This is the form I-134 Declaration of Financial Support. It should be filled-out or submitted by or/with the US Citizen petitioner or lawful Permanent Resident petitioner. I made a sample on how my husband filled out all the pages before he submitted it to USCIS and sent a copy to me for my visa interview.. This is based on our own experience and it might be a help for your reference as well.
