    Kegel Exercises Technique for Prostatectomy (Beginners)
    The correct Kegels technique to use for pelvic floor muscle strength and endurance for radical prostatectomy:
    *Shorten or retract the penis
    *Contract and tighten as if stopping the flow of urine
    Best Position for Kegel Exercises
    The best position for kegel exercises for prostatectomy is standing (1). This position is the most effective position for pelvic floor muscle strengthening because the pelvic floor muscles work hard against the downward force of gravity. Standing is the position where most men experience bladder incontinence after radical prostatectomy. Standing Kegels optimize pelvic floor muscle strengthening and restore bladder control.
    Long Kegel Exercises
    Long contraction Kegel exercises train the slow twitch (endurance) muscle fibers in the pelvic floor. These long sustained Kegel exercises train the pelvic floor muscles for lasting longer and for bladder control during bladder filling.
    Aim to perform 10 long (10 second contractions) 3 times daily.
    Short Kegel Exercises
    Short fast Kegels train the fast twitch pelvic floor muscle fibers for power for stress incontinence ie incontinence coughing, lifting, sneezing, standing and/or walking.
    Aim to perform 10 fast (1 second contractions) 3 times daily.
    Preoperative Prostatectomy Kegel Exercises
    Kegel exercises start at least 5 weeks prior to radical prostatectomy. Prehab pelvic floor exercises strengthen the pelvic floor muscles so they are working optimally before radical prostatectomy. Postoperatively the pelvic floor muscles have improved strength and endurance.
    Postoperative Prostatectomy Kegel Exercises
    Kegel exercises are recommenced postop when the catheter is removed, usually 1 week after surgery. The same preoperative Kegel exercise training protocol is resumed. Postoperative Kegels are performed standing upright.
    Research: Pelvic floor Muscle Training in Radical Prostatectomy
    Milios et al (2019) implemented this high volume pelvic floor training protocol in radical prostatectomy patients. Researchers compared low volume (LV) pelvic floor exercises performed 3 times daily with high volume (HV) pelvic floor exercises performed 6 times daily. The LV group trained lying, sitting and standing, The HV group performed all exercises standing. Both groups commenced pelvic floor muscle training exercises 5 weeks prior to radical prostatectomy and resumed exercises postoperatively following catheter removal. Milios et al (2019) reported faster return to bladder continence and improved pelvic floor muscle function with HV training.

      I was never told by any urologist prior to my radical prostatectomy to do kegel exercises. Only after my catheter was removed, was I instructed to start them. Sounds like a silly question, but is this going to be an issue with my urinary incontinence, or ED recoveries? It has been 7 weeks post surgery for me as of this comment. Thank you 🙂

    I had Robotic Radical Prostatectomy here in Bangalore, India, about 2 months back,
    Thereafter I had lot of incontinence,
    Then as per your guidance, I started doing Kegel exercises and within 20 days, it’s gradually reducing day by day,

I am 9 days post-op from robotic radical prostatectomy and 2 days since my catheter was removed with no bladder control except for the faintest of indications this morning. Yes the exercises should help you regain bladder control over the next 12weeks - start gently, focus on correct technique then gradually increase strength if contractions standing upright if you can.       Hi Michelle, thank you for your interest in my health and the link. That confirmed I am doing the exercises correctly as I felt the downward bulge in the peri-anal region as I performed the Kegel exercise. It seems so counter-intuiative to feel a downward buldge as you try to lift the pelvic floor but it works.. A 1-hr incontinence session just prior to my surgery also confirmed I was doing the exercises correctly. In this first week of the exercises after catheter removal,, I am beginning to see the short-lived ability to stop/start he urine stream when I stand from lying down. Gaining full control and controlling the all day drip will be a work in progress but this is encouraging. Thanks again! Steve

    Hi Michelle,
    Following a procedure of the bladder for retention and dripping have now been advised to limit the intake of fluids and confine the passage of urine to every 2 hours.
    To grow accustomed to this unfamiliar process i started to use the techniques stated here and am pleased to state they've been indeed helpful.

    Hi Michelle, thanks for your videos. I had a radical prostatectomy 2 1/2 months ago. Prior to surgery I was advised to perform these exercises using the NHS Sqeezy app as a guide and reminder but it was never fully explained how. Luckily I found out about your videos and have been following your advice. Once I'd had the surgery and had the catheter removed I continued and am now almost dry and in control.

