Most salient point was" can't speed up healing" . To me that means accepting the pain, swelling and immobility; But, push and hook-up with a good PT. Also, rely on you PT. Most Surgeons feel their job is done when the Resident or PA close the wound.
Hello. I'm up for bilateral knee replacements in the spring. My right leg has hardware and a deformity cuz the ligaments have loosened on the inside so bad. The left one is "kinda normal", but both are bone on bone with severe contractures BEFORE surgery. I'm only 43 and scared but ready. ❤😢😊

I had my TKR surgery 13 weeks ago. I have just now seen this video. I would like to tell everyone who sees it, listen to the man and follow his instructions he's absolutely right on every point. I think he should have started with the instruction DO NOT compare your progress with someone elses progress etc. You are you, your progress will be unique to you. I healed and progressed rapidly but I also did exercises in the pool for 2-3hours 2-3 times a week in the year before surgery and I followed all the instructions and exercises 3-4times a day everyday after surgery for about 6weeks. Then I started walking. Each time I went out walking I set a further distance goal. Doing 7kms now. Still doing the original exercises everyday as well. Got kicked out of physio class for being too far advanced after only 2sessions. So how you progress is up to you. Slow and steady, and just a little bit more each time is my motto. 😂 Enjoy the process, the changes happen remarkably quickly once you get going.😊

Hi Dr Mike, You're welcome. I should have added that I was no twiggy the year before surgery I was 109kg so I was working to lose excess body mass as well as trying to strengthen my arthritic knee. So in part my excellent recovery was aided by both successes. Today I weigh 99kg. I'm not a strong swimmer, so most of my pool exercises were like old fashioned calisthenics at my speed. I built up from 10reps to 100reps of each exercise which is why it took 2-3hours, but I was going at my own pace and no one was or had ever told me what to do. It was my Choice and I enjoyed it . If I wasn't enjoying it I did one of two things...keep going because it was good for me but drop the reps to 50, or occasionally I quit went for a soak in the spa pool for 5-10mins then went back to the exercises with a new mindset. The major workout is not in the exercises for the body ... It in the exercise of the mind. Kind firm Self talk is crucial, you have to become your own cheerleading squad. A cheerleading squad of One can achieve just as much if not more than a huge team. ❤ To anyone who reads this and says oh but you don't know the troubles that I have.. I'd say this.... No I don't but you don't know mine either and I've had a lot of serious ones, major depression for 30years being just one of them. I've overcome that been free of it for two years but I could fall again at anytime so I've been told. Maybe, but not if I can help it. I'm going to fight for my good health everyday until I die. My 66th birthday is in a couple of weeks and I'm not going to give up and die a sick weak fat person. I intend to be slim and healthy when I die. You can be too if you just let yourself. Fight, it's a good fight and you'll feel great as you do a little more each day. Nobody needs to run a marathon to heal slow and steady gets you there with less chance of injury. Let me ask you.... Why does everyone have to rush.? Where are you hurrying to? You can grow strong without lifting weights ...just using your own body as a weight, likewise you can strengthen your self control by making small changes to your thinking, so it follows that when you have an injury or a surgery small steady progressive changes every couple of days will improve your recovery far quicker than rushing to it and possibly setting it back by weeks or worse. Hi Dr. Mike! I have severe ( bone- on- bone) osteoarthritis in both knees, but worse on the left knee. I'm 68 years old & played pickleball for years, even when I started feeling pain in my left knee. Now the pain is really intense. Cortisone shots only lasted 9 or 10 days. Gel shots haven't even kicked in & it's been almost 8 weeks. I'm considering getting knee replacements. With your clients who have osteoarthritis in both knees, was it more difficult for them to recover? * We live on the top floor of a two- flat in Chicago. 😩

Sounds like you’re a very active person and I love to hear people finding new ways like pickleball to keep their exercise enjoyable! I’ve heard a lot of good things with the growth of the sport! Anyways many of my clients have dealt with similar experiences and came to a point where all conservative care was not providing enough relief. Surgery allowed them to get back to doing the things they loved again. And for those with osteoarthritis in both knees they usually get the most painful side done first and then later get the other one. By no means am I saying that it’s going to be a walk in the park. It will take 3-6 months to start feeling somewhat normal again and sometimes some people take up to 1 year. It’s part of the process but in the long run it could provide you with many more years of an active lifestyle without any pain or limitations! Hope that helps and I’m wishing you the best!
I’m three months past complete TKR in the left knee and two weeks into right knee TKR. Dr. Mike is absolutely correct in what he says. I want to add my two cents about what I’ve learned, which I think follows Dr. Mike. 1. Have someone you trust keep track of when and how much medication you take, because your mind will be foggy and your short term memory will be unreliable for awhile. My husband put together a spreadsheet on the computer. Keep ahead of the pain with regular meds. This is not a time to think you can survive pain better than other folks. 2. If you can’t sleep at all at night, rent an icing machine that can be pre-programmed to cycle through an on/off icing program. Or just at least get a gel cooling bag or bag of frozen peas on your leg. Remember to elevate your leg too. I think inflammation and pain explode at night, so talk to your doctor about added pain medications. Don’t push your leg with exercises too hard before evening and overnight. Timing of exercise makes a difference. I know from experience that it’s almost unbelievably excruciating at times, but not all the time, so remind yourself it will get better. Listen to music or very quiet level voices- no politics. 3. Exercise, but you don’t need to torture yourself, either. Keep experimenting to find out how much pain with exercise is reasonable, and get advice from your PT. With my first TKR, I had so much fear that I wasn’t pushing the ROM and quad exercises enough, that I overdid it and set my progress back. After I learned how to balance pain and exercise better, I started healing ahead of schedule without superhuman effort. The body needs to heal itself and your pain will be made worse by torturous exercise. 4. Remember you’re not alone!! Ask for pep talks or motivation from experts. My PT helped a lot with morale. It’s OK to cry, too. I hope this might help someone suffer less!

Had TKR on my right knee in 2020, in the beginning of Covid. The therapist would come to the house but sent me a exercise schedule. Would have been better if he helped in person. Today? Zero pain. No limp. 80% bending, I can live with that. Now the left knee is acting up, I’m looking to get gel injections first, hold off on surgery for as long as possible. Those with pain and suffering, I hope you all get better soon.

Here I am, 9 days post surgery, and I have learned more about recovering from it from you, than from any of the "teachings" I was given prior to discharge - which happened less than 24 hours after my surgery was completed. A quick update...I am just over 9 + weeks out now, and continue to be happy with my new knee... so much so that I am booked to have my 2nd knee done on February 29th...I am hopeful that everything I have learned from the first surgery will help me be fully prepared for the 2nd one, and that my recovery from that one will be just as smooth and equally speedy as the first time around...

The best advice I can give you is get your own bandages -they gave me ones you have to use tape with -I found big bandage ones that are tape included from CVS or Walgreens I got the 6x6 and used two just cut part of the end tape off one & put them together so much easier

I’ve had two Physio appointments already and I was 85 last Monday and 90 on Wednesday. It’s hard to bend from playing down position however I’ve been doing chair sitting up and down and letting my leg hang with gravity for 30 seconds and feel that bending like this is good, just not in laying position which I was beating myself up about. I’m going to take your advice and continue to walk with walker until I’m strong enough. You truly uplifted my spirits. I’m so worried about the time allotted for full motion. I heard one person mention it’s within two weeks and if you don’t get full bend and straight that ship has sailed. It scared me. I will continue doing what I can daily at home. Thank you.
Never compare! I did this, too. I'm 2 weeks post-op and have pushed myself to bend my knee to 119 degrees. My PT really had a heart to heart with me. He said, "That's great! But let's slow down." I am expecting far too much too soon. I've had to reevaluate my expectations and accept them. I've now slowed down, and I'm trying to compare myself to no one! Just go your pace, be diligent, and do what your PT tells you.
I played several sports and have had 19 major surgeries as a result. I am 63 years old and had a TKR 1 year ago. For some reason it is a competition with some people. I swear some these people will tell you they didn't need anesthesia. I see that with some of these comments. What this Dr. is telling you is spot on. For example, his comments regarding the use of a walker are right on the money. The PT does not want you to stop using the walker till you can walk normally WITH it and then the same for a cane. A friend of mine told me she stopped using the walker in the less than a week. I told my Dr. at the two week post-op visit this because I thought I shouldn't be using a walker still. He laughed and said, first she didn't have a total knee replacement, second if she did she is crawling around her kitchen or third she is full of it. So this is a quote from my Dr. he said: "The first two weeks after surgery you will hate me, weeks 3-6 you will hate me less, by the third month mark you will begin to see why you had it done." He was exactly right and it is the best my knee has felt in 8 years.

