- How to apply for the extended form in NYC | In this video we will show you, step-by-step, how to apply the extended Marriage Certificate here in New York City. Getting married in New York, it couldn't be easier.
PLEASE NOTE! Due to COVID-19, some of the rules have changed. You DO need to make an appointment now with the City Clerk's Office to pick up an extended form. You cannot just walk in anymore.

When you go in person to the Clerk Office you get the Extended Marriage Certificate on the same day or they mail it to your house
You can apply for and take it with you the same day.

Regan, you would need the "extended form" at the very latest when you want to change your name in your passport later. The passport department will ask you for that extended certificate. You can get the form either during your stay here in NY (fastest) or you can apply for it later through the mail when back at home in the UK. That way it will take about 6-8 weeks to receive. Why would you need a extended marriage certificate
Nathalie, good question! So you need an extended certificate if you are from another country than the US. So if you are from Europe, the UK and Australia for example. Not every country requires it. So it is always a good idea to check with your local Marriage Bureau what the requirements are in your area. Not all countries ask for the extended certificate but most do. Those countries also require the extended certificate if they want to change their names in their passports. The passport agency often asks for the extended certificate. Hope that helps.
