The thing that I find fascinating about idioms is their backstory. How did they come about? Why do we use the idioms as we do? For instance, I saw someone explain in another video that the reason Americans call that compartment at the back of the car, a “trunk”, is because when cars were just coming along they would strap a truck to the back of the vehicle, and the word stuck when car manufacturers started including that space in the body of the car itself. That is just so interesting to me!😊

You said “how do you do “ is old-fashioned BUT isn’t it still used as a formal way to respond when we’re introduced to someone of a higher rank? I wish I could give the benefit of the doubt ..at least in American English I guess it’s quite commonly used.
People learn this expression at school and then use it in modern, informal situations, and it just sound weird. This is why I always recommend learning more common expressions. Is "How do you do" used as a formal way to respond when we’re introduced to someone of a higher rank? Pfff.... I can't remember being in that situation, to be honest. I certainly would never use it when being introduced to a boss, or the CEO of a company I worked for. I'm not saying nobody ever uses it, but there are a ton of more common options we use.