Many of my patients experience the same situation where they regret having the surgery early on but after weeks go by begin to realize it was a good decision in the long run!! First two weeks were absolute hell. At 14 days it began to subside. I’m questioning my life decisions right now 😂. At some point I’ll have to have the other knee done 😢

However, at 3 months it really did turn and 1 plus years later my knee is better than it has been in 8 years (I needed a knee in my late fifties and made it to 62). Hang in there and don't let anyone tell you how you should feel. The Dr. and the PT are the voices you want to listen to. Take care!

I’m two weeks post op TKR and already at 125 degrees on my knee bend. It will also lay flat. It’s still tight but working it out. I’ve always done more than the expected me too in PT from day one. I don’t go for long walks but I do get up at least every hour and walk, even outside weather permitting. I have a bicycle that sets in the floor and I use it before and after all my PT. Started walking with a Cain at 9 days post op. My swelling is very minimal now. I still ice a lot and work the stiffness out afterwards. I have no more pain or what little I have had is less that before surgery when I was bone on bone. Stay strong people and good luck.

And Just what exactly is your age? There are different expectations based on the age of the patient as well as the difference in Every TKR surgery. Some patients will recover faster, some slower. Go at your OWN pace!
Doing both of mine 4/22/24. I'm learning all I can. I'm pretty damn nervous at this point. I just hope to get all I can from this surgery. I've been suffering for 7 years probably. I think I would be in a wheelchair in a year. I'm 56 and my quality of life is nearly 0. Well, maybe not that bad but it's bad.
I am 3 weeks into my knee replacement surgery and everybody needs to listen to the words that you just said because it is the honest to goodness truth and if you do not exercise you know do all the stretches your knee will swell up and it the more you have swelling the harder it is to get out and do those exercises
I am just over three weeks after my second knee replacement. Listen to this man, his advice is gold. Do not baby yourself or neglect your rehab exercises. I also highly recommend you rent or buy an icing machine. I purchased one before my first TKR, knowing I would be doing the other knee. It was cheaper to buy it than rent it twice. Do not skimp on this to save a few dollars. Your knee will thank you.

Absolutely! Mine done 7 years ago. One day at a time and pace yourself as over doing it will increase swelling and you, not the over doer’s will feel the pain! Ask me how I know???!!! Best of luck as you recover.
I am 6 weeks out, from my TKR. I think everyone should listen to this video. I am very fortunate that my therapist told me every one of these except keeping my knee straight. I already knew to keep my knee straight from the ladies in my swim class. I worked on keeping my knee straight as soon as I got home from the hospital on day 2. The walker I used for 2.5 weeks because it hurt my shoulders, and arm. I used 2 canes, and my therapist was very impressed of how straight I was able to walk.

@@kimparigoris1472 It is similar for me. I spend most of my time in the recliner except when doing my PT. I can only sleep for about 1 1/2 hr at a time so far.
I had my right knee surgery 2 years ago. My approach was not to add stress to myself and allowed it to recover properly. Up to this stage I still could not understand the need of doing physio 3 days after surgery, when my knee strength had not revived. I attended regular physio but ignored doing it on my own at home. This approach helped me personally because my knee did not develop swelling. Just be gentle to the knee and to myself. I walked slowly using u-frame as it helped regaining my knee strength gradually. I slept by keeping my knee straight upward with supporting round pillow behind my knee underneath. My recovery journey was not a breeze but I was glad that be gentle to myself lead to excellent total revovery after 1 year. Dont push yourselves people. You will hurt your knee if u fall. If u need to use a wheel-chair, please do. As I did, especially to shower et'al. Ignore people who tried to push your limit by dictating the dos and donts. U are your own doctor during your recovery period.
This is great information to give to people. I learned all of this from my at-home PT the first 2 weeks after both my knee surgeries. I have fibromyalgia and reactive arthritis, so the nerve block only lasted 8 hours and then I had to have an upgrade in pain meds and my at home PT taught me how to lessen the pain with certain movements. Being used to PT, I followed what she said as much as I could and told her where my weaknesses were so she could teach me how to mitigate those. She had me at almost full range by the end of her stint and my outpatient PT got me the rest of the way. Listen to your PT people, tell them your struggles, and follow what they say - it's the best way to advance at a normal to slightly better rate which will help you in the long run. I didn't do the exercises as much as I was supposed to because of my chronic issues but when I did them, I was faithful. I did a lot more exercises as I was stationary which kept everything limber.

Best thing I ever done! I had my left knee done almost 2 years ago. I couldn’t sleep for a couple of months, I had stiffness, tightness, but I did the right thing it will get better. Getting ready to have my right knee done in July.
First don't compare yourself to others. The longer you go without bending the harder it will be. I had my tka 10 weeks ago. I agree the tightness, numbing on out side of knee and burning stinging on inside of knee is awful . It does get better. Remodeling takes a long time. Lots of structural changes b4 surgery and now there are more structural changed. Getting the knee alignment back to normal. Don't give up keep up with your exercises. My new knee is now starting to feel like it belongs to me. Your body is saying what is this foreign object. You will slowly begin to accommodate to the new knee.
Hi, all. I had TKR 9 days ago. 3rd p/t was today. 106° bend. First p/t was 50°. Walking with just a cane but I can walk without it. I think I'm progressing OK. No problem at all sleeping. In fact, I'm sleeping better than I have in 3 yrs. I'm 75 and everyone tells me I'm doing great. Going to attempt riding my recumbent bike next week. Had my knee replacement 3 wks ago. I can stretch out the leg but bending is another thing. I'm probably around 80 degrees at the moment..there's so many videos on here I'm not sure if I'm coming or going.people saying their walking after a wk and total bending of the knee..I'll do it at my pace.exercise as i should but I'm not going to put myself through hell..it's bad enough as it is..swollen,tight numb electric pins and needles feeling..oh and no sleep..wish I'd never had this done 😢.. 5 months later and still numb on outer side of knee.nerve pain up the total of my leg along with hip pain still.still not able to sleep on my side due to the pain..walking is still on the slow side..as for knee bending I still can't fully bend it..wasn't this bad before the the operation so definitely not having the other done..no chance. Just a quick question thrown out there..anyone else suffering from total leg ache..my knee replacement leg is aching 24/7..even when I try to sleep I can feel it aching and going numb

I had my right knee done February 20,23, and my left knee done October 9. My recoveries were night and day different. I will say, NO regrets about doing both in the same year. First time was tough because I had been so sedentary due to constant severe pain and restricted mobility for so long. PT and my physical therapist were life savers! Encouraging and great directions for self care and recovery strategies. Second time I was much more quickly able to bend etc, but holy crow! My legs are SORE. My PT pointed out that my gait is completely different than pre first surgery. My legs are straight and I no longer limp/ waddle due to the pain. Dr Mike great advice in this video!

Had my left knee replacement 1.5 weeks ago. Right knee was replaced May 2021. Conformis replacement. I'm 68 now. Still working. New construction plumbing. First knee is awesome. I still play senior and league softball. Have played probably 60 games on right one. So far I've reached 123 degrees on bending, still about 2 degrees on straightening. Therapist been coming to me last 1.5 weeks 3 daysxa week. Start going out next week for theraphy. Be off work 3 months because of work that i do. Climbing ladders, bending knees constantly. Zero meds for last 3 days for pain. Do all my exercises 3 times a day. Ride a arydyne bike for 15 minutes after exercises. Then ice. Can't stop or you'll go backwards. Did that on first surgery. Lost 11 degrees in less than 2 days. Have like 130 on it now. Be safe everyone and good luck.

