide states the reasons:
The psychological autopsy of the completed suicides among transgender persons has revealed that the factors such as break-up of love relationship initiated by the partner (64.3%), serious altercations with family members (14.3%), refusal of gender/sex reassignment by the family members (9.5%), financial problems (9.5%), being diagnosed with HIV positive in the past few days/weeks (2.4%) have triggered the act of suicide among the victims.

@@ApacheMagic First, thank you for believing that I single handedly produced a scientific study that was not only recognized but published by my peers. Secondly, there are many ways they can get the information for a study of this nature such as interviews & therapy notes from the individual, interviews with the family, su*cide notes, videos or social media posts from the person. In one recently I saw from the Netherlands, the person was seeking out physician assisted su*cide and had a file that was about 8 inches thick that had to be reviewed by five separate doctors. There are many ways to get this information. Also De-transitioniers have given a ton of information about how they felt post surgically. But if you don’t believe me, Google how a suc*de study is conducted. Because they have done them for other reasons and topics before.

I am so tempted to say something similar but how "easy" it is for us, who don't have this particular problem, to say this. But either way, I send my very best to Patrick and all who have been through something similar. I'm afraid we're not going to see the whole havoc for many years. Also, I'm not one who normally cries "sue their asses off", but this is such a clear case, with such devastating results, that I hope he does exactly that.

The way you talk doesn't sound like you understand this part also explained in the Dutch documentary, his psychology.
If he really wanted to die - he could, euthanasia and suicide... People regret nose jobs and BBL - but we don't have the same conversations here. He also could get a re-reassignment, but that would also not fix his personal and psychological problems. He wasn't trans to begins with!

It's frightening how many parents are pushing this confusion on their children. These documentaries are so important. With some doctors in this field it's about money and not the person.

@@michellezoske7581 the kids are pressuring parents as well. I know some parents who went through horrible pressure from kids and others. It’s trendy in schools, and they don’t take no for an answer.
A lot of the children are testing their boundaries asking if the parents love them enough to say No. They are all feeding off each other that is why they all say the same arguments. I'm a grandmother so I've had alsorts Verbals, tantrums from my own 3 Children.

The main issue is encouraging young children and teens to change their genders. Yet they have to wait until maturity for many items ( military service, any other medical procedures, marriage). Age causes maturity, waiting until they can determine whether it’s really what they want. Our brain is still developing for such a Monumental decision.
Age also causes loss of innocence. The adults sculpting these barbaric procedures are not innocent.
Surgical mutilations to treat a mental disorder should be criminal.
Jordan Peterson has said that the literature has been quite clear for many years that for nearly 90% of these younger sufferers, the problem works itself out by the age of 19 or so.

These doctors know they are dealing with mentally ill people then act like they are surprised when a person has regrets. Gender dysmorphia in itself is classified as a mental disorder and if a psychiatrist cannot determine the difference between someone having a manic episode or some other kind of break with reality and genuine body dysmorphia then how can the patient decipher between the two? There needs to be a consensus between medical professionals instead of just doing what the patient wants them to do. In what other medical practice can a patient come into a doctors office and tell them what their own treatment should be? They can tell a doctor but that doesn’t mean the doctor will agree. In these transgender clinics and hospitals it seems the doctors just agree to whatever diagnosis the patient wants to give themselves and their own treatment and surgeries. It sounds ludicrous when you actually think about it. Imagine a patient coming into a doctors office with anorexia and demanding that the doctor give them a Sleeve Gastroplasty? Or someone that has Body Integrity Disorder and a doctor cutting off their healthy limbs. Sounds insane doesn’t it?? Well, this is exactly what these doctors are doing to patients and people are celebrating it. I’m not a psychic but I can see in the future a lot more people coming forward with regrets. I think there are probably more people right now then these doctors want to admit to. I feel sorry for these people. I’ve heard of young people being told on forums that if they are feeling depressed or have anxiety it might be gender dysmorphia and that surgery and hormones will cure depression. Of course these people will feel duped and ashamed and when anyone does come forward to tell their story the trans activist brutally go after them and shame them and guilt them into staying quiet. The bravest thing you can do in this culture of craziness is to speak out. Don’t let other people go through what you went through. Having true body dysmorphia is very rare and I believe that there are really people who have this illness and need treated but doctors also know that gender dysmorphia is very rare so seeing a significant spike in numbers like this should have given them pause. They should have waited to see what was really happening in our culture instead of jumping on the trend just to make a lot of money. They are ruining people’s lives. They are tearing out and cutting off healthy organs. They will gladly give these young people a lifetime of pain to line their own pockets and when the 💩 finally hits the fan and I promise you it will eventually hit the fan they will make all kinds of excuses and justify their reasons for doing what they have done but make no mistake it was all about the money for them. They are no better than any other horrible doctor that did “research” and experiments on healthy people in our past. We look on them as horrible people now.
Btw, thank you for making this video. I know it must be tough in today’s climate. It was done really well and I shouldn’t generalize all doctors. I know there are some that disagree. I’m just angry and annoyed at the ones who just want to line their pockets. It’s not all doctors. I’m angry for the young people who are being hurt, medicalized for life and sterilized.

