Scurvy isn't a disease, it's a condition. The difference being the method of contracting. You get scurvy from not having enough vitamin C in your diet. A person with scurvy could sneeze in your mouth and you'd never be at risk of catching scurvy. You could develop scurvy on your own, but that would be completely unrelated. That all being said, I don't think Oda is making that distinction. In modern One Piece I doubt there will be any real world conditions either, same as the diseases.
Also another reason for using non deadly diseases is the fact that there could be friendly fire. Even uf he is immune, which I doubt it, his crewmates certaintly wouldn't be

Like Luffy, Blackbeard has a way of earning the trust and probably loyalty of his crew
It would be fitting that he's riddle with horrible diseases, but it's his fruit that enables him to survive. Maybe he gave himself a Mr Burns kind of scenario where a lot of diseases are trying to go through the door at the same time, so they cancel each other.
I can see the fight between him and Chopper turning out to be that he made himself patient zero for a particularly horrible disease at the cost of his own life and that puts chopper on a timer to figure out how to cure the "ultimate disease" that Q has spread to everyone on the battlefield, kinda calling back to the Ice Oni plague that Queen set off on Onigashima except this time there isnt a cure readily available
And it turns out that the only cure is that mushroom he gave Dr. Hiriluk or cherry blossoms, and Chopper has to find either of those, and it turns out that Van Augur's teleport fruit is the key to him getting it. So a fight goes down, and Usopp helps chopper get Van Augur to teleport them to Drum Island and back while fighting. somehow
There has to be limits to this fruit besides haki like "only 1 disease active at a time" or "the diseases are always random and can be beneficial or harmful. No way of knowing" or "user must also be infected with the disease in order to spread it" otherwise it can be crazy OP
Maybe there's a healing factor fruit for its literal counter. I remember an episode of X-men where Bishop came from the future came and threw a disease that wiped put mutants onto Wolverine. Wolverine then cured it with his mutation, the healing factor by creating antibodies so maybe.
I think it's gonna be limited to infectious diseases and have a time limit because of that. A full immune respone take about ten days, in One Piece it wil be faster depending on how strong you are. But still faster than irl. That would be my guess.

The topic of doctors reminds me of the Saki clan from Kingdom. Known as the most sadistic, torturous, twisted clan in a gang of thieves. Turns out, them doing all those things was a result of wanting to be able to save their friend whom they couldn’t because they were kids and didn’t know how. But they’re the best healers, or doctors, of their time now because of it. Once again, Kingdom deserves more and all the recognition.

So in order for this fruit to ENSURE cancer, is he would have to cause continuous nonstop damage/mutation to a person's cells until one has lasting traits. Most cancer cells die on their own or are removed. It's about developing traits that allow it to continue consuming nutrition and evading the immune system that matters. So Doc Q would have to engineer a virus that can alter a cell in such a way.
Doc Q could just give all the Pirate King contenders Buggy's "You'll die if you go to that island" disease and ensure none of them can ever reach Laughtale. Or give that to marines to make their home base impossible to access by marines. Heck give them "you'll die if you leave this island" disease and slaves will never escape.

It also needs to be considered that the spread of diseases can not really be controlled. Diseases make no difference between friend or foe. I'd argue that the showcase of the power was only effective in the first place since the Heart Pirates were explicitly trying to avoid the Blackbeard Pirates.
Even if it wasn't stated, i believe the reason Doc Q chose such a silly disease to attack the heart pirates is because he can't control the spread of it, and Blackbeard seemed to bank on his knowlege of Law's powers, which would include teleportation. Imagine if Doc Q gave them a "progressively stop breathing disease", and law shambled one sick crew onto Blackbeard's ship, he would have lost a lot of underlings!
My guess on the actually parameters of the sick sick fruit, is that it works like a mirror of the horm horm fruit, effectively gaining potentially infinite range via transmission, in exchange for a lack of control over said transmission and any cure spreading just as easily. So Ivanokov needs to hit you up close and personal, but then you are stuck with whatever he did to you(unless law's haki based counter would work for that but then you need enough strength and skill with haki for that) but Doc Q just needs to expose you to the disease and it will propogate. I bet whatever the Blackbeards did to breach the Heart Pirate's sub carried the disease, just inflicting disease by thinking about it would be too powerful. and not good for story reasons.
I’m now excited for Bonny to be Doc Qi’s opponent. A battle of attrition with imagination as sounds wacky and fun. Also the young next generation defeating the older previous generation has room to be thematically relevant.

The diseased can save himself from symptoms by using haki but keeping haki up all the time not to get killed by the disease is draining your live to a point that you will loose lots of life expectancy.
What if the condition to create and give a illness to someone was for the user to also be affected by that same sickness momentarily? Would explain why he can't go with deadly ones or choose annoying illness that he can endure on the back of his horse while his ennemies are going crazy.

So thousands of things can cause cancer. Merely healing from an injury can lead to cancerous growth. So in order for this fruit to ENSURE cancer, is he would have to cause continuous nonstop damage/mutation to a person's cells until one has lasting traits. Most cancer cells die on their own or are removed. It's about developing traits that allow it to continue consuming nutrition and evading the immune system that matters. So Doc Q would have to engineer a virus that can alter a cell in such a way.

It also needs to be considered that the spread of diseases can not really be controlled. Diseases make no difference between friend or foe. I'd argue that the showcase of the power was only effective in the first place since the Heart Pirates were explicitly trying to avoid the Blackbeard Pirates.
Scurvy is technically a disease. That’s shown in the story back when Johnny and Yosaku show up.

Even if it wasn't stated, i believe the reason Doc Q chose such a silly disease to attack the heart pirates is because he can't control the spread of it, and Blackbeard seemed to bank on his knowlege of Law's powers, which would include teleportation. Imagine if Doc Q gave them a "progressively stop breathing disease", and law shambled one sick crew onto Blackbeard's ship, he would have lost a lot of underlings!

My guess on the actually parameters of the sick sick fruit, is that it works like a mirror of the horm horm fruit, effectively gaining potentially infinite range via transmission, in exchange for a lack of control over said transmission and any cure spreading just as easily. So Ivanokov needs to hit you up close and personal, but then you are stuck with whatever he did to you(unless law's haki based counter would work for that but then you need enough strength and skill with haki for that) but Doc Q just needs to expose you to the disease and it will propogate.
I bet whatever the Blackbeards did to breach the Heart Pirate's sub carried the disease, just inflicting disease by thinking about it would be too powerful. and not good for story reasons.

Even when restricted to One Piece diseases its pretty scary considering white amber lead and sapphire scales would be possible. Sapphire Scales in particular would be similar to having your shadow removed by the shadow fruit.

When viola explained her powers for the first time i was utterly shocked at how far she can actually see. 4,000km radius mean if in the center she can literally see all of asia, russia, and a large portions of europe and south east asia. She could literally hang around in india and have her eye go to Moscow, Russia deep in the bunker Putin lives in and peer through his deepest darkest and most embarrasing thoughts and make it known to the world. She can travel 4,000km deep and reach the earth's f***ing core and tell people whats down there. The fact that she cant even see other islands or the red line other than dressrossa is a real eyeopener to how big the one piece planet actually is!
Awakening the fruit might enable the user to inflict long lasting diseases. Maybe thats what happened to rodger. The diseased can save himself from symptoms by using haki but keeping haki up all the time not to get killed by the disease is draining your live to a point that you will loose lots of life expectancy.