Don’t push it to hard, baby steps only. I pushed to hard and think I caused more scare tissue.. sore still especially on the outside of my Rt knee. Constant swelling. It truly sucks..
Listen to this therapist! Mine replaced 7 years ago, therapy is hard and he knows what he is talking about! He mentions a wedge, Amazon has a wedge pillow that is awesome! Skip the pillows, get the wedge! Millard leg elevation wedge and another thing that helped me was the leg glide. Good luck!!

Dr. Mike, good stuff. To me, key is regular therapy at home or with therapist. Honestly, I would have never gotten to 115degrees after 6 weeks within my therapist. They push you which is needed at this painful time. At home, most of us are to wimpy to push us hard. Good luck.

Had surgery 6 weeks ago, doing ok. I think this might be the best video I’ve seen. Overdid the walking way too early and paying for. I have never gotten a consistent handle on the pain. Again great video and will be better prepared for my second knee.

I have had two knee replacements, my surgeon did not start me on PT until two weeks after my surgery. I did at home exercises and after two weeks I was ready for my PT, which went really really well.

I’m 3 weeks Post Surgery, and doing pretty well. I wasn’t really keen on going out a lot in public with my walker, but about 5 days post surgery, I was frustrated by the nice sunny days, and told my wife let’s head to the store. I found that pushing a shopping cart was a great clandestine substitute for my walker, and was able to put the time in working on my gait. I’ve been hitting the stores about 3 times a week for this purpose. I’m just about right there with ROM, but still have a ways to go to relieve the tension around the joint.
Very good advice… when I had my knee replaced I had to do therapy twice a day for twelve weeks… a lot of work but worth it… I have no pain and no limitations…. I had to work real hard to get that zero extension…. My knee got better and better even after 1 1/2 years later… I still have to keep my muscles strong by exercising. Please always follow your Physical Therapist instructions….

Hi, I started walking in the house without the walker at the end of my first week. I noticed that I was not using the leg right. There was no bend in it. I didn't like the limp, so I grabbed the walker again, and came to RUclips. Thank you for this post I was doing things wrong. From now on I'm going to do things better. It doesn't make any sense to experience this much pain and not try my best to never have to do this again
I’m 3 weeks in and I’m glad I turned in to your video not my therapist isn’t doing a good job with me. I learned a few things that I didn’t know concerning a walker which I went 7 days without my walker because the delivery for whatever reason never got it to me. Then when I finally got it I really didn’t need it

I’m 4 weeks PO today. ROM is “only 1-80” but 4 days prior, my numbers were “only 2-75”. The PA-C in my surgeon’s office was alarmed when I said 80! Her response, “OK. You can catch up though. You’ll need to work very hard, doing all your exercises every day. Do you think you can do that?”’ Ummm. Yeah, you mean like I HAVE BEEN DOING all along? Yeah, I can KEEP doing that! It was so discouraging. Always nice to see the reminders that our bodies all respond and heal differently. A week ago, I said, “Ive never worked to hard for so little in return.” I guess I’m learning patience with myself if nothing else.

Great video! I had both hips replaced 2yrs ago w/no one for support & now my left knee is scheduled in Nov/RT knee in Feb. I was told knees are more difficult than hips 😦😳 So I'm going to call in everyone I know for assistance this time. 😊

This was absolutely the best advice I've read regarding what to avoid after knee replacement. These advices or admonitions are not available to read about in Sweden (in Swedish). 17 days ago I had surgery and have been doing great, now I'm cutting back on my pain relievers and the anti-inflammatory drugs but yesterday my walk with two crutches was too far. I am meticulous and follow my training program daily and I have also walked daily and increased from 2x400 meters to 2x3.5 km daily with crutches at a fast pace. yesterday I suffered a setback which means that I now have severe pain in my knee and can hardly move forward. I understand now that I have to calm down. Thanks for the good advice

I’m 10 days out from bilateral knee replacements - I’m guilty of walking with out my crutches around the house and I have noticed a limo so today have made sure I use my crutches even in the house Thank you
All good advice. I was fortunate to have an excellent physical therapist who helped me stay on track. That is a very important part of recovery. Perhaps the most important thing is to have an advocate to keep track of everything. I thought that I could easily do this and I discovered that my memory was nowhere near as good as I thought it was. Thankfully my lovely bride was keeping me in line.

I got my surgery on Monday. Today is Saturday and I’ve done a lot of things wrong, but I couldn’t help some of the things. My caregivers were in Northwestern, North Carolina and couldn’t get to me because of the flooding and then my husband came down with a really bad back and leg and he couldn’t help that just said everything back For me I’ve been in a lot of pain today. I haven’t wanted to take the oxycodone because it was making me crazy but I had to take it today because I think I overdid my physical therapy yesterday and I was as well as not sleeping last night or the night before, but I did take a nice nap, this is brutal but I do appreciate his tips and I will continue using my walker and I’m glad I heard this because I wasn’t using it as much as I did a day or so ago God bless all of you!

Excellent Advice!! I’m SO glad I found your channel! Just had left knee replaced today - all out patient - no pain pump- just oxy and Tylenol. I’m ok today but know the pain will start tomorrow as the surgical anesthesia wears off. I was elevating with pillows under my knee/ will change that to a wedge - I fear I’m the type to overdo and try to rush the healing process with too much walking- too soon. I’ll slow down and focus on good form.

Great reminders. LTKR on 30/12/23 . I had a 15% valgus that is reduced down to 6%. It’s been a shock how impactful this is on my pro-perception. I’ve walked with a pronounced limp for a decade. I’m being mindful of how I manage my recovery as I want to be as balanced and smooth as I can be . Have been doing prehab for a full year in preparation for this.!feels like I’m learning to walk again.! Have good ROM. Thanks Dr Mike for your informative videos .

I labeled alarms on each medication to take at what time every day and night. That was a real save because you need take the meds regularly and then know when to take them as you're tapering off pain meds. Had 2 knee replacements 7 months apart. Worked both times. My sister came to help assist me and coach me too.
I really got tripped up in comparisons. When u heard some people got both knees at once and i was struggling so much with one, I felt so badly about myself. But all i could do is my best. Poco a poco, little by little, and I've made progress.

Doing all the exercise and PT I'm supposed to 2 weeks after surgery. Pain is absolutely unbearable even with pain pills. Look if I get 2 hours of sleep at night. Hope things get better. Could use your prayers 🙏
I had both replaced five weeks apart. The biggest issue was sleeping. 1-2 hours then 3-4 hours awake. My legs became very ache and painful at night. This lasted a solid 8 weeks before the sleeping improved. Pain medication didn't seem do much, and I was taking as directed. Glad it's all behind me.
My mrs had surgery right leg on 13/03/24 and left leg on 24/03/24 . Now she is feel better and doing her work. Thank you dr for your information and advice. Thanks lot

I am almost 6 months post op. There is a lot of information out there whether by other surgery patients or by medical people. I must say that I wish I had seen your video before surgery as I think it would have made my immediate post op recovery better. Several of your points were things I should have done or not done at the time. Your presentation is clear and supported by your knowledge. I can see where I went wrong when I had a bit of a set back for about a week. Things finally progressed and I can walk easily and have gotten my life back! It all just takes time and we need to be patient. Thanks for putting this information out there to help others as they head into surgery.

I had TKR (Osseti cementless) 12 days ago. After a week the doctors office told me to do three 10 minute walks a day in addition to my twice daily exercises and 5-10 minutes of stationary bike. I’m fairly young (40) and know my body well from years of consistent hard training. I did three 10 minute walks the first day it was recommended and knew immediately after I needed to pull back. Took the next day much slower and have been building from there. Man, this sh*t is painful though. I was only given a week’s worth of narcotics and now I wake up in the middle of the night in extreme pain.

I had both knees replaced. He gave good advice. Bending exercises are important. My doctor ordered a machine I strapped my leg to and it bent my knee. Every few days. PT upped the bend degree and reps. That was quite helpful.