I‘m relieved that there still are some concerned caring psychiatrists who get that correlation! ( Many mental issues as underlying disorders in „transpeople“ who are not trans but think they can enter the „happy world of trans“ and all other suffering will be gone. That‘s how it‘s often presented and that‘s very dangerous for vulnerable people as we can see. Why are people so against thoroughly exploring, questioning, diagnosing ?!
Those „real trans“ should be happy about it if they are so sure about themselves bc it will free the way for them to get treatment, so what‘s the problem. Don‘t you want people to be happy? What I also think needs to change: Every patient who is a candidate for surgeries ( SRS) has to be informed that they never will have real functioning organs and that there are huge health risks. They need to be told that they will never feel physically and sexually like people of the other sex. It‘s an imitation and not a good one bc there‘s no technology that could offer it at this point. That‘s reality and if someone still likes to go along, have surgery, they at least know a bit better what they are going to deal with. It‘s only fair.
For Patrick I feel very sorry. How could that happen when he wasn‘t even diagnosed as trans and obviously had many other issues mentally?! That is horrible. I‘m worried that happens a lot these days. These are real people, not „cases“, please dr‘s and therapists, be aware of that!!

For some patients seeking "affirming" surgeries, and their surgeons it seems that one or both are both suffering from narcissitic personality disorder. The patient seeks the affirmation of the medical team and the medical professional seeks the affirmation from the patient that his surgical prowess is the superb solution. It is problematic when either patient or surgeon is NOT narcissitic or sociopathic. It seems inevitable that people will be harmed.
The idea that the medical establishment is selling is ultimately a false one and for this reason, it appears there are more surgeons and medical doctors suffering from this trait than the other way around. Those seem to be the ones directing the current guidelines from above.
Gay rights activists fought for the right to tell the truth about themselves. Trans rights activists and the medical teams affirming them, via these treatments and surgeries, are fighting for the "right" to lie about themselves and to force everyone else to lie to them as well.
Follow the money.
Everyone with gender dysphoria should be psychiatrically examined. Juvenile surgery should be banned! You must be old enough to get a driver's license, buy alcohol or cigarettes or to get married, but you can change sex as a child. The consequences are irreversible. For me, it's a crime

If a patient went to a doctor and said, "I dream that I shouldn't have any legs, I want to amputate my legs, I want to be a paraplegic." Used to, the doctor would refuse a patients wishes that would cause harm, be detrimental to their health, and get them psychiatric help..not anymore. Forget the "shall first do no harm" oath..Now the oath is only to money. "Shall first make money and make life long patients for more money"...

I disagree with the idea of assisted suicide for depressed people. As a depressed person I don't need any encouragement in that direction. I think that's criminal. A dear friend of mine lives in the Netherlands who suffers from Parkinson's disease. He plans on taking advantage of euthanasia when he's tired of dealing with his disease progression. He is depressed too, but thankfully he hasn't taken that route for depression.
He had sexual reassignment surgery. The side effects of this butchery are painful and difficult to live with. I think it's a bit more complex than depression. Read up on these surgeries and the lifelong physical suffering they experience, you might then understand why someone would want to self delete.
Some people have such severe and persistent depression that doesn't respond to medication or any of the other treatments available. They should try psilocybin though, because that seems to help in some of those cases but is still taboo. But it's cruel to not help someone end it when they've been living in hell for decades without a light at the end of the tunnel. It's very sad but these people clearly have something wrong with them that can't be fixed at this point.
Life is a gift. Don't throw it away. :(

