The exam you are referring to is known as the MSRA- and it's an exam IMGs have been taking for quite some time to enter GP, radiology, pediatric, etc training. Nothing to worry about!

Or can I directly enter into a training post in NHS?

Does that mean I can apply for that if I have only internship experience? Or is that a registrat level post?

It's up to you.

The first instance is correct in your situation. Without post internship experience, you wouldn't get your CREST signed.