After having both knees replaced in the last 15 months I totally agree with everything Dr Mike stated. I made mistake #6, but compared my progress with left knee to the quicker and less painful recovery and rehab of the right knee. Don’t do that. It was frustrating and depressing. Now, 4 months post 2nd knee surgery, they are pretty similar in strength and mobility,etc. I cannot stress Physical Therapy enough, it makes a world of difference in your recovery. Baby steps are still progress! ❤
I'm on my 9th day of TKR on my left knee. I have the best home support. My wife is great charting my meds with I stopped taking pain meds after 3 days. My in home PT is so good and the pre-opt and surgery team i had at KP were the greatest. Dr. Mike this video is great. Do the stretching exercises and use that walker. My post-op is in 7 days and I plan on walking with a cane for that visit.
I appreciate your clear and concise video. I had TKR on Marc 22, 2024 and today is April 11, 2024. I totally understand what you mean about staying with the walker to have a balanced gait rather than having a limp pattern start to set in. Thank you for taking the time to make this video.

Thanks for the excellent video. I am 2 weeks post surgery and doing well. I would suggest a 7th mistake is not enough icing. I applied a lot of ice treatment during waking hours on the first 3 days followed by ice as needed and post exercise. I found the trick to recovery is to reduce swelling with ice but also wean to less ice treatment over time. Ice reduces swelling and no ice promotes healing. It is a fine balance that is individual and varied.

That’s a major issue! I’m sick of all those people bragging about taking only Tylenol after a few days, and being back on the golf course in a couple weeks. Are they crazy, delusional or had a different surgery!?!
@@user-wk1mw9nj3i76 I had a relative who I have not seen in a long time that told me he never took the first pain pill after he got home. He said it was sore but no pain. I'm sorry, but I find that difficult to believe. He didn't even know what I meant when I told him I had TKR. I think he must have had something else done to the knee. Maybe a meniscus repair. I have never heard anyone say they did not have pain. For me the first 2 weeks were brutal.

I am going through a bit of depression, mainly due to the inability to do the things I did before, lack of appetite and the pain that drives me crazy. Many people feel down because they can’t do the things that they use to do now. But it’s all temporary! Two weeks out from TKR and so glad I came across this post. I’m making all the mistakes! Got rid of the walker too soon (vanity) and will bring it back into practice as my walking gait has deteriorated. Major stiffness in knee from working too hard trying to “fix this fast”. Need to do more on range of motion. I do have an excellent support system and just need to check my ego so I can make the most out of this surgery.

The pain after the knee replacement was excruciating for me. They were so chintzy with the pain medications, which made me angry. This is what pain meds are for, post-surgery. The meds they gave me didn’t touch the pain. If you’re unable to sleep due to pain, one thing I did was get a tens unit and put the leads on my knee/legs. This tens unit sensation distracted my brain from the acute pain so that I could fall asleep. The acute post-op pain lasted for exactly 30 days. After that, it was smooth sailing.

As a home physical therapist, I really enjoyed your video. However, after 46 years of practicing physical therapy and doing a lot of total knee rehab, I disagree with your third mistake. Too often I see my patients who were instructed to not put anything under their knee so that their muscles will not tighten up. This increases their pain dramatically. I always suggest that the patients support their knee and have about 10 to 15° of flexion. This way they're not having as much pain and it reduces the swelling as well. Another thing that I emphasize is more elevation. The less swelling in the knee the faster the range of motion will return. Overall, a nice presentation. By the way, I always emphasize doing some active knee extension exercises so the muscles do not contracture
@PaulRich-oy4xh hey thanks for stopping by to watch and for taking the time to share your expertise. It’s always nice to hear from fellow PTs and I respect your input. Let me clarify that when I instruct my patients to remove any support behind the knee I also have them do this gradually just in case they do experience sudden increase in pain. This might look like spending 3-5 minutes at a time initially and then building up to 20 minutes without support if they can tolerate it. I’ve seen too many times where people get comfortable with that 10-15 degree bend in their knee which leads to stiffness behind the knee limiting their ability to fully extend their knee. And I agree with you that use of the wedge with elevation can help with not only the pain but swelling control! These are great points that you bring up! Thank you for sharing!
I had both of mine done at the same time it’s been over a year. I finally feel like there’s some real progress. My work demands a lot on my feet and my lifestyle does also hang in there. I think it does finally feel better.

Great advice. I did everything that you went over and my knee is so much better now. Had my knee replacement surgery just over 14 weeks now. First two weeks were not bad. Second to sixth weeks were the worst. Two to three hours of severe pain every night. Was not able to sleep for three weeks even with the pain medication. During the day was fine. Don't know why the pain gets so bad at night. Finally off of the pain medication. Just pain when I first stand up. Just waiting for that to go away. If not, back to see the doctor. It will be some time before I can forget the pain. Will stick to corotisone for my other knee for now.

I'm 6 days out from total knee replacement. Luckily I had really good range of motion before the surgery (I'm physically active and bike a lot). But my leg extension was not good, and straightening has been painful. I can't believe how tight and painful the knee is every morning! Seems like it gets more painful by the day. One of the hardest things for me to deal with is being stuck on the bed or sofa indoors for weeks to come. Not looking forward to months of this pain and discomfort. I can see why people get depressed.😢

Consider progress a week and possibly a month at a time. I just had a revision, partial to total knee replacement. Nothing can compare you for this experience, it’s a lifetime of pain in a month. I was discharged 16 hours after my revision and the pain was horrendous. I should have had an epidural, continuous nerve block, PCA Dilaudid, or IV narcotics for the first two days. They need to a better job with pain management and stop looking at cutting costs by tossing people out the door.
I'm now 8 days after tkr surgery, agree 100 % with keeping up with the pain meds which will in turn allow you to complete your rehab so much easier. Will it hurt, you better believe it will. Personally I had pretty good range in the knee before leaving hospital at 3days post op. Around 80° the leg has tightened since then but physio not concerned, just keep exercises going and keep pushing it a little more every day. Personally I have over the top of the thigh to the point it feels like I'm tearing at stitches when I get past 80°. Will run that one past the physio on her next visit on Monday. Plus the IT band is very tight but that's just me. This was a very informative video, and basically you are going to get out what you put in.
Yes thank you the pins & needles pain is persistent after 3 months of surgery despite following 95%of what you said to do or not to do... How to overcome..i am praying & it will help & will continue with exercise & medicines for some more time ok 🙌
Thanks Dr Mike! I actually looked at one of your other videos that showed different exercises and this one popped up in my feed. I’m four weeks post surgery and am progressing good. I’m walking about 30 minutes twice a day and doing my range of motion exercises about three times a day. But my problem was putting the pillow under my knee…I was in so much pain that first week…I just had to. I am still guilty of it at night. But during the day I try to concentrate on laying the leg flat and putting a wedge under the ankle and tolerating it for about five minutes take it out and then repeat. But that is something that I am guilty of from day one…

See myself doing several of what the doctor mentioned. My physical therapist often asks me do I have a cane? I only have my Walker which I rely on. Now that i've seen this video I want to take my time but continue my journey. And it is giving me encouragement not to do some of the things that I have been doing. In order to get better and be able to walk without help.
I am quite capable of managing my pain relief, just as well as I don’t have much choice as I don’t have anyone who can help me. Not everyone does, and not having anyone to help should not discourage people from having necessary surgery.
I am sorry you had no support system. I live by myself but thankfully family and friends have been able to step in to fill in the gaps. It definitely made a difference having someone my first 10 days. I am now 14 days put.
Also if insurance allows, and no support system, sometimes inpatient rehab for the first week or two can be an option. I researched and was prepared for that, until my sisters surprised me and came in from out of town to help, since their schedules allowed

I’m 4 weeks out from TKR. Thank you for the advise about the walker. I try the cane for practice occasionally but I do feel I have a better gait with my walker for now Also I pushed too hard on the walking one day and did have a set back. So now I walk in smaller sessions but more consistently

Day 3 post TLKR yesterday and I walked 2300 steps over about 6 sessions. It was too much as last night needed breakthrough pain meds x 2 to get some rest.
I am 5 weeks RTK post-op and up walking with my Rollator. I am able to straighten my knee, however my flexion is about 65-70 degrees. Doing home exercises and out patient PT. The swelling is still prominent. Elevating and icing but swelling still preventing from gaining flexion. Frustrated. But am moving along slowly but surely. Thanks for the video.