People really need to THINK before they run the risk of creating a child. Because for many of us, life is a carelessly-bestowed and unwanted and non-returnable "gift" for which we never asked, but now are stuck with. Most countries don't offer euthanasia, you know?
Children are the most vulnerable group in society. Regardless if you’re a man or a woman, puberty is essential for adulthood. Not just sexual reproduction, but vertical growth, bone density, lung capacity, brain chemistry, organ growth, etc.
Let’s talk about hormone consumption from teen to adulthood. There are no studies showing long term consequences of hormone therapy when taken by the opposite sex.
A medical ethicist suggesting that the patient bears co-responsibility for his mis-treatment? Who are the professionals in this situation? "Affirmative care" which is what overturned Patrick's 1999 assessment, is an abandonment of the admittedly difficult work of critical professional judgment. Regrets are NOT rare because I have seen many videos where it shows just the opposite.
In the so-called old school transitioning regrets were indeed rare and those numbers are often used in wrong context.
Transition used to take years, involve psychotherapy and thorough clinical evaluation -> decent medical process with adults, less regret.

In Europe these cases are generally rare since the screening takes 1 year in most countries. Not anything like the US. We do not do gender affirming care here (yet). Also sex operations are getting more rare. Gender psychology has come quite a long way. As long as we keep the activists out of it. BPD should be a HUGE red flag with the identity problems and suicidality (not due to gender dysphoria) especially in young females, and there should be attention for paraphilia's as autogynephilia in grown men, which can be addressed by occasionally wearing women clothes. I do no understand how the doctor missed this.
Europe? Which country do you mean? Here in Spain we have this affirming care in most autonomies of the country. Schools are using the chosen name of the child without informing the parents….

Patrick is severely mentally ill and already had these issues before he transitioned. He now has pain and discomfort in his pelvic area due to the vaginoplasty surgery that they can't fix/he's not willing to risk it because it could get even worse. I honestly think that he would still be severely mentally ill had he not gone through transition, but at least he wouldn't have had the medical damage/trauma and the psychiatric care he got for transition could have instead been used to work on his real issues.
Life is very hard sometimes, but with God's grace, there is hope and will to be joyful and happy. Seek and reach out to God, whatever happens. He is the answer to all our problems if only you knew... just be truthful and be patient ...

I wouldn’t let a kid of mine get a tattoo, much less take hormones or remove or add body parts. They’d need to have a fully developed healthy brain and body before making those kinds of decisions and that doesn’t happen until a person is in their early 20’s. Then, if he/she wanted to transition, I’d do everything in my power to help. And I have no issue with any kid living as another gender.
I hope you don't live in the U.S. or send your kids to public schools. There's a concerted effort by U.S.. schools and the healthcare system to provide what they euphemistically call "gender affirming care" for young people who claim to be transgender. (It's not clear to me what credentials or experience public school counselors and other school staffers have that qualifies them to diagnose a teen or young 20s student who is unhappy with her life/body as "transgender" and not, say, troubled by other issues that have nothing to do with transgenderism.)
For example, in my state, there is an effort to withhold from parents that their minor female children in school are binding their breasts or involved in other potentially unhealthy practices, in the name of "gender affirmation". (In my state, a public school staff member recently wanted to know if parents had to be notified that their "transgender" daughter was binding her breasts _if_ that daughter fainted in PE or music class.) In other states, teens, even minor teens, can receive services that will permit them to start taking hormones--or hormone blockers--without parental consent.

Have psychiatrists and related professionals thrown in the towel on helping depressed/traumatised/deluded people or are they just being denied access/the language to discuss these problems? I have Dissociative Identity Disorder. Fortunately I have control over my 6 alters and few are even aware I have it. However I know when someone is out. "I" is the sentry, always present, but not my core being. My core being, or the 'original' has at times been absent. Once for nearly two years. It's my observation that some transitioners might suffer from complete dissociation of an undiagnosed DID alter. As far as children go, I'm fully against surgery! You know why! Kids don't know if they are gay until puberty!
