Do you think whether it's a good idea to approach them before getting an interview? I mean would that increase the chance of getting an interview?
Not necessarily, no. They may say you can contact them if you have any questions about the post itself, but it wouldn't necessarily sway their decision.

Since this video has been made, the job seeking in the NHS has changed dramatically. After over 300 applications, I have got only one single interview in a period of 10 months, and that one even did not go through, despite the fact that I have a good post-graduation experience (4 years) in various fields of medicine. I follow the Person specification as closely as I can, but they have started adding things like 6-12 months NHS experience as well as MRCP Part 1 as essential criteria. Honestly it has been so frustrating that I am even thinking of giving up on the whole idea of working in the UK...

Many IMGs are still able to land jobs within 2-4 months of applying. It all comes down to how well you present yourself and your achievements.

Yes but depending on what UKVI has currently mandated, you may need to return home to apply for your work visa and then return to the UK.