Most salient point was" can't speed up healing" . To me that means accepting the pain, swelling and immobility; But, push and hook-up with a good PT. Also, rely on you PT. Most Surgeons feel their job is done when the Resident or PA close the wound.

Hello. I'm up for bilateral knee replacements in the spring. My right leg has hardware and a deformity cuz the ligaments have loosened on the inside so bad. The left one is "kinda normal", but both are bone on bone with severe contractures BEFORE surgery. I'm only 43 and scared but ready. ❤😢😊

I would like to tell everyone who sees it, listen to the man and follow his instructions he's absolutely right on every point. I think he should have started with the instruction DO NOT compare your progress with someone elses progress etc. You are you, your progress will be unique to you.
I healed and progressed rapidly but I also did exercises in the pool for 2-3hours 2-3 times a week in the year before surgery and I followed all the instructions and exercises 3-4times a day everyday after surgery for about 6weeks. Then I started walking. Each time I went out walking I set a further distance goal. Doing 7kms now. Still doing the original exercises everyday as well. Got kicked out of physio class for being too far advanced after only 2sessions. So how you progress is up to you. Slow and steady, and just a little bit more each time is my motto. 😂 Enjoy the process, the changes happen remarkably quickly once you get going.😊

Hi Dr Mike, You're welcome. I should have added that I was no twiggy the year before surgery I was 109kg so I was working to lose excess body mass as well as trying to strengthen my arthritic knee. So in part my excellent recovery was aided by both successes. Today I weigh 99kg.
I'm not a strong swimmer, so most of my pool exercises were like old fashioned calisthenics at my speed. I built up from 10reps to 100reps of each exercise which is why it took 2-3hours, but I was going at my own pace and no one was or had ever told me what to do. It was my Choice and I enjoyed it . If I wasn't enjoying it I did one of two things...keep going because it was good for me but drop the reps to 50, or occasionally I quit went for a soak in the spa pool for 5-10mins then went back to the exercises with a new mindset.
The major workout is not in the exercises for the body ... It in the exercise of the mind. Kind firm Self talk is crucial, you have to become your own cheerleading squad. A cheerleading squad of One can achieve just as much if not more than a huge team. ❤ To anyone who reads this and says oh but you don't know the troubles that I have.. I'd say this.... No I don't but you don't know mine either and I've had a lot of serious ones, major depression for 30years being just one of them. I've overcome that been free of it for two years but I could fall again at anytime so I've been told. Maybe, but not if I can help it. I'm going to fight for my good health everyday until I die.
My 66th birthday is in a couple of weeks and I'm not going to give up and die a sick weak fat person. I intend to be slim and healthy when I die. You can be too if you just let yourself. Fight, it's a good fight and you'll feel great as you do a little more each day. Nobody needs to run a marathon to heal slow and steady gets you there with less chance of injury.
Let me ask you....
Why does everyone have to rush.? Where are you hurrying to?
You can grow strong without lifting weights ...just using your own body as a weight, likewise you can strengthen your self control by making small changes to your thinking, so it follows that when you have an injury or a surgery small steady progressive changes every couple of days will improve your recovery far quicker than rushing to it and possibly setting it back by weeks or worse. I wish you all a slow and steady recovery. Look for ways to enjoy the process😂❤🎉

I have severe ( bone- on- bone) osteoarthritis in both knees, but worse on the left knee.
I'm 68 years old & played pickleball for years, even when I started feeling pain in my left knee.
Now the pain is really intense. Cortisone shots only lasted 9 or 10 days. Gel shots haven't even kicked in & it's been almost 8 weeks.
I'm considering getting knee replacements. With your clients who have osteoarthritis in both knees, was it more difficult for them to recover?

Sounds like you’re a very active person and I love to hear people finding new ways like pickleball to keep their exercise enjoyable! I’ve heard a lot of good things with the growth of the sport! Anyways many of my clients have dealt with similar experiences and came to a point where all conservative care was not providing enough relief. Surgery allowed them to get back to doing the things they loved again. And for those with osteoarthritis in both knees they usually get the most painful side done first and then later get the other one. By no means am I saying that it’s going to be a walk in the park. It will take 3-6 months to start feeling somewhat normal again and sometimes some people take up to 1 year. It’s part of the process but in the long run it could provide you with many more years of an active lifestyle without any pain or limitations! Hope that helps and I’m wishing you the best!
I’m three months past complete TKR in the left knee and two weeks into right knee TKR. Dr. Mike is absolutely correct in what he says. I want to add my two cents about what I’ve learned, which I think follows Dr. Mike.
1. Have someone you trust keep track of when and how much medication you take, because your mind will be foggy and your short term memory will be unreliable for awhile. My husband put together a spreadsheet on the computer. Keep ahead of the pain with regular meds. This is not a time to think you can survive pain better than other folks.
2. If you can’t sleep at all at night, rent an icing machine that can be pre-programmed to cycle through an on/off icing program. Or just at least get a gel cooling bag or bag of frozen peas on your leg. Remember to elevate your leg too. I think inflammation and pain explode at night, so talk to your doctor about added pain medications. Don’t push your leg with exercises too hard before evening and overnight. Timing of exercise makes a difference. I know from experience that it’s almost unbelievably excruciating at times, but not all the time, so remind yourself it will get better. Listen to music or very quiet level voices- no politics.
3. Exercise, but you don’t need to torture yourself, either. Keep experimenting to find out how much pain with exercise is reasonable, and get advice from your PT. With my first TKR, I had so much fear that I wasn’t pushing the ROM and quad exercises enough, that I overdid it and set my progress back. After I learned how to balance pain and exercise better, I started healing ahead of schedule without superhuman effort. The body needs to heal itself and your pain will be made worse by torturous exercise.
4. Remember you’re not alone!! Ask for pep talks or motivation from experts. My PT helped a lot with morale. It’s OK to cry, too.

Had TKR on my right knee in 2020, in the beginning of Covid. The therapist would come to the house but sent me a exercise schedule. Would have been better if he helped in person. Today? Zero pain. No limp. 80% bending, I can live with that.
Now the left knee is acting up, I’m looking to get gel injections first, hold off on surgery for as long as possible. Those with pain and suffering, I hope you all get better soon.

A quick update...I am just over 9 + weeks out now, and continue to be happy with my new knee... so much so that I am booked to have my 2nd knee done on February 29th...I am hopeful that everything I have learned from the first surgery will help me be fully prepared for the 2nd one, and that my recovery from that one will be just as smooth and equally speedy as the first time around...

The best advice I can give you is get your own bandages -they gave me ones you have to use tape with -I found big bandage ones that are tape included from CVS or Walgreens I got the 6x6 and used two just cut part of the end tape off one & put them together so much easier

I’ve had two Physio appointments already and I was 85 last Monday and 90 on Wednesday. It’s hard to bend from playing down position however I’ve been doing chair sitting up and down and letting my leg hang with gravity for 30 seconds and feel that bending like this is good, just not in laying position which I was beating myself up about. I’m going to take your advice and continue to walk with walker until I’m strong enough. You truly uplifted my spirits. I’m so worried about the time allotted for full motion. I heard one person mention it’s within two weeks and if you don’t get full bend and straight that ship has sailed. It scared me. I will continue doing what I can daily at home. Thank you.

