I was diagnosed years ago with sesamoiditis and my podiatrist never mentioned tight calf muscles causing issues. He just suggested I get injections or surgery to remove them. He told me I didn't need them. Needless to say I never went back. I just use foot pads when inflamed. Knowing stretching my calf muscles can help gives me hope!! Will give it a try!!

Been suffering with sesamoiditis for over a year now. Originally got it from running too much but it didn't fully heal even after I reduced running and started cycling. Got an mri which confirmed sesamoiditis. Stopped running entirely and also massively reduced cycling. The foot didnt feel any better 6 months later so got another mri scan. Apparently the foot had managed to get even worse and is now borderline fracture. Currently wearing a boot for 2 months. Genuinely one of the most frustrating injuries there are. Even small walks are painful.

I have it on one side. Probalby from hiking. Maybe also for a short oppoitse leg 0.5 inch discrepancy. I don't want to stop hiking. stopped using sketchers. Have shoe inserts from my podiatrist. What I found that helps is a different shoe. For me, the Red Wing Classic MOC. It's flat and sturdy, and doesn't need inserts, but as soon as I put the sketchers back on, with the perscription inserts, pain agian, even in a half day.

Thank you for this video! I broke one of my left sesamoids and didn’t even know it! There wasn’t much pain but it was sore enough for me to get it checked out. The podiatrist has me “floating the bone” with a dancers pad, but I’m going to tape my toe too. He wasn’t specific about what sort of shoes or exercises I can do, so I’m using common sense. Your information has been very helpful.

Unfortunately, petechiae can mean fracture, or it can just mean bruising. They are definitely a sign of injury, but even no bruising can still show a fracture on an X-ray. There is no perfect way to diagnose it without more advanced imaging.

Personally I would double check this with an X-ray and an ultrasound to check for fracture/injury or bone bruising, then I would, get a premade insole with some metatarsal padding to attempt to offload the area and assess any decrease in symptoms/pain. I would also perform a biomechanical exam, frequently there is increased pressure due to tight back/hamstring/calf that worsens the issue. 😊🙏

I’ve had sesamoiditis in the fibular sesamoid of my left foot since 2019. In September 2022 it evolved into a stress fracture. After 6 weeks in an orthopedic boot and plenty of physio, orthotics and better footwear, I was back to 80% of my normal capacity. Unfortunately I developed tendinitis in my right foot over this summer and that has caused a flare-up of the inflammation… I’m considering surgery and will see my surgeon on 1 September. Last year the surgeon talked about screw fixation, but I’m afraid that the problem may reoccur when the screw is taken out as my patience has already been tested for a long time with these multiple injuries. At this stage, would you recommend going for a removal as opposed to screw fixation? Is there a way to replace the bone with something else to help stabilize the joint? Thanks.

Hi, I broke in 2 parts my external sesamoid in June.
Walked with a boot during 3/4 weeks And moved only when necessary,
Then I had custom orthotics with soft material under the sesamoide
It’s 17 October and I still use the orthotics but I fell a little pain at the middle of the feet where I put all the pressure to save the sesamoide
I try to be healthy 100% I don’t smoke drink a lot of water, eating fibre so it will fix faster
Last month I went to a surgeon, for the first time in 3 months I moved my big toe, I always had pain but he stretched it whitout hesitating, he told me it’s very rough and I have pain because it’s not moving enough
Today it’s been 4 months and a half and I fell like a can walk correctly but I’m still scared a lot to go skate again, I’m scared that It will break again.
I can move my toe how I want, no longer big, my toe and sesamoide area is kinda like my other feet, I don’t fell pain, I fell something weird but that must be because I changed my way of walking ?
    Not sure about this but
Not sure if I should go to normal shoes and walk like nothing or if I still should wait...
Any advice on my situation ? It would be very helpful

Christian Garcia I have had a fractured tibial sesamoid for 2.5 years and it doesn’t hurt all the time. I didn’t realize it was actually broken for the first year. The MRI suspected it but CT scan had to confirm it. It’s like a small chip in the front. This is a really complicated injury. Everyone’s story is different but it’s seems like for every one healing from a sesamoid bone fractures takes years and/or you get the surgery. I’m going non surgical because mine is not that bad. The shoes i found that work best are New Balance Fresh Foam More and Hoka Gaviotas or Bondis, with orthotics.

Non-surgical I mean. And yes mine has very slowly improved after wearing a boot and hardly walking for months, but I wore it for too long. I am trying to walk more and more in shoes and orthotics. The area is always irritated and has to be kept from bending. I’ve accepted it as probably a lifelong thing to deal with. Dr Richard Blake has a blog with lots of sesamoid injury advice. Also there’s a sesamoid injuries group on FB.

Any recommendations for zero drop or close to zero drop shoes which will help me heal my sesamoid? I am also currently waiting to be fitted for custom orthotics.

I fell down the escalator on Vienna airport on February 21st 2020, I went to the doctor once in my country in march and I have chondromalancy II in my left knee (which bleed when I fell). Then pandemic hit, and everything closed and once it opened I am to scare to go to the doctor, but while on lockdown the sesamoids of my right foot started hurting, I am guessing I hurt them when I fell the escalator though I cannot be really sure, my sesamoids only hurt at night when resting, not when walking, and if it touch them to much, like massaging, I feel in my big foot like tickling like when you sit in the same position for too long and your foot feels like it has ants walking on it. Can it be treated? Did I break it? If I go to the doctor when there is a vaccine (hopefully in March) will it be too late to do something? Is it too late now, anything you say can be helpful, thank you very much!

Never did any physical activities because a week ago I was just rushed to the ER because of some issues with my neck spine, the xray says its really straight thats why i’m having issues swallowing. I landed on a pointy rock trail running right onto my sesamoid bone and injured it. It took about a week or two before I could walk / run again. Since then though I feel like the sesamoid bone sticks out and is a lot more noticeable by looks and feel on my left foot that the injury happened on than my other foot. There is no pain at all but its as if the sesamoid bone is bigger now on the injured foot, can the injury have caused the bone to thicken/grow or something I just don't get it.

    • @DrTomBiernacki

In challenging cases we usually see a high arch foot with a much tighter ankle leg leaning all of the bodyweight into the big toe joint. This means when the fracture improves, it just starts hurting right back up again. Generally there are more advanced ways to stretch the calf and leg out, I prefer daily stretching and then potentially using casting or procedure if indicated.

I ran down the stairs, I was wearing very thin-soled boots, I felt immediate pain as my foot hit the floor.
The pain was gone, after 2 minutes it started again as I was walking on a flat surface.
This happened a week ago, and last night I felt that my foot was swollen and in a lot of pain (I've never hurt this much) and now I'm worried that it might have been more serious than I anticipated.