    Thanks so much for the Video's Michelle. I am 4 weeks after Catheter removal and am down to one pad a day after using 4 or 5 in the first days. It's great to just load your video 3 times a day and just do it rather that going solo. My Physio agve me instructions but the videos make it so much easier with yout motivation !     Thanks for doing these videos Michelle. I had RP on 10/6/2021 and I still leak around an ounce a day. I've been doing these exercises laying down 4 times a day, 7 days a week using only a gentle squeeze. I'm going to start doing these standing up with a harder squeeze to see if I can get over the last 1 ounce hump.

    Its about time I found this video thank you I am gong to have prostatectomy in the middle of July so I have 4-5 weeks to do this exercise. It thesame as my Doctors gave me to do before my surgery. I wish I have time to do it even only 2 times a day. Thanks again.

      ...All the best on your upcoming surgery. I am 9 days out post-op and know it was the right treatment as opposed to radiation. You will feel better knowing the cancer was removed from your body. Glad your health allows you to be a good candidate for the surgery. Hang in there with recovery. Surgery first opens up a great 2nd option of reduced radiation load, if any tumor cells happen to survive the surgery. On the other hand,, radiation first to irradiate the entire prostate would greatly complicate future surgery...

    I did these exercises 5 weeks before my surgery and I have continued the exercises for 4 weeks after my catheter was removed. I am completely incontinent. I never have the feeling to urinate! Have you heard of this occurring with other patiences?

I wanted to ask you a question about Reverse Kegels. you must've seen this question a lot but I was wondering : Do these help with overcoming PE (Premature Ejaculation) ? I've seen in a subreddit that doing them everyday while pleasing yourself would help in the course of a few months. Could this lead to potential problems?       @Kira great question! Reverse Kegels will only help in the case of the pelvic floor muscles being too tight. There is a lot of misunderstanding about reverse kegels. They actually relax the pelvic floor muscles which are responsible for male erection. So unless the pelvic floor muscles are tight/in spasm there is no benefit for PE.     After having a radical prostatectomy in 2019 and a few other related surgeries and many, many catheters, I tried to regain some urine control by performing the Kegal exercises. One thing I did notice, overworking the those Kegal muscles made the incontinence worse, even during the night.
    If you wish to try these exercises, I highly advise you to take your time with these and don't over work them or you could do permanent or possible irreversible damage. I know!

    I had surgery 4 years ago. Working with a physical therapist was very helpful and got me 95% of the way there. I still need to wear a light pad and have a small amount of leakage. I've had more that one urologist tell me that after a year, there's little benefit from doing Kegels and that I just needed to accept where I am. Do you agree with that assessment? Is it possible I'll ever be 100% dry? I continue to do daily Kegels thinking that I just need to avoid losing the gains I've made.

      @Paul Guyer I’m so glad to read of your improvement to date, well done indeed. Unfortunately I can’t make an assessment on your individual condition and the only person who can do this is your treating Physical Therapist who knows the pelvic floor training you’ve performed. I would also suggest the importance of functional training which is over-overlooked but can provide the icing on the cake so to speak. This involves training and using the pelvic floor muscles in the situations that stress your pelvic floor and cause the leakage eg before and during cough, sit to stand, etc. Perhaps it’s worth returning to your Physical Therapist and asking if there is any further progression possible for your training too.     Thanks again for your videos Michelle! My prostate was removed Jan 20th of this year. I started doing kegels last summer, before the surgery and with your help I'm now doing 2-3 sets daily. However, I leak urine each time I contract and each time I relax my muscles. Plus, I experience significant leakage throughout the day. What are your thoughts?

      @Lee Bryant Well done on your improvement to date. Are you able to access treatment with a pelvic floor physiotherapist who treats men post prostate surgery? I think this would be a helpful way to proceed to help you check on your technique and assist you with progress. I’m not sure which country you are writing from, however, there are many excellent physiotherapists who treat this field worldwide. Perhaps contact your specialists rooms for a recommendation or your national physiotherapy association for recommendations. Wishing you all the best 🙏

    I had a prostatectomy 8 days ago. The catheter came out today but i couldn't go for a pee. The hospital let me leave because of my constant dripping which means my bladder is emptying. Pre op i was regularly doing pelvic exercises.
    I've bought some pads but the dripping is non stop. I know it's early days & I expected some leakage but not like this. God knows how I'm going to sleep. I hope this plan can help me 🙏

Doctors do smear tests all the time and think nothing of it, try to breathe deeply and relax the way I've taught you and focus on doing that if you can as it helps alot.     I found the verbal-only invitation to start each excercise difficult to follow (I wasn't clear when I should be contracting and when not). One the second set of fast contractions, you used a hand-gesture to indicate when we should be contracting and when to relax. I think it would have been good to use this hand-gesture consistently through all the excercises, from the start. That aside, a very useful video. Thanks.