Never compare! I did this, too. I'm 2 weeks post-op and have pushed myself to bend my knee to 119 degrees. My PT really had a heart to heart with me. He said, "That's great! But let's slow down." I am expecting far too much too soon. I've had to reevaluate my expectations and accept them. I've now slowed down, and I'm trying to compare myself to no one! Just go your pace, be diligent, and do what your PT tells you.
I played several sports and have had 19 major surgeries as a result. I am 63 years old and had a TKR 1 year ago. For some reason it is a competition with some people. I swear some these people will tell you they didn't need anesthesia. I see that with some of these comments. What this Dr. is telling you is spot on. For example, his comments regarding the use of a walker are right on the money. The PT does not want you to stop using the walker till you can walk normally WITH it and then the same for a cane. A friend of mine told me she stopped using the walker in the less than a week. I told my Dr. at the two week post-op visit this because I thought I shouldn't be using a walker still. He laughed and said, first she didn't have a total knee replacement, second if she did she is crawling around her kitchen or third she is full of it. So this is a quote from my Dr. he said: "The first two weeks after surgery you will hate me, weeks 3-6 you will hate me less, by the third month mark you will begin to see why you had it done." He was exactly right and it is the best my knee has felt in 8 years.

First two weeks were absolute hell. At 14 days it began to subside. I’m questioning my life decisions right now 😂. At some point I’ll have to have the other knee done 😢

However, at 3 months it really did turn and 1 plus years later my knee is better than it has been in 8 years (I needed a knee in my late fifties and made it to 62). Hang in there and don't let anyone tell you how you should feel. The Dr. and the PT are the voices you want to listen to. Take care!

I’m two weeks post op TKR and already at 125 degrees on my knee bend. It will also lay flat. It’s still tight but working it out. I’ve always done more than the expected me too in PT from day one. I don’t go for long walks but I do get up at least every hour and walk, even outside weather permitting. I have a bicycle that sets in the floor and I use it before and after all my PT. Started walking with a Cain at 9 days post op. My swelling is very minimal now. I still ice a lot and work the stiffness out afterwards. I have no more pain or what little I have had is less that before surgery when I was bone on bone. Stay strong people and good luck.

And Just what exactly is your age? There are different expectations based on the age of the patient as well as the difference in Every TKR surgery. Some patients will recover faster, some slower. Go at your OWN pace!

Doing both of mine 4/22/24. I'm learning all I can. I'm pretty damn nervous at this point. I just hope to get all I can from this surgery. I've been suffering for 7 years probably. I think I would be in a wheelchair in a year. I'm 56 and my quality of life is nearly 0. Well, maybe not that bad but it's bad.
I am 3 weeks into my knee replacement surgery and everybody needs to listen to the words that you just said because it is the honest to goodness truth and if you do not exercise you know do all the stretches your knee will swell up and it the more you have swelling the harder it is to get out and do those exercises

I am just over three weeks after my second knee replacement. Listen to this man, his advice is gold. Do not baby yourself or neglect your rehab exercises.
I also highly recommend you rent or buy an icing machine. I purchased one before my first TKR, knowing I would be doing the other knee. It was cheaper to buy it than rent it twice. Do not skimp on this to save a few dollars. Your knee will thank you.

I am 6 weeks out, from my TKR. I think everyone should listen to this video. I am very fortunate that my therapist told me every one of these except keeping my knee straight. I already knew to keep my knee straight from the ladies in my swim class. I worked on keeping my knee straight as soon as I got home from the hospital on day 2. The walker I used for 2.5 weeks because it hurt my shoulders, and arm. I used 2 canes, and my therapist was very impressed of how straight I was able to walk.

I sleep in a recliner.. with an ice machine. I sleep pretty well until I need a pain med but then go right back to sleep after that.
@@kimparigoris1472 It is similar for me. I spend most of my time in the recliner except when doing my PT. I can only sleep for about 1 1/2 hr at a time so far.
I haven’t been able to sleep for years properly with my knee pain anyway. What’s a few more months?!

I had my right knee surgery 2 years ago. My approach was not to add stress to myself and allowed it to recover properly. Up to this stage I still could not understand the need of doing physio 3 days after surgery, when my knee strength had not revived. I attended regular physio but ignored doing it on my own at home. This approach helped me personally because my knee did not develop swelling. Just be gentle to the knee and to myself. I walked slowly using u-frame as it helped regaining my knee strength gradually. I slept by keeping my knee straight upward with supporting round pillow behind my knee underneath. My recovery journey was not a breeze but I was glad that be gentle to myself lead to excellent total revovery after 1 year. Dont push yourselves people. You will hurt your knee if u fall. If u need to use a wheel-chair, please do. As I did, especially to shower et'al. Ignore people who tried to push your limit by dictating the dos and donts. U are your own doctor during your recovery period.

This is great information to give to people. I learned all of this from my at-home PT the first 2 weeks after both my knee surgeries. I have fibromyalgia and reactive arthritis, so the nerve block only lasted 8 hours and then I had to have an upgrade in pain meds and my at home PT taught me how to lessen the pain with certain movements. Being used to PT, I followed what she said as much as I could and told her where my weaknesses were so she could teach me how to mitigate those. She had me at almost full range by the end of her stint and my outpatient PT got me the rest of the way. Listen to your PT people, tell them your struggles, and follow what they say - it's the best way to advance at a normal to slightly better rate which will help you in the long run. I didn't do the exercises as much as I was supposed to because of my chronic issues but when I did them, I was faithful. I did a lot more exercises as I was stationary which kept everything limber.

Best thing I ever done! I had my left knee done almost 2 years ago. I couldn’t sleep for a couple of months, I had stiffness, tightness, but I did the right thing it will get better. Getting ready to have my right knee done in July.

First don't compare yourself to others. The longer you go without bending the harder it will be. I had my tka 10 weeks ago. I agree the tightness, numbing on out side of knee and burning stinging on inside of knee is awful . It does get better. Remodeling takes a long time. Lots of structural changes b4 surgery and now there are more structural changed. Getting the knee alignment back to normal. Don't give up keep up with your exercises. My new knee is now starting to feel like it belongs to me. Your body is saying what is this foreign object. You will slowly begin to accommodate to the new knee.
Hi, all. I had TKR 9 days ago. 3rd p/t was today. 106° bend. First p/t was 50°. Walking with just a cane but I can walk without it. I think I'm progressing OK. No problem at all sleeping. In fact, I'm sleeping better than I have in 3 yrs. I'm 75 and everyone tells me I'm doing great. Going to attempt riding my recumbent bike next week. Had my knee replacement 3 wks ago. I can stretch out the leg but bending is another thing. I'm probably around 80 degrees at the moment..there's so many videos on here I'm not sure if I'm coming or going.people saying their walking after a wk and total bending of the knee..I'll do it at my pace.exercise as i should but I'm not going to put myself through hell..it's bad enough as it is..swollen,tight numb electric pins and needles feeling..oh and no sleep..wish I'd never had this done 😢..
5 months later and still numb on outer side of knee.nerve pain up the total of my leg along with hip pain still.still not able to sleep on my side due to the pain..walking is still on the slow side..as for knee bending I still can't fully bend it..wasn't this bad before the the operation so definitely not having the other done..no chance.
Just a quick question thrown out there..anyone else suffering from total leg ache..my knee replacement leg is aching 24/7..even when I try to sleep I can feel it aching and going numb

I had my right knee done February 20,23, and my left knee done October 9. My recoveries were night and day different. I will say, NO regrets about doing both in the same year. First time was tough because I had been so sedentary due to constant severe pain and restricted mobility for so long. PT and my physical therapist were life savers! Encouraging and great directions for self care and recovery strategies. Second time I was much more quickly able to bend etc, but holy crow! My legs are SORE. My PT pointed out that my gait is completely different than pre first surgery. My legs are straight and I no longer limp/ waddle due to the pain. Dr Mike great advice in this video!

Had my left knee replacement 1.5 weeks ago.
Right knee was replaced May 2021. Conformis replacement. I'm 68 now. Still working. New construction plumbing.
First knee is awesome. I still play senior and league softball. Have played probably 60 games on right one.
So far I've reached 123 degrees on bending, still about 2 degrees on straightening. Therapist been coming to me last 1.5 weeks 3 daysxa week. Start going out next week for theraphy. Be off work 3 months because of work that i do. Climbing ladders, bending knees constantly. Zero meds for last 3 days for pain.
Do all my exercises 3 times a day. Ride a arydyne bike for 15 minutes after exercises. Then ice. Can't stop or you'll go backwards. Did that on first surgery. Lost 11 degrees in less than 2 days. Have like 130 on it now. Be safe everyone and good luck.

