I would like to help with two questions in the form of DS-160. 1- HAVE YOU EVER BEEN ISSUED A U.S Visa 2- HAVE YOU EVER BEEN BEEN Refused A U.S. Visa or Been Refused Admiysion to the United States, Or Withdrawn Your Application for Admiysion at The Port Entry. How should I answer these two questions as someone who has been eliminated from Greencard and has never applied for an American visa before.
So if you select no to have you made any specific travel plans i.e. I want to make sure I can get a visa before booking my trip, what should I put in the Address Where You Will Stay in the U.S.?

I have put all the details of my present employer in the present employer section. Then for the previous employer it asks the information for all the previous employers in the last 5 years. You pay the visa fees on link called "USTRAVELDOCS".
and to pay so, you will be needing this DS 160 form application ID as well.