    On the second set of long Kegel exercises, at about 6:50 in the video, right after the FOURTH long kegel exercise, you say okay we’ve got TWO MORE to go. So, your second set of long Kegels is a total of SIX, not FIVE. I don’t mind; just thought you might want to know.

Jo Milios et al (2019) research found that men who included these exercises into preop routine had a faster return to strength and had significantly less bladder leakage so we now recommend these exercises are commenced at least 5-6 weeks prior to surgery.       @@michellephysio Hi Michelle, couldn't be more thankful, one, for your videos, and two, that you respond so professionally. . . My important follow up is I actually don't have surgery scheduled but I will be scheduled for an MRI fusion biopsy in the next weeks or so and I am just thinking can I work these into a daily exercise regimen even though no specific treatment - surgery/radiation/focal therapy - has been set. I just want to be as proactive as possible but I also don't want to overdo it. Be super great to hear from on this follow-up, with the big thanks and kindest regards.

Until I get the dx, plan, etc. after the upcoming biopsy I am just doing 10 reps long and10 reps quick standing once a day after my regular workouts that are typically 30 mins eliptical and 4 sets on some weight lifting machine. I am almost 60 and consider myself in very good shape, diet and exercise (plan the same diet and workout regimen in perpetuity : ), If the word comes in that treatment is needed, I will shift to the full pre-op regimen. I am so thankful for all this information, with the most kind regards!

      @Terence Gunner as you probably know, radiotherapy can affect the tissues in the pelvis. I’m not aware of research investigating exercises after radiotherapy however I don’t see that there’s any reason not to try Kegels to help regain bladder control and erectile function. Both in terms of improving flexibility and strength. If I can locate relevant research I’ll be sure to post. Wishing you all the best!🙏

    I also suggest doing squats, and squats holding weights in your hands. After prostate removal, I performed about a million kegels and remained incontinent. After two weeks of squats and other pelvic floor exercises, the water flow has finally turned off.

    I am 78 and has Robotic surgery 7 months ago. The incontinence continues unabated. I haven’t done enough exercises though and I continue to wear diapers.
    Any exercise will necessarily be messy what do you suggest - should I do the exercises naked or what else you suggest

    Hello, I've been doing Kegels on and off for years after prostrate surgery.
    I am thinking of doing the AUS operation but would rather avoid it.
    From your experience, can I be helped with the Kegels.
    My operation was about 10 years ago and I am in pretty good health.( 75 but exercise every day am still work. CPA)
    I would rather do serious Kegels than be operated on.
    Have you seen anybody like me recover?

      @Robert Jackson nice to hear from you. I can really only say that there are many factors that depend on the likelihood of success with Kegels. It seems they are most effective in men with mild to moderate bladder leakage. Obviously, Kegel technique needs to be correct using muscles at the front along with sufficient Kegel exercise daily dosage, preferably performed standing upright. It may be worth seeing a Pelvic Floor Physio for assistance if you’re not 100% sure on your technique too. Wishing you all the best🙏

    Who experienced blockage because of the prostata? And what happened after this? And are your kidney parameters going back to normal (BUN and creatinin)? I experienced a blockage and I was underwent to TURP before 3 months (May 2022). My BUN and creatinin-parameters worsened because of the blockage. Is somebody who experienced the same? Returned at you the BUN and creatinin value to normal? Or not more? Thank you for answering me. best greetings from Vienna.

    Some of your prostratectomy recovery videos say 10 second holds while another says 5 second hold. Which is best?

      @RobertWeil-yh8kq Start with what you can manage comfortably focusing on correct technique - then increase length/duration of holds to 10 seconds in standing 6 times per day. This is correct training dose that the most recent research findings have shown to be most effective pre and post prostatectomy for overcoming bladder leakage.     I try to void my bladder prior to workout, but find that I still leak with the relax phase . Is this common? I’m 9 days post op

      I have been experiencing the same thing (at 2 weeks post surgery,) something I just started trying was doing these exercises after urinating while uncovered at the toilet, it seems to work well and keeps me dryer which should help with irritation. Just a thought. 😊

    Can these be effective after radio therapy following radical surgery? Made full recovery after surgery but struggling after radio therapy.

      @Mike Hewett yes this would be the exercise protocol to follow. The research into pelvic floor exercises for men is favouring high volume workouts for recovery.     I had my robotic Radical Prostatectomy on 28.2.24.Still have urine incontinence and 3 diapers are using daily...In the midnight my normal urine is passing and throughout the day I have no desire to go to toilet and even go to toilet no urine passes.....Kegel exercises doing.....Please advise..