Don’t push it to hard, baby steps only. I pushed to hard and think I caused more scare tissue.. sore still especially on the outside of my Rt knee. Constant swelling.

Listen to this therapist! Mine replaced 7 years ago, therapy is hard and he knows what he is talking about! He mentions a wedge, Amazon has a wedge pillow that is awesome! Skip the pillows, get the wedge! Millard leg elevation wedge and another thing that helped me was the leg glide. Good luck!!

Dr. Mike, good stuff. To me, key is regular therapy at home or with therapist. Honestly, I would have never gotten to 115degrees after 6 weeks within my therapist. They push you which is needed at this painful time. At home, most of us are to wimpy to push us hard. Good luck.

Had surgery 6 weeks ago, doing ok. I think this might be the best video I’ve seen. Overdid the walking way too early and paying for. I have never gotten a consistent handle on the pain. Again great video and will be better prepared for my second knee.

I have had two knee replacements, my surgeon did not start me on PT until two weeks after my surgery. I did at home exercises and after two weeks I was ready for my PT, which went really really well.

I’m 3 weeks Post Surgery, and doing pretty well. I wasn’t really keen on going out a lot in public with my walker, but about 5 days post surgery, I was frustrated by the nice sunny days, and told my wife let’s head to the store. I found that pushing a shopping cart was a great clandestine substitute for my walker, and was able to put the time in working on my gait. I’ve been hitting the stores about 3 times a week for this purpose. I’m just about right there with ROM, but still have a ways to go to relieve the tension around the joint.

Very good advice… when I had my knee replaced I had to do therapy twice a day for twelve weeks… a lot of work but worth it… I have no pain and no limitations…. I had to work real hard to get that zero extension….
My knee got better and better even after 1 1/2 years later… I still have to keep my muscles strong by exercising.
Please always follow your Physical Therapist instructions….

Hi, I started walking in the house without the walker at the end of my first week. I noticed that I was not using the leg right. There was no bend in it. I didn't like the limp, so I grabbed the walker again, and came to RUclips. Thank you for this post
I was doing things wrong. From now on I'm going to do things better. It doesn't make any sense to experience this much pain and not try my best to never have to do this again

I’m 3 weeks in and I’m glad I turned in to your video not my therapist isn’t doing a good job with me. I learned a few things that I didn’t know concerning a walker which I went 7 days without my walker because the delivery for whatever reason never got it to me. Then when I finally got it I really didn’t need it

I’m 4 weeks PO today. ROM is “only 1-80” but 4 days prior, my numbers were “only 2-75”. The PA-C in my surgeon’s office was alarmed when I said 80! Her response, “OK. You can catch up though. You’ll need to work very hard, doing all your exercises every day. Do you think you can do that?”’ Ummm. Yeah, you mean like I HAVE BEEN DOING all along? Yeah, I can KEEP doing that! It was so discouraging. Always nice to see the reminders that our bodies all respond and heal differently. A week ago, I said, “Ive never worked to hard for so little in return.” I guess I’m learning patience with myself if nothing else.

This was absolutely the best advice I've read regarding what to avoid after knee replacement. These advices or admonitions are not available to read about in Sweden (in Swedish). 17 days ago I had surgery and have been doing great, now I'm cutting back on my pain relievers and the anti-inflammatory drugs but yesterday my walk with two crutches was too far. I am meticulous and follow my training program daily and I have also walked daily and increased from 2x400 meters to 2x3.5 km daily with crutches at a fast pace. yesterday I suffered a setback which means that I now have severe pain in my knee and can hardly move forward. I understand now that I have to calm down.

All good advice. I was fortunate to have an excellent physical therapist who helped me stay on track. That is a very important part of recovery. Perhaps the most important thing is to have an advocate to keep track of everything. I thought that I could easily do this and I discovered that my memory was nowhere near as good as I thought it was. Thankfully my lovely bride was keeping me in line.

I got my surgery on Monday. Today is Saturday and I’ve done a lot of things wrong, but I couldn’t help some of the things. My caregivers were in Northwestern, North Carolina and couldn’t get to me because of the flooding and then my husband came down with a really bad back and leg and he couldn’t help that just said everything back For me I’ve been in a lot of pain today. I haven’t wanted to take the oxycodone because it was making me crazy but I had to take it today because I think I overdid my physical therapy yesterday and I was as well as not sleeping last night or the night before, but I did take a nice nap, this is brutal but I do appreciate his tips and I will continue using my walker and I’m glad I heard this because I wasn’t using it as much as I did a day or so ago God bless all of you!

Excellent Advice!! I’m SO glad I found your channel! Just had left knee replaced today - all out patient - no pain pump- just oxy and Tylenol. I’m ok today but know the pain will start tomorrow as the surgical anesthesia wears off. I was elevating with pillows under my knee/ will change that to a wedge - I fear I’m the type to overdo and try to rush the healing process with too much walking- too soon. I’ll slow down and focus on good form.

Great reminders. LTKR on 30/12/23 . I had a 15% valgus that is reduced down to 6%. It’s been a shock how impactful this is on my pro-perception. I’ve walked with a pronounced limp for a decade. I’m being mindful of how I manage my recovery as I want to be as balanced and smooth as I can be . Have been doing prehab for a full year in preparation for this.!feels like I’m learning to walk again.! Have good ROM. Thanks Dr Mike for your informative videos .

I labeled alarms on each medication to take at what time every day and night. That was a real save because you need take the meds regularly and then know when to take them as you're tapering off pain meds. Had 2 knee replacements 7 months apart. Worked both times. My sister came to help assist me and coach me too.
I really got tripped up in comparisons. When u heard some people got both knees at once and i was struggling so much with one, I felt so badly about myself. But all i could do is my best. Poco a poco, little by little, and I've made progress.

A heating pad on the muscles around the knee, but not on the knee, loosens the muscles and relieves pain. Do the front and back of the leg.

I had both replaced five weeks apart. The biggest issue was sleeping. 1-2 hours then 3-4 hours awake. My legs became very ache and painful at night. This lasted a solid 8 weeks before the sleeping improved. Pain medication didn't seem do much, and I was taking as directed. Glad it's all behind me.
My mrs had surgery right leg on 13/03/24 and left leg on 24/03/24 . Now she is feel better and doing her work. Thank you dr for your information and advice. Thanks lot

I am almost 6 months post op. There is a lot of information out there whether by other surgery patients or by medical people. I must say that I wish I had seen your video before surgery as I think it would have made my immediate post op recovery better. Several of your points were things I should have done or not done at the time. Your presentation is clear and supported by your knowledge. I can see where I went wrong when I had a bit of a set back for about a week. Things finally progressed and I can walk easily and have gotten my life back! It all just takes time and we need to be patient. Thanks for putting this information out there to help others as they head into surgery.

I had TKR (Osseti cementless) 12 days ago. After a week the doctors office told me to do three 10 minute walks a day in addition to my twice daily exercises and 5-10 minutes of stationary bike. I’m fairly young (40) and know my body well from years of consistent hard training. I did three 10 minute walks the first day it was recommended and knew immediately after I needed to pull back. Took the next day much slower and have been building from there.
Man, this sh*t is painful though. I was only given a week’s worth of narcotics and now I wake up in the middle of the night in extreme pain.

I had both knees replaced. He gave good advice. Bending exercises are important. My doctor ordered a machine I strapped my leg to and it bent my knee. Every few days. PT upped the bend degree and reps. That was quite helpful.

After having both knees replaced in the last 15 months I totally agree with everything Dr Mike stated. I made mistake #6, but compared my progress with left knee to the quicker and less painful recovery and rehab of the right knee. Don’t do that. It was frustrating and depressing. Now, 4 months post 2nd knee surgery, they are pretty similar in strength and mobility,etc. I cannot stress Physical Therapy enough, it makes a world of difference in your recovery. Baby steps are still progress! ❤

I'm on my 9th day of TKR on my left knee. I have the best home support. My wife is great charting my meds with I stopped taking pain meds after 3 days. My in home PT is so good and the pre-opt and surgery team i had at KP were the greatest. Dr. Mike this video is great. Do the stretching exercises and use that walker. My post-op is in 7 days and I plan on walking with a cane for that visit.

Thanks for the excellent video. I am 2 weeks post surgery and doing well. I would suggest a 7th mistake is not enough icing. I applied a lot of ice treatment during waking hours on the first 3 days followed by ice as needed and post exercise. I found the trick to recovery is to reduce swelling with ice but also wean to less ice treatment over time. Ice reduces swelling and no ice promotes healing. It is a fine balance that is individual and varied.

That’s a major issue! I’m sick of all those people bragging about taking only Tylenol after a few days, and being back on the golf course in a couple weeks. Are they crazy, delusional or had a different surgery!?!
@@user-wk1mw9nj3i76 I had a relative who I have not seen in a long time that told me he never took the first pain pill after he got home. He said it was sore but no pain. I'm sorry, but I find that difficult to believe. He didn't even know what I meant when I told him I had TKR. I think he must have had something else done to the knee. Maybe a meniscus repair. I have never heard anyone say they did not have pain. For me the first 2 weeks were brutal.

Today is my 24th day after TKR on my right knee. Thank you for your video. Very informative and very helpful. I am going through a bit of depression, mainly due to the inability to do the things I did before, lack of appetite and the pain that drives me crazy. I notice that I am sleeping a lot, but it’s not always restful sleep. I have enjoyed your video and can see what I am doing wrong. Thank you.

Many people feel down because they can’t do the things that they use to do now. But it’s all temporary! Two weeks out from TKR and so glad I came across this post. I’m making all the mistakes! Got rid of the walker too soon (vanity) and will bring it back into practice as my walking gait has deteriorated. Major stiffness in knee from working too hard trying to “fix this fast”. Need to do more on range of motion. I do have an excellent support system and just need to check my ego so I can make the most out of this surgery.

The pain after the knee replacement was excruciating for me. They were so chintzy with the pain medications, which made me angry. This is what pain meds are for, post-surgery. The meds they gave me didn’t touch the pain. If you’re unable to sleep due to pain, one thing I did was get a tens unit and put the leads on my knee/legs. This tens unit sensation distracted my brain from the acute pain so that I could fall asleep. The acute post-op pain lasted for exactly 30 days. After that, it was smooth sailing.

As a home physical therapist, I really enjoyed your video. However, after 46 years of practicing physical therapy and doing a lot of total knee rehab, I disagree with your third mistake. Too often I see my patients who were instructed to not put anything under their knee so that their muscles will not tighten up. This increases their pain dramatically. I always suggest that the patients support their knee and have about 10 to 15° of flexion. This way they're not having as much pain and it reduces the swelling as well. Another thing that I emphasize is more elevation. The less swelling in the knee the faster the range of motion will return. Overall, a nice presentation. By the way, I always emphasize doing some active knee extension exercises so the muscles do not contracture
@PaulRich-oy4xh hey thanks for stopping by to watch and for taking the time to share your expertise. It’s always nice to hear from fellow PTs and I respect your input. Let me clarify that when I instruct my patients to remove any support behind the knee I also have them do this gradually just in case they do experience sudden increase in pain. This might look like spending 3-5 minutes at a time initially and then building up to 20 minutes without support if they can tolerate it. I’ve seen too many times where people get comfortable with that 10-15 degree bend in their knee which leads to stiffness behind the knee limiting their ability to fully extend their knee. And I agree with you that use of the wedge with elevation can help with not only the pain but swelling control! These are great points that you bring up! Thank you for sharing!
I had both of mine done at the same time it’s been over a year. I finally feel like there’s some real progress. My work demands a lot on my feet and my lifestyle does also hang in there. I think it does finally feel better.

Great advice. I did everything that you went over and my knee is so much better now. Had my knee replacement surgery just over 14 weeks now. First two weeks were not bad. Second to sixth weeks were the worst. Two to three hours of severe pain every night. Was not able to sleep for three weeks even with the pain medication. During the day was fine. Don't know why the pain gets so bad at night. Finally off of the pain medication. Just pain when I first stand up. Just waiting for that to go away. If not, back to see the doctor. It will be some time before I can forget the pain. Will stick to corotisone for my other knee for now.

I'm 6 days out from total knee replacement. Luckily I had really good range of motion before the surgery (I'm physically active and bike a lot). But my leg extension was not good, and straightening has been painful. I can't believe how tight and painful the knee is every morning! Seems like it gets more painful by the day. One of the hardest things for me to deal with is being stuck on the bed or sofa indoors for weeks to come. Not looking forward to months of this pain and discomfort. I can see why people get depressed.😢

Consider progress a week and possibly a month at a time. I just had a revision, partial to total knee replacement. Nothing can compare you for this experience, it’s a lifetime of pain in a month. I was discharged 16 hours after my revision and the pain was horrendous. I should have had an epidural, continuous nerve block, PCA Dilaudid, or IV narcotics for the first two days. They need to a better job with pain management and stop looking at cutting costs by tossing people out the door.

I'm now 8 days after tkr surgery, agree 100 % with keeping up with the pain meds which will in turn allow you to complete your rehab so much easier. Will it hurt, you better believe it will. Personally I had pretty good range in the knee before leaving hospital at 3days post op. Around 80° the leg has tightened since then but physio not concerned, just keep exercises going and keep pushing it a little more every day. Personally I have over the top of the thigh to the point it feels like I'm tearing at stitches when I get past 80°. Will run that one past the physio on her next visit on Monday. Plus the IT band is very tight but that's just me.

Yes thank you the pins & needles pain is persistent after 3 months of surgery despite following 95%of what you said to do or not to do... How to overcome..i am praying & it will help & will continue with exercise & medicines for some more time ok 🙌

Thanks Dr Mike! I actually looked at one of your other videos that showed different exercises and this one popped up in my feed. I’m four weeks post surgery and am progressing good. I’m walking about 30 minutes twice a day and doing my range of motion exercises about three times a day. But my problem was putting the pillow under my knee…I was in so much pain that first week…I just had to. I am still guilty of it at night. But during the day I try to concentrate on laying the leg flat and putting a wedge under the ankle and tolerating it for about five minutes take it out and then repeat. But that is something that I am guilty of from day one…

See myself doing several of what the doctor mentioned. My physical therapist often asks me do I have a cane? I only have my Walker which I rely on. Now that i've seen this video I want to take my time but continue my journey. And it is giving me encouragement not to do some of the things that I have been doing. In order to get better and be able to walk without help.

I am quite capable of managing my pain relief, just as well as I don’t have much choice as I don’t have anyone who can help me. Not everyone does, and not having anyone to help should not discourage people from having necessary surgery.

I am sorry you had no support system. I live by myself but thankfully family and friends have been able to step in to fill in the gaps. It definitely made a difference having someone my first 10 days. I am now 14 days put.
Also if insurance allows, and no support system, sometimes inpatient rehab for the first week or two can be an option. I researched and was prepared for that, until my sisters surprised me and came in from out of town to help, since their schedules allowed

I’m 4 weeks out from TKR. Thank you for the advise about the walker. I try the cane for practice occasionally but I do feel I have a better gait with my walker for now
Also I pushed too hard on the walking one day and did have a set back. So now I walk in smaller sessions but more consistently

Day 3 post TLKR yesterday and I walked 2300 steps over about 6 sessions. It was too much as last night needed breakthrough pain meds x 2 to get some rest.
I am 5 weeks RTK post-op and up walking with my Rollator. I am able to straighten my knee, however my flexion is about 65-70 degrees. Doing home exercises and out patient PT. The swelling is still prominent. Elevating and icing but swelling still preventing from gaining flexion. Frustrated.
But am moving along slowly but surely. Thanks for the video.
